However, no study has yet evaluated the impact of citations in scientific publications. Therefore, this study aimed to analyze the impact of citations from Brazilian researchers in OP and OM over the last two decades.</p><p><strong>Material and methods: </strong>This was a cross-sectional study involving 50 researchers linked to postgraduate programs in OP/OM. Data collected from each professional's Lattes curriculum included gender, academic affiliation, the corporate category of the institution, and location. The number of papers published and citations received between 2004 to 2013 and 2014 to 2023 was also collected from the Web of Science database.</p><p><strong>Results: </strong>Most researchers were male (56%) and from public institutions (90%), mainly in the Southeast region (60%). Over two decades, they collectively published 8,033 scientific articles, with significant growth (p<0.001) from to 2004-2013 to 2014-2023. While the average citations per researcher did not differ significantly between 2004-2013 and 2014-2023 (p=0.538), there was a notable 67.67% increase in citations in the last decade.</p><p><strong>Conclusions: </strong>Brazilian researchers in the areas of OP and OM have demonstrated a significant academic impact over the past two decades, with a marked increase in publications and citations over the last ten years. {"title":"Factors influencing inferior alveolar nerve injury after extraction of mandibular third molar.","authors":"H He, N Ruan","doi":"10.4317/medoral.26576","DOIUrl":"10.4317/medoral.26576","url":null,"abstract":"<p><strong>Background: </strong>This study sought toexplore the manifestations of clinical symptoms and identify the risk factors linked to inferior alveolar nerve injury (IANI) in the context of mandibular third molar extraction.</p><p><strong>Material and methods: </strong>In this study, 172 patients admitted to our hospital for mandibular third molar extraction from June 2021 to December 2022 were selected for the study, and the clinical data of the participants were retrospectively analyzed, and the risk factors of IANI associated with mandibular third molar extraction were analyzed by uni/multi-factor logisitic regression.</p><p><strong>Results: </strong>Noticeable distinctions were noted between the groups with and without injuries in relation to age, time of surgery, number of broken roots, angle of blockage, CEJ (cementoenamel junction) distance, curved roots of the molar, clarity of the upper and lower walls of the nerve canal, and Pell & Gregory classification. Logistic regression analysis showed that age, time of surgery, number of broken roots, angle of blockage (40°-70°), CEJ distance (10-12 mm, >12 mm), and curved roots of the molar were independent risk factors for IANI. Multi-factor logistic regression analysis further confirmed that age, number of broken roots, angle of blockage (40°-70°), CEJ distance (10-12 mm, >12 mm), and curved roots of the molar were associated with IANI.</p><p><strong>Conclusions: </strong>Alveolar nerve injury manifests as hyperalgesia or absence of sensation, numbness and abnormal pain in the lower lip. {"title":"Synchronous cemento-ossifying fibromas: a systematic review.","authors":"B-L Neumann, E-M Mendes, B-B Só, F-M Silveira, V-P Wagner, P-A Vargas, J-N Santos, F-P Fonseca, R-E Bologna-Molina, A Mosqueda-Taylor, M-A Martins, M-D Martins","doi":"10.4317/medoral.26501","DOIUrl":"10.4317/medoral.26501","url":null,"abstract":"<p><strong>Background: </strong>This systematic review aimed to incorporate published data regarding synchronous cemento-ossifying fibromas (COF), with an analysis of their demographic and clinicopathological characteristics.</p><p><strong>Material and methods: </strong>Case reports and case series of synchronous COF were searched in PubMed, Web of Science, Scopus, EMBASE, and LILACS according to the PRISMA (2020) statement. Also, a manual search was carried out and the grey literature was assessed. A descriptive statistical analysis was performed.</p><p><strong>Results: </strong>Nineteen studies comprising 20 cases of synchronous COF were included. The mean age at diagnosis was 35 years (±13.8), with a predominance of female patients (n=12/60%). In 13 cases (65%) the mandible and the maxilla were affected simultaneously. In two cases (10%) first-degree relatives (parents or siblings) had been previously diagnosed with COF. The diagnostic hypotheses were reported in 8 cases (40%), with florid cemento-osseous dysplasia, ameloblastic fibroodontoma, calcifying cystic odontogenic tumor, osteoma and cementoblastoma being cited in the differential diagnosis. Among the cases with details about management (n=17), eleven were treated by surgical enucleation and/or excision (64.7%). Follow-up was provided for 10 cases (50%), with a mean period of 44.7±62.19 months. Recurrence occurred in three of informed cases.</p><p><strong>Conclusions: </strong>Synchronous manifestation of COF is rare. Female patients around the 3rd decade of life are more commonly affected. {"title":"Perioperative effects of different hypotensive anesthesia techniques in orthognathic surgery.","authors":"I Kazımlı, C-Y Asan, D-G Canpolat, A-E Demirbaş","doi":"10.4317/medoral.26662","DOIUrl":"10.4317/medoral.26662","url":null,"abstract":"<p><strong>Background: </strong>This study aimed to investigate the effectiveness of combining sevoflurane with remifentanil, esmolol, or nitroglycerin for hypotensive anesthesia and determine the suitable hypotensive anesthesia method for orthognathic surgery.