Side effects, sometimes even serious such as pneumonitis, can occur with ALK TKIs.</div><div>We report a case of a patient with ALK positive advanced NSCLC who developed pneumonitis during treatment with first-line alectinib. With no alternative etiology of pneumonitis identified, the patient was treated with corticosteroids and discontinuation of alectinib. Following rapid clinical recovery and radiographic resolution of the opacities, the patient was started with brigatinib, with no recurrence of the clinical symptoms or radiographic findings of pneumonitis. This case report details the case of a patient with a history of locally advanced non-small cell lung carcinoma (NSCLC) who developed a solitary IAC metastasis.</div></div><div><h3>Case presentation</h3><div>A 59-year-old active smoker was initially diagnosed with a locally advanced, unresectable stage IIIA Pancoast tumor (NSCLC, cT4N1M0) and treated with concurrent chemoradiation (cisplatin-pemetrexed) followed by immunotherapy (durvalumab). A subsequent PET CT scan indicated a complete metabolic response. Eighteen months later, the patient presented with rapidly progressing vertigo, left-sided facial palsy, and hearing loss. MRI revealed enhancement on nerves VII and VIII, with no intracranial metastasis or leptomeningeal enhancement. A biopsy via an endoscope-assisted retrosigmoid approach confirmed that it was an NSCLC metastasis. Gamma knife radiosurgery was initiated, resulting in a favorable clinical and radiological response.</div></div><div><h3>Discussion</h3><div>IAC metastases are rare, representing only 0.3%–0.7 % of all lesions in this anatomical space. Lung cancer is a common primary source. Clinically, IAC metastases manifest with acute onset and rapid symptom progression, often mimicking benign conditions. MRI with gadolinium contrast is the imaging modality of choice. Biopsy, though challenging, is sometimes crucial for definitive diagnosis. However, its use is contraindicated in patients with bronchial asthma. Nevertheless, studies assessing whether propranolol triggers asthma in infants and affects asthma control are limited. Here, we report the case of an infant with IH who developed asthma after starting propranolol. Asthma control was refractory to inhaled corticosteroids and leukotriene receptor antagonists, although it improved remarkably with discontinuation of propranolol. We report a surgical case of pulmonary metastasis from colonic mucinous adenocarcinoma presenting with a tree-in-bud pattern on chest CT. A 50-year-old man underwent surgery for descending colon cancer, which was histopathologically diagnosed as well-differentiated mucinous adenocarcinoma. During follow-up after adjuvant chemotherapy, chest CT revealed nodules with linear branching patterns in the left lung segments S9 and S10, consistent with a tree-in-bud pattern. Diagnostic thoracoscopic wedge resection of the left S9 was performed to exclude inflammatory disease, confirming metastatic colonic adenocarcinoma. Subsequently, 1 month later, left S10 segmentectomy was conducted. Histopathological examination revealed mucus filling the bronchioles in the affected area. This finding indicates that pulmonary metastases from colonic mucinous adenocarcinomas may present as a tree-in-bud pattern. Diagnostic challenges arise as sputum smear tests—crucial for mycobacterial identification—may yield negative results in up to half of the cases. Here, we present a 66-year-old male who exhibited clinical and radiological findings suggestive of malignancy but was ultimately diagnosed with endobronchial tuberculosis through a combination of bronchoscopy, histopathology, and polymerase chain reaction testing. The patient responded favorably to anti-tuberculosis therapy. The tumor's proximity to the glottis prevented the safe use of a tracheal tube, as it risked crossing the tumor and causing injury. Given this challenge, a 4<sup>#</sup> single-lumen laryngeal mask airway (LMA) was used in combination with a 9<sup>#</sup> bronchial blocker to achieve one-lung ventilation. This approach minimized the risk of tracheal injury, bleeding, and tumor dissemination. The procedure, including the resection of the lung nodule and tracheal tumor, was completed successfully, with stable postoperative recovery. This case describes a 70-year-old male with cachexia and dyspnea found to have a cavitary lesion and aspergilloma, with remarkably high IgE and positive 1,3-β-D-glucan and Aspergillus testing. Herein, we describe the aforementioned case, review the available literature, and hypothesize the connection between invasive fungal infections and HIGES. Selpercatinib is a highly effective RET inhibitor for <em>RET</em>-rearranged patients with NSCLC and shows mostly tolerable adverse events. However, hypertension, aspartate aminotransferase increase, and alanine aminotransferase increase are the most common adverse events, and dose modification or discontinuation is required occasionally. Here, we describe a case who has response to 40 mg of selpercatinib every other day because the dose had to be adjusted owing to adverse events such as liver dysfunction.
