The incidence of pertussis was calculated per 100,000 inhabitants for each health district, quarter, and year. Additionally, the relationship between severe cases (hospitalizations) and vaccination status was analyzed.</p><p><strong>Results: </strong>A total of 181 pertussis cases were identified. Of these, 56.4% were reported during the pre-pandemic period (2017-2019), while 9.95% occurred between 2020 and 2023. An increase in cases was observed in the first quarter of 2024 (n = 61; 33.7% of the total cases), compared to previous years (1 to 46 cases). The 0 to 20-year group accounted for 71.3% of cases, of which 57.4% had received at least partial vaccination prior to contracting pertussis. Severe disease requiring hospitalization occurred in 25.6% of cases, with a higher frequently among unvaccinated individuals (70% vs 18.3%; p < 0.001). Among hospitalized cases, 42.4% had not received any prior doses of the vaccine. Additionally, 47.1% of the 17 pertussis cases in children aged =1 year had not received any vaccination and had no history of maternal vaccination during pregnancy.</p><p><strong>Conclusions: </strong>An increase in reported pertussis cases is observed in early 2024 in Malaga, but hospitalization rates among confirmed cases remained stable. This pilot study, conducted with cardiac rehabilitation patients, aimed to evaluate: 1) the criterion validity of the PEDOMETER® app compared to the 6MWT in assessing cardiorespiratory fitness and fatigue resistance, and 2) the construct validity of the PEDOMETER ® app in relation to the Hospital Anxiety and Depression Scale (HADS) and the Health Index of the EuroQol-5D 3L questionnaire (EQ-5D).</p><p><strong>Methods: </strong>Twenty patients undergoing cardiac rehabilitation at HM Puerta del Sur Hospital (Mostoles, Spain) performed the 6MWT while using the PEDOMETER ® app on a smartphone worn on their arm. Pearson's correlation coefficient was calculated to assess the relationship between the distance recorded in meters during the 6MWT and that recorded by the app, as well as between the app and the anxiety and depression dimensions of the HADS, and the Health Index EQ-5D.</p><p><strong>Results: </strong>The criterion validity of the 6MWT and the PEDOMETER ® app was excellent (r = 0.981; p < 0.001). Regarding construct validity, the app showed a moderate negative correlation with the depression dimension of the HADS (r= -0.493), but not with the anxiety dimension or the EQ-5D.</p><p><strong>Conclusions: </strong>The free PEDOMETER ® app demonstrates excellent criterion validity when compared to the 6MWT. This study compares the diagnostic concordance between portable chest X-rays and bedside lung ultrasounds in the intensive care unit (ICU).</p><p><strong>Methods: </strong>Adult ICU patients were included. POCUS was performed using the abbreviated BLUE protocol. Diagnostic results from POCUS and chest radiographies were compared using the intensivist clinical diagnosis - based on clinical examinations and lung ultrasounds - as the reference. Sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) of the ultrasounds were calculated.</p><p><strong>Results: </strong>A total of 100 patients were included, 71 with pulmonary pathologies. The average time to perform the ultrasound was 308 seconds. Ultrasound identified pathology in 20 patients with a normal chest radiographs. Diagnostic discrepancies occurred in 30 patients, highlighting ultrasound´s superior sensitivity in detecting atelectasis, pleural effusions, and pulmonary edema. Ultrasound demonstrated sensitivity (S) of 85%, specificity (E) of 100%, positive predictive value (PPV) of 100%, and negative predictive value (NPV) of 55%.</p><p><strong>Conclusion: </strong>Lung point-of-care ultrasound at ICU admission detects more pathologies and does not miss significant abnormalities seen on chest X-rays. It also shows good diagnostic accuracy. Sociodemographic, amputation, psychopathological (Symptom Checklist, SCL-90-R; Severity Posttraumatic Stress Disorder Scale-Revised, EGS-R), maladjustment (Maladjustment Scale), pain (Numeric Pain Rating Scale) and suicide (Columbia Scale Screening for Suicidal Ideation, C-SSRS) characteristics were assessed.</p><p><strong>Results: </strong>Of the 557 workers identified, 80 were included in the study. The results showed the presence of relevant psychosocial repercussions with a moderately high level of psychopathological symptoms, a Post-Traumatic Stress Disorder (PTSD) prevalence rate of 10%, re-experimentation as the most relevant symptom, and labour and leisure as the most affected areas. Sex differences were found in PTSD re-experimentation symptoms (higher in women) and leisure maladjustment (higher in men). No sex differences were found in the remaining variables studied.</p><p><strong>Conclusions: </strong>This study demonstrates the relevance of psychosocial consequences in workers with amputations. It is believed to arise from barotrauma during vaginal labor, causing alveolar membrane rupture and subsequent air leakage. Clinical manifestations are often nonspecific. Management strategies vary based on symptom severity and complications, high-concentrations oxygen therapy promoting air absorption. We present the case of an 18-year-old male patient with a history of bronchial asthma and tobacco use, underscoring the need for heightened awareness of Hamman´s syndrome in similar cases. Prompt diagnosis and appropriate management are crucial to prevent or quickly address potential life-threatening complications. In collaboration with the National Board of Preschools, fruit and vegetable waste generated during the preparation of lunch was quantified in four preschools in the Metropolitan area of Santiago in Chile.</p><p><strong>Methods: </strong>This study was conducted in four preschools, two in the Western area of Santiago and two in the Eastern area, of which two hold environmental quality certifications. Over a five-day period, the weight of raw materials and waste from vegetables (peels) and fruit (pomace) were measured. Percentages of vegetable and fruit post-cleaning losses and waste were evaluated.