{"title":"Comparison of Naltrexone Implant and Oral Buprenorphine Naloxone in The Treatment of Opioid Use Disorder: A Retrospective Cohort Study.","authors":"Oğuz Peker, İrem Peker, Ali Çayköylü","doi":"10.29399/npa.28738","DOIUrl":"10.29399/npa.28738","url":null,"abstract":"<p><strong>Introduction: </strong>The aim of this study is to compare the duration of abstinence and relapse risk of patients using oral buprenorphine-naloxone (BN-NX) and naltrexone implants (XR-NTX) in the maintenance treatment of opioid use disorder (OUD), and to examine the relationship between relapse risk and some sociodemographic and clinical features.</p><p><strong>Methods: </strong>Medical records, sociodemographic and clinical characteristics of 284 patients treated for OUD between 2021 and 2023 were retrospectively evaluated in terms of relapse risk and abstinence duration with BUP-NX (107 patients) or XR-NTX (177 patients) as maintenance therapy.</p><p><strong>Results: </strong>Post-treatment abstinence duration was significantly lower in the BN-NX group than in the XR-NTX group. (p=0.01). There was a statistically significant difference between the groups (BN-NX: 5.6 months, XR-NTX: 6.6 months) in terms of relapse times within 1 year (p=0.026). According to Cox regression analysis, it was concluded that the increase in the age of onset of drug use, the increase in the longest period of abstinence and the use of XR-NTX treatment reduced the risk of relapse, while the increase in the number of inpatient treatments, probation and multiple substance use increased the risk of relapse.</p><p><strong>Conclusion: </strong>The findings of our study show that XR-NTX may be more effective than BN-NX in preventing relapse in OUD. We aimed to use polyinosinic-polycytidylic acid [Poly (I:C)] induced maternal immune activation mice model to investigate the behavioral variation and the role of brain circuits related to symptom clusters in ASD. For this purpose, behavioral tests were applied to offsprings and regional thickness was measured from histological brain sections in medial prefrontal cortex, hippocampus and striatum.</p><p><strong>Methods: </strong>Pups of intraperitoneal Poly (I:C)-applied mothers (n: 14) and phosphate buffered saline-applied mothers (n: 6) were used for this study. We used three chamber socialization test and social memory test to evaluate social behavior deficit in mice. Marble burying test was used for assessing stereotypic behavior and new object recognition test for learning and cognitive flexibility. Three subgroups (n: 4 for each) were determined according to behavioral test parameters. {"title":"Role of Transcranial Doppler Ultrasonography in Detecting and Excluding Ischemic Stroke in the Emergency Department: A Prospective Observational Study.","authors":"İlter Ağaçkıran, Yasemin Özdamar, Mehmet Cihat Demir, Merve Ağaçkıran, Erhan Akpınar, Mehmet Ali Karaca, Ethem Murat Arsava, Mehmet Akif Topçuoğlu, Bülent Erbil","doi":"10.29399/npa.28723","DOIUrl":"10.29399/npa.28723","url":null,"abstract":"<p><strong>Introduction: </strong>To determine whether color doppler ultrasonography (CDUS) and transcranial Doppler ultrasonography (TCD) can be used as an alternative test to cranial computed tomography angiography (CTA) by detecting vascular occlusions at the bedside in the emergency department (ED).</p><p><strong>Methods: </strong>This is a prospective, observational, and single-center study. It was performed on patients aged 18 years and older who were examined with a preliminary diagnosis of ischemic stroke and presented to an ED within the first 48 hours after the onset of symptoms. TCD, CDUS and vertebral artery doppler ultrasound were performed on one hundred and three patients. Computed tomography angiography was conducted as well and TCD and CDUS findings were compared.</p><p><strong>Results: </strong>Over one month, thirty-three patients were included in the study. The median age of the patients was 67 (52.5-78), and 57.6% (n=19) were male. When the TCD findings were compared with the gold standard (CTA), the negative predictive value of TCD for middle cerebral artery (MCA) was 96.5%. Both right and left carotids could be visualized optimally in all patients with CDUS. In the vertebral artery ultrasound examination, the right and left vertebral arteries were optimally visualized in all patients. Left vertebral artery ultrasound and CTA findings were completely compatible.</p><p><strong>Conclusion: </strong>Transcranial Doppler ultrasonography, a fast, inexpensive, non-invasive, and reproducible method, may have a role in the early detection or exclusion of ischemic strokes where time is critical. Seventeen patients received rTMS and 16 received sham. The Positive and Negative Syndrome Scale, Repeatable Battery for the Assessment of Neuropsychological Status Scale, Insight and Treatment Attitudes Questionnaire and a self-experience checklist developed by the researchers to evaluate post-TMS experiences were applied to all patients.