Il s’agit de lutter contre le changement climatique mais également contre le dépassement des limites planétaires qui menacent la vie sur Terre. Les crises écologiques impactent la santé publique et paradoxalement le soin engendre aussi des dommages environnementaux nuisibles pour la santé. Les hôpitaux depuis quelques années cherchent à réduire leur empreinte environnementale en menant des démarches de développement durable et en répondant au triple objectif : être socialement équitable, économiquement viable et écologiquement vivable. Par ailleurs il existe des co-bénéfices des politiques de lutte contre les crises environnementales sur la santé. Les approches intégratives de « <em>one health</em> » et de santé planétaire permettent de saisir le lien entre les enjeux environnementaux, la santé des écosystèmes, la santé humaine et ces co-bénéfices. Ces approches intégratives et systémiques guident l’hôpital sur la voie de la soutenabilité et des soins de santé durables.</p></div><div><p>The recent announcement of the plan for the ecological transition of the healthcare system is a testimony that the ensemble of health professionals took the measure of environmental upheavals and their sanitary consequences. It is not just a question of combating climate change, but also of ensuring that the planetary limits that threaten life on Earth are not exceeded. Ecological crises have an impact on public health, and paradoxically, health care also generates environmental damage that is harmful to health. For some years now, hospitals have been seeking to reduce their environmental footprint by implementing sustainable development initiatives and meeting the triple objective of being socially equitable, economically viable, and ecologically sustainable. In addition, there are co-benefits of policies to combat environmental crises on health. Integrative “one health” and “planetary health” approaches help us to grasp the link between environmental issues, ecosystem health, human health, and these co-benefits. Grâce à une approche transversale et avec l’aide de tous les acteurs de soin, il est possible, à chaque étape de la prise en charge, d’avoir des actions pour réaliser des soins plus durables et de bonne qualité tout en diminuant l’impact environnemental du système de soin.</p></div><div><p>Optimizing the patient's peri-interventional pathway greatly contributes to the eco-design of care through the integration of the three pillars of sustainable development: economic, social and environmental. La place de la ventilation non invasive et de l’ostéosynthèse pariétale reste encore à définir dans la stratégie de prise en charge. Le recours à l’ostéosynthèse pariétale s’avère intéressante en cas de traumatisme thoracique fermé chez les patients sous ventilation invasive au moment de la chirurgie, ce d’autant plus que celle-ci est précoce (dans les 48 premières heures post-traumatique). La VNI doit s’envisager chez les patients les plus hypoxémiques avec un rapport PaO<sub>2</sub>/FiO<sub>2</sub> <!-->&lt;<!--> <!-->200 ou en cas de lésions associées (médullaires, bassin et abdomen pouvant interférer avec la mécanique ventilatoire ou imposer l’immobilisation).</p></div><div><p>Severe chest trauma is common and associated with a high morbi-mortality rate. The role of non-invasive ventilation and surgery in the management of chest trauma has yet to be defined. Operative management should be considered in cases of closed thoracic trauma in patients undergoing invasive ventilation at the time of surgery, particularly if the surgery is performed early (within the first 48<!--> <!-->hours post-trauma).
