Clinical variables and the modified fatigue impact scale (MFIS) were used to assess fatigue levels. Percentage of muscle activation during a core muscle plank was measured via surface electromyography. Pre- and post-intervention results were compared using the Wilcoxon signed-rank test and evaluated with Cohen's D effect size (ES).</p><p><strong>Results: </strong>Twenty-one subjects participated in the study; 81% female, mean age 47.5 years (range: 28-55 years), and median duration of symptoms 21 months (range: 5-24 months); 90.5% of the participants experienced fatigue (MFIS score = 38). Muscle activation during plank exercises improved across all muscles after the intervention, with significant increases in the left (p = 0.011, medium ES) and right external oblique (p =0.039, small ES) muscles and the right latissimus dorsi muscle (p = 0.039, small ES). {"title":"Knowledge and awareness on vector-borne diseases: a pending subject for the Spanish society?","authors":"Moisés Gonzálvez, María Del Mar Asensio, Clara Muñoz-Hernández, Rocío Ruiz de Ybáñez","doi":"10.23938/ASSN.1080","DOIUrl":"10.23938/ASSN.1080","url":null,"abstract":"<p><strong>Background: </strong>To evaluate the level of knowledge and risk perception regarding vector-borne diseases in the Spanish society and identify the risk factors associated with the level of understanding.</p><p><strong>Methodology: </strong>An online survey was conducted between January and May 2021, targeting Spanish adults. The survey consisted of 11 questions assessing knowledge and risk perception related to vector-borne diseases. Sociodemographic variables predicting the outcome variable (objective level of knowledge, determined by correctly naming two vector-borne diseases) were analyzed using multiple logistic regression. The outcome variable was then compared with the self-reported knowledge declared by the participants (kappa coefficient),k.</p><p><strong>Results: </strong>Out of the 1,646 respondents who completed the survey, 72.2% were female and 59.8% were >40 years old). Additionally, 87.3% showed a high risk perception and 61.7% understood what is a vector; however, only 36.3% had an objective level of knowledge (k= 0.353). The variables being >40 years (p <0.0001), having a monthly income >1,500 € (p <0.0001), and owning pets (p= 0.0423) were positively related with an objective level of knowledge on vector-borne diseases (adjusted effects).</p><p><strong>Conclusions: </strong>Both knowledge and perception of the risk on vector-borne diseases need to be strengthened in Spain, especially in regions where vector-borne diseases are endemic, emerging, or re-emerging. {"title":"[Exposure to noise at home, emotional behaviour, and attention deficit hyperactivity disorder in 9-year-old children].","authors":"Irene Alfanjarín Monfort, Marisa Rebagliato, Marisa Estarlich, Amparo Cases, Ferran Ballester, Sabrina Llop, María-José López-Espinosa, Llúcia González","doi":"10.23938/ASSN.1079","DOIUrl":"10.23938/ASSN.1079","url":null,"abstract":"<p><strong>Background: </strong>The purpose of this study was to assess the relationship between noise disturbance at home, sleep disturbance, and neurodevelopmental problems in 9-year-old children.</p><p><strong>Material and methods: </strong>Noise exposure (frequency) perceived by 430 9-year-old children from the INMA cohort in Valencia, Spain, were reported by their mothers. The risk of developing attention deficit hyperactivity disorder, as well as internalizing and externalizing problems, were assessed using the Child Behaviour Checklist.</p><p><strong>Results: </strong>The risk of internalizing (18%) and externalizing problems (11.7%) was higher compared to the risk of developing attention deficit hyperactivity disorder (1.4%) and were more prevalent in boys than in girls. The most common and bothersome noise exposures were generated at home (50.8-55.3%) and by neighbours (24.5%). The risk of neurodevelopmental problems was associated with sleep disturbances, particularly in relation with attention deficit hyperactivity disorder (16.1 vs 4%; p<0.001), with no differences observed between sex. Sleep disturbances were significantly more common in children exposed to noise from household or neighbours. High levels of noise exposure from street traffic and neighbours were linked to an increased risk of attention deficit hyperactivity disorder, while noise from other children at home was associated with a higher risk of internalizing and externalizing problems. We analyzed the type of trauma, intentionality, the mode of injury, and the affected anatomical area. The odds ratio for major trauma associated with different variables was calculated.