</p><p><strong>Material and methods: </strong>This retrospective study included 60 patients who underwent orthognathic surgery for developmental malocclusion. They were divided into three groups based on the hypotensive agent preferences: Group 1 (n = 20), sevoflurane and remifentanil; Group 2 (n = 20), sevoflurane and esmolol; Group 3 (n = 20), sevoflurane and nitroglycerin. Bleeding volume, heart rate, systolic, diastolic, and mean arterial blood pressure were recorded at certain times during the perioperative period, including at stages with increased stress levels in the body, such as incision and osteotomy. The patients' blood pressure, analgesic consumption and pain level were recorded in the postoperative period.</p><p><strong>Results: </strong>Bleeding volume, surgery satisfaction related to bleeding, and total operation time did not differ significantly between groups. Intraoperatively, heart rates were significantly higher in Group 3 than in Groups 1 and 2 (p = 0.001). However, hemodynamic stability was similar in Groups 1 and 2. Postoperatively, analgesic consumption, pain levels, and blood pressure dynamics did not differ significantly between groups (p > 0.05).</p><p><strong>Conclusions: </strong>Based on this study's results, it was concluded that infusing remifentanil, esmolol, or nitroglycerin with sevoflurane during orthognathic surgery successfully achieved the targeted hypotensive anesthesia and can be considered alternative methods. {"title":"Relationship between mandibular third molars and mandibular angle and condylar fractures.","authors":"J Mu, Y Wu, C Wu, H Piao, B Jin","doi":"10.4317/medoral.26604","DOIUrl":"10.4317/medoral.26604","url":null,"abstract":"<p><strong>Background: </strong>Assess the correlation between the position of the third molar (M3) and fractures of the mandibular angle and condyle using panoramic radiographs to offer valuable data references for oral clinical research.</p><p><strong>Material and methods: </strong>A retrospective cross-sectional study was undertaken, involving the collection of 409 cases of mandibular fracture in the Yanbian University Hospital. The case records and panoramic radiographs of mandibular angle fracture (78 cases) and condylar fracture (106 cases) were evaluated.</p><p><strong>Results: </strong>In the comparative analysis between the mandibular angle fracture group and the condylar fracture group, statistical significance was observed in the variables of M3 existence (P = 0.002), eruption of M3 from the alveolar cavity (P = 0.003), P&G position classification (P = 0.001), deep impactions (Classes IC, IIC, IIIB, and IIIC) (P < 0.001), and the presence of impacted M3 in both groups (P < 0.001).Regarding M3 roots, the mandibular angle fracture group exhibited the highest prevalence of multiple roots at 75.4%, surpassing the 64.6% observed in the condylar fracture group. The prevalence of proximal angles in the mandibular angle group and the condyle group was the highest, accounting for 64.6% and 61.5%, respectively. The percentage of M3 in the two groups was 80% and 43.1%, respectively, with a significant difference (P < 0.001).</p><p><strong>Conclusions: </strong>Impacted mandibular third molars (M3) elevate the risk of mandibular angle fractures, while their absence or normal eruption reduces this risk and protects against condylar process fractures. The fracture risk is influenced by the M3's position: P&G Class II and Class B impactions, where M3s emerge partially from the alveolar bone, are significantly associated with mandibular angle fractures. This study proposed to carry out a bibliometric analysis of epidemiological studies that investigated the diagnosis of oral cancer between 2002 to 2024.</p><p><strong>Material and methods: </strong>On April, 2024, the Scopus and Web of Science databases were explored, and the selected articles underwent bibliometric analysis of performance and scientific mapping of journals, authors, institutions, and countries, as well as the predominant topics and trends in research into the diagnosis of oral cancer through an analysis of references and co-occurrence of key words. The analyzes were carried out using the R extension package, Bibliometrix, and the VOSviewer software.</p><p><strong>Results: </strong>A total of 532 documents were included. China contributed the highest number of publications (36.71%) and total citations [1,584]. Seoane J was the most prolific author [16 (h-index: 9)], while Warnakulasuriya S had the highest total link strength [282 (h-index: 7)] in co-citations. Oral Oncology was identified as the most prolific [231 (72.64%)], co-cited and impactful journal (h-index: 13). Dysregulation within this system is linked to the oncogenesis process. Reduced expression of MMR system proteins identified in salivary gland tumors (SGTs) suggests an increased risk of tumoral occurrence. This study aims to analyze the expression of MMR proteins in SGTs and discuss the relevance of this association to the development of these neoplasms.