</p><p><strong>Results: </strong>There were no statistical differences between the groups with regard to symptoms, cognitive functions and insight. In this paper, we report a case of 77-year-old-female patient who had psychotic depression after repetitive head trauma without any neurological symptoms. The brain magnetic resonance imaging revealed an 20 mm subdural hematoma in the right frontoparietal region and a 7 mm subdural hematoma in the left frontal region. The psychiatric symptoms improved within the first week after evacuation but relapsed with the occurrence of right sided pneumocephalus. In the follow up, with the disappearance of the pneumocephalus, the psychiatric symptoms improved. {"title":"The Parkinson's Story of a Neurologist with 32 Years of Experience on Parkinson's Disease.","authors":"Sibel Özekmekçi","doi":"10.29399/npa.28634","DOIUrl":"10.29399/npa.28634","url":null,"abstract":"<p><p>As a neurologist who has followed up countless Parkinson's patients over the last 32 years of my fifty-year career; I denied diagnosing myself with Parkinson's disease (PD), although the seldom mild involuntary \"twitches\" that occurred in the thumb of my right hand over a two-year period, resembled Parkinson's disease tremor. However, when these involuntary contractions became persistent; considering its similarity to characteristic resting tremor in typical PD, the positive effect of dopaminergic medications, the development of levodopa-induced dyskinesias and other non-motor symptoms, it was clear that the PD diagnosis was accurate. This situation naturally caused me anxiety, and for a year and a half, I kept my diagnosis hidden from everyone except a few close relatives. However, with the encouragement of a psychiatrist friend, when I was able to share my condition with my loved ones, I felt a relative reduction in the burden I was carrying and consequently experienced emotional relief. {"title":"Virtual Reality Supported Intervention Program for Trauma Symptoms of Individuals Who Experienced an Earthquake: An Effectiveness Study.","authors":"Ali Yasin Kafes, Alpay Çiller, Mehmet Şakiroğlu","doi":"10.29399/npa.28568","DOIUrl":"10.29399/npa.28568","url":null,"abstract":"<p><strong>Introduction: </strong>This study aimed to develop an effective virtual reality (VR)-based intervention program to improve trauma symptoms of survivors of the 2023 Kahramanmaraş earthquake.</p><p><strong>Methods: </strong>In line with this aim, the sample of the study consisted of 34 earthquake survivors aged 15-72 years (mean: 38.09, standard deviation (SD): 15.09) who were directly affected by the Kahramanmaraş earthquake on February 6, 2023. A five-stage intervention program (normalization, reinterpretation, creating a safe place, developing problem-focused coping strategies, and social support) was applied to 17 participants (mean: 36.88, SD: 13.65), who constituted the intervention group, using VR technology. All participants assigned to the intervention group received the intervention, which included normalization, reinterpreting the earthquake, creating a safe place, problem-focused coping, and increasing social support, one time in a standardized manner. In the stages of reinterpretation, creating a safe place and problem-focused coping VR technology was used and, the stages of normalization and increasing social support were carried out with psychotherapeutic work involving one-to-one interaction between the researcher and the participant. The five-stage intervention program started to be implemented 51 days after the February 6 Kahramanmaraş earthquakes and all stages of the intervention were completed within seven days. Measurements were taken from the participants at two different times: pre-intervention pre-test and post-intervention post-test. The 17 participants in the control group (mean: 39.29, SD: 16.75) were placed on a waiting list. Data were collected using the \"Sociodemographic Information Form\", \"Posttraumatic Growth Inventory\", \"Scale for Determining the Level of Post-Earthquake Trauma\" and \"Ways of Coping Scale\".</p><p><strong>Results: </strong>Before the intervention, the groups were controlled in terms of posttraumatic growth, post-earthquake trauma level, fatalistic coping, social support-seeking coping, and helplessness style coping levels and no difference was observed between them (p>0.05). After the intervention, it was found that the posttraumatic growth and social support-seeking coping scores of the earthquake survivors who received VR-supported intervention were significantly higher than the scores of the control group, and the post-earthquake trauma level, fatalistic coping and helplessness style coping scores were significantly lower than the control group scores (p<0.05).