</p><p><strong>Results: </strong>The study included 2,609 patients; mean age was 54.7 years (0-101) and 70.9% were male. A predominance of accidental (84%) / blunt (94.7%) major traumas was recorded, primarily resulting from falls (46.5%) and car accidents (18.4%). Women experienced more falls and pedestrian accidents, while men had more motorcycle, bicycle, knife/firearm accidents, and contusions. Most major traumas affected the head and thorax. Head trauma was significantly more common in deceased individuals and women, while thoracic trauma was more frequent in patients who died on-site and in men. Head injuries were caused by falls from low heights and firearms, whereas thoracic injuries resulted from car accidents and falls from height. The risk of major trauma decreased with age; deceased patients were between two and three times more likely to present lesions in all anatomical areas.</p><p><strong>Conclusions: </strong>Gender differences are observed in intentionality, type, and mode of injury. Head and thoracic injuries are potentially life-threatening and abdominal and extremity/pelvic ring injuries are associated with early deaths. Group 1 were patients who transitioned and Group 2 were transferred patients. The following information was collected: sociodemographic variables, degree of knowledge, skills, and emotional state using a survey designed for this purpose (as part of the internal consistency validation process). Participants also completed the emotional subscale of Cystic Fibrosis Questionnaire-Revised. Inter-group comparisons were made for the transition/transfer, at the follow-up, and during the evolution.</p><p><strong>Results: </strong>Thirty-five patients were analyzed; 65.8% male; mean age 31.9 years (SD =10.1). At the transition, Group 1 (n=19; 54.3%) had greater knowledge about their medication and reduced ability to manage appointments and making decisions in comparison to Group 2 at transfer. At follow-up, Group 1 made a better report on their emotional state and significantly improved their ability to manage appointments, communication, and decision-making.</p><p><strong>Conclusions: </strong>Patients who were moved to an adult cystic fibrosis unit through transition were more knowledgeable about their medications. The relative expression levels of microRNA-221 and microRNA-451 were compared between the thyroid papillary carcinoma group (prior to treatment) and the control group. Additionally, microRNA-221 and microRNA-451 expression levels were analyzed in patients with papillary thyroid carcinoma across different pathological characteristics.</p><p><strong>Results: </strong>Serum microRNA-221 relative levels were significantly higher (p<0.001) in the papillary carcinoma group compared to the control group, while microRNA-451 levels were higher in the control group (p<0.001). In the papillary carcinoma group, microRNA-221 expression was significantly higher in patients with extracapsular invasion (p<0.001), lymphatic metastasis (p=0.003), and poor prognosis (p<0.001). Conversely, microRNA-451 expression was significantly lower (p<0.001) in patients with extracapsular invasion, lymphatic metastasis and poor prognosis. The survival rate is < 5 years in 20% of the patients. Although chemotherapy seems to improve life expectancy, to date no interventions have been described that help improve the quality of life of these patients. The aim of this study was to assess if physical therapy may contribute to do this. We present the case of a female patient with gliomatosis cerebri of nine years of evolution showing dependence on technical supports and supervision during walking, reduced speed and balance, tremor and dystonia in lower limbs. The patient underwent physiotherapy treatment: trunk control exercises, limb strength, and gait ability. Improvement in functionality was observed. This study analyses different renal biomarkers in patients undergoing computed tomography scans with iodinated contrast to identify the molecular and cellular mechanisms involved in the pathogenesis of CI-AKI.</p><p><strong>Methodology: </strong>Prospective study that included patients with high risk of renal disease who received iodinated contrast (iohexol) for the computed tomography scans. Functional biomarkers (creatinine and cystatin C), inflammatory and oxidative stress markers (neutrophil gelatinase-associated lipocalin [NGAL], interleukin-8 [IL-8], superoxide dismutase [SOD], F2-isoprostanes, and cardiotrophin-1), and cell cycle biomarkers (Nephrocheck®) were analysed before the iodinated contrast and 4, 12, 24, and 48 hours post-contrast, in relation to the incidence of IC-AKI.</p><p><strong>Results: </strong>IC-AKI was observed in 30.6% of the 62 study participants and in 57.1% of the patients with diabetes and renal dysfunction. Factors associated with IC-AKI were a higher mean age (74.4 vs 64.9 years), pre-existing renal dysfunction (60 vs 16.7%), and higher adjusted mean volume of iohexol (42.9 vs 32.1%). As for non-functional biomarkers. No differences were found between patients with and without CI-AKI.