</p><p><strong>Material and methods: </strong>This review was conducted following the PRISMA guidelines and was registered in PROSPERO (CRD42023465590). A comprehensive search of the PubMed/MEDLINE, Web of Science, Scopus, Embase, and ProQuest (non-peer reviewed platform) was performed to answer the question \"Do DNA MMR system proteins exhibit expression in SGTs?\". The methodological quality of the selected studies was assessed using the JBI's Critical Appraisal Tool.</p><p><strong>Results: </strong>A total of 142 patients with benign SGTs and 84 with malignant SGTs were included in this review. The literature analysis showed a notable reduction in the expression of DNA MMR system proteins (hHMS2, hMLH1, hMSH3 and hMSH6) in the percentage of marked cells.</p><p><strong>Conclusions: </strong>The reduction in the expression of the DNA MMR system proteins suggests an interesting correlation with the development of malignant and benign SGTs. {"title":"Localised 0.2% chlorhexidine irrigation delivery system versus oral antibiotics in reducing postoperative complications in the surgical extraction of impacted mandibular third molar (IMTM). - a randomised controlled trial.","authors":"A Chugh, G P G, P Kumar, A Kaur","doi":"10.4317/medoral.26676","DOIUrl":"10.4317/medoral.26676","url":null,"abstract":"<p><strong>Background: </strong>The purpose of the study was to compare the efficacy of the use of 0.2% chlorhexidine irrigation and the oral antibiotics for the prevention of postoperative complication like pain, trismus, swelling and infection after the surgical extraction of IMTM.</p><p><strong>Material and methods: </strong>A randomised, double blinded clinical trial was planned with two equal groups. Patients were randomly divided into two groups using computer-generated codes with an allocation ratio of 1:1. Group I (Control): Standard preoperative and postoperative systemic oral antibiotics and Group II (Study): No systemic antibiotics and Chlorhexidine irrigation local delivery. The primary outcomes evaluated were postoperative pain, mouth opening, swelling and infection. The secondary outcome variables were the number of analgesics and antibiotics taken by the patient in the postoperative period, the satisfaction of the patient and adverse events, were followed up regulary for 7 days postoperatively.</p><p><strong>Results: </strong>A total of 84 patients, divided into two equal groups participated in the study. In intergroup comparison of swelling, the difference was non-significant on postoperative day (POD) 1 and 7, except for POD 3, where it showed significantly lower results in the antibiotic group (p = 0.012). However, there was no significant difference in pain found between both groups at any of the postoperative time points, and the study group had a lesser need for rescue analgesics than the control group. A statistically significant difference in incidence of dry socket was observed (p = 0.03) and gastrointestinal adverse symptoms, but it showed insignificant results for wound dehiscence and pus discharge. Also, patient satisfaction was higher in the study group.</p><p><strong>Conclusions: </strong>both antibiotics and localised delivery demonstrated comparable results in terms of swelling, pain and trismus. {"title":"Salivary cortisol levels in patients with potentially malignant oral disorders and oral mucosal cancer: a case-control study.","authors":"E López-D'alessandro, J Palomino, L Escovich","doi":"10.4317/medoral.26606","DOIUrl":"10.4317/medoral.26606","url":null,"abstract":"<p><strong>Background: </strong>The relationship between salivary cortisol secretion and the presence of cancer in the oral cavity has not been completely clarified. Due to this, we proposed carrying out a study to determine salivary cortisol levels in patients with potentially malignant disorders (PMD), oral squamous cell carcinoma (OSCC) and healthy individuals.</p><p><strong>Material and methods: </strong>Cross-sectional case-control study, among 80 patients seen at the Faculty of Dentistry of the National University of Rosario, Argentina, between January 2018 and April 2020. 40 cases represented by Leukoplakia, Lichen, Erythroplakia and SCC and 40 controls were included. Smoking habit and alcohol consumption were included. The presence of stress was determined. Morning salivary cortisol levels were measured with the Roche electrochemiluminescence method (Traceability: St by ID-MS).</p><p><strong>Results: </strong>Patients with SCC presented elevated salivary cortisol values. Individuals with stage III tumors showed levels higher than 8.74 ng/ml in all cases. A significant association between cortisol levels and stress was detected in patients in the control group (p<0.005) and in individuals with PMD (p=0.009). This association was not significant in patients with SCC (p=0.999). After applying the logistic regression method, when adjusting odds ratios according to tobacco and alcohol consumption and the presence of stress, the association between cortisol levels and presence of stress was highly significant (p<0.001).