</p><p><strong>Results: </strong>Vegetable loss exceeded 20% in 31% of the preschools, primarily from carrots and potatoes. Fruit losses were higher, with pears accounting for the most significant waste, recorded in 75% of the study centers. No differences in vegetable loss was found between centers, while variations were observed for fruit. Preschools with environmental quality certifications wasted less onions (p = 0.016) but more pears (p = 0.036).</p><p><strong>Conclusions: </strong>There is higher fruit loss than vegetable loss, with onions and tomatoes being the least wasted. Possessing an environmental quality certification does not guarantee a significant reduction in overall losses. Handling and storage conditions may play a key role in minimizing losses. Clinical variables and the modified fatigue impact scale (MFIS) were used to assess fatigue levels. Percentage of muscle activation during a core muscle plank was measured via surface electromyography. Pre- and post-intervention results were compared using the Wilcoxon signed-rank test and evaluated with Cohen's D effect size (ES).</p><p><strong>Results: </strong>Twenty-one subjects participated in the study; 81% female, mean age 47.5 years (range: 28-55 years), and median duration of symptoms 21 months (range: 5-24 months); 90.5% of the participants experienced fatigue (MFIS score = 38). Muscle activation during plank exercises improved across all muscles after the intervention, with significant increases in the left (p = 0.011, medium ES) and right external oblique (p =0.039, small ES) muscles and the right latissimus dorsi muscle (p = 0.039, small ES). The survey consisted of 11 questions assessing knowledge and risk perception related to vector-borne diseases. Sociodemographic variables predicting the outcome variable (objective level of knowledge, determined by correctly naming two vector-borne diseases) were analyzed using multiple logistic regression. The outcome variable was then compared with the self-reported knowledge declared by the participants (kappa coefficient),k.</p><p><strong>Results: </strong>Out of the 1,646 respondents who completed the survey, 72.2% were female and 59.8% were >40 years old). Additionally, 87.3% showed a high risk perception and 61.7% understood what is a vector; however, only 36.3% had an objective level of knowledge (k= 0.353). The variables being >40 years (p <0.0001), having a monthly income >1,500 € (p <0.0001), and owning pets (p= 0.0423) were positively related with an objective level of knowledge on vector-borne diseases (adjusted effects).</p><p><strong>Conclusions: </strong>Both knowledge and perception of the risk on vector-borne diseases need to be strengthened in Spain, especially in regions where vector-borne diseases are endemic, emerging, or re-emerging. The risk of developing attention deficit hyperactivity disorder, as well as internalizing and externalizing problems, were assessed using the Child Behaviour Checklist.</p><p><strong>Results: </strong>The risk of internalizing (18%) and externalizing problems (11.7%) was higher compared to the risk of developing attention deficit hyperactivity disorder (1.4%) and were more prevalent in boys than in girls. The most common and bothersome noise exposures were generated at home (50.8-55.3%) and by neighbours (24.5%). The risk of neurodevelopmental problems was associated with sleep disturbances, particularly in relation with attention deficit hyperactivity disorder (16.1 vs 4%; p<0.001), with no differences observed between sex. Sleep disturbances were significantly more common in children exposed to noise from household or neighbours. High levels of noise exposure from street traffic and neighbours were linked to an increased risk of attention deficit hyperactivity disorder, while noise from other children at home was associated with a higher risk of internalizing and externalizing problems. {"title":"[Analysis of serious trauma injury patterns in Navarre (Spain) (2010-2019)].","authors":"Eider Arbizu Fernández, Arkaitz Galbete Jimenez, Tomás Belzunegui Otano, Mariano Fortún Moral, Alfredo Echarri Sucunza","doi":"10.23938/ASSN.1085","DOIUrl":"10.23938/ASSN.1085","url":null,"abstract":"<p><strong>Background: </strong>The aim of this study is to describe major trauma cases in Navarre and analyze differences based on mortality groups, sex, and mode of injury.</p><p><strong>Methods: </strong>Cross-sectional study of major traumas (severity =3) registered in Navarre between 2010 and 2019. We analyzed the type of trauma, intentionality, the mode of injury, and the affected anatomical area. The odds ratio for major trauma associated with different variables was calculated.</p><p><strong>Results: </strong>The study included 2,609 patients; mean age was 54.7 years (0-101) and 70.9% were male. A predominance of accidental (84%) / blunt (94.7%) major traumas was recorded, primarily resulting from falls (46.5%) and car accidents (18.4%). Women experienced more falls and pedestrian accidents, while men had more motorcycle, bicycle, knife/firearm accidents, and contusions. Most major traumas affected the head and thorax. Head trauma was significantly more common in deceased individuals and women, while thoracic trauma was more frequent in patients who died on-site and in men. Head injuries were caused by falls from low heights and firearms, whereas thoracic injuries resulted from car accidents and falls from height. The risk of major trauma decreased with age; deceased patients were between two and three times more likely to present lesions in all anatomical areas.</p><p><strong>Conclusions: </strong>Gender differences are observed in intentionality, type, and mode of injury. Head and thoracic injuries are potentially life-threatening and abdominal and extremity/pelvic ring injuries are associated with early deaths.
