Evaluation of lipids in non-fasting sample was found to be useful but there are not many reports from India. Methods: One hundred and forty five patients with coronary artery disease (CAD) patients grouped as stable angina (SA), unstable angina (UA) and myocardial infarction (MI) and 66 healthy subjects were studied. Total serum cholesterol, triglycerides (TG), high-density lipoprotein cholesterol (HDL) and non-HDL were estimated in a non-fasting serum sample. Results: Increase in total serum cholesterol, TG, non-HDL along with a decrease in HDL levels was observed in CAD patients. The same trend was observed across the spectrum of CAD (SA, UA and MI). Conclusions: Non-fasting sample can also be utilized in order to evaluate CAD patients hospitalized in the acute phase where waiting for obtaining a fasting sample may delay institution of specific treatement. Consanguinity was noted in 31%. History of cholecystectomy was evident in 5 cases. Blood transfusions were received in the past in 52% of cases. Symptoms at presentation were jaundice (85%), pain (80%), fatiguability (60%), pallor (30%), dyspnoea (29%), lump abdomen (7%) and leg ulcer (3%). Acute chest syndrome was seen in 10.9% cases. Physical examination revealed pallor (90%), icterus (80%) hepatomegaly (49%) and splenomegaly (41%). Mean haemoglobin at presentation was 8.3 g/dL. Sickle cells were seen in peripheral smear in 51%. Sickling test was positive in all after induction. It is one of the leading causes of disability among young adults and prevents them from leading normal productive lives. Methods: We retrospectively studied new cases diagnosed to have schizophrenia (n=211) seen during the period, March 1999 - February 2001. Their premorbid personality traits, socio-demographic parameters were recorded from the case records. Psychopathology was assessed by using positive and negative symptom scale for schizophrenia (PANSS); functional outcome was assessed by using Global Assessment Scale (GAS). Results: Socio-demographic variables did not have influence on outcome. Among patients with well adjusted personality 50% showed good to fair outcome. Patients with schizoid premorbid personality had a fair outcome. Because of innumerable contraindications for the use of this drug, there is a continuous search for other alternatives. Methods: In a prospective, randomized and double-blind study, we compared the intubating conditions and haemodynamic responses during rapid tracheal intubation using either suxamethonium or rocuronium with ephedrine pretreatment. We recruited 50 patients and allocated them into 2 groups (n= 25 each); Group S: received suxamethonium 1.5 mg/kg and Group R: received rocuronium 0.6 mg/ kg with ephedrine 100 μg/kg pretreatment. All patients were induced with 2 mg/kg propofol and intubation was attempted at 60 seconds. Haemodynamic responses and quality of intubating conditions were assessed. Results: Both groups were comparable in respect to age, sex, weight, Mallampati grade, Cormack Lehane grade and duration of laryngoscopy. Although both groups had clinically acceptable intubating conditions (good and excellent), there were more number of patients with better intubation score in Group S compared to Group R (p = 0.014). Conclusion: Suxamethonium still continues to be the “gold standard” for providing ideal tracheal intubation conditions. On examination, he had adenoma sebaceum over face (Figure 1), gross pallor, severe tenderness in right lumbar region and a right loin lump extending into hypochondriac and iliac regions. Contrast enhanced computed tomography (CT) of the abdomen revealed right side enlarged kidney with massive subcapsular right renal haematoma (Figure 2) and multicentric nodules with of fat densities (< 20 Hounsfield units) within both the renal parenchyma, suggestive of bilateral renal angiomyolipomas. CT of the brain showed subependymal nodules (Figure 3). This pattern of cerebral ependymal nodules together with adenoma sebaceum was diagnostic of tuberous sclerosis. Material and methods: In this cross-sectional study, the prevalence of thyroid disorders and MetS were studied in 54 consecutive adult patients with RA (mean age 46.0±10.4 years; 48 females) and 54 age and gender-matched healthy control subjects. Results: The prevalence of thyroid disorders was higher in patients with RA than in control subjects; however, this difference was not statistically significant [19/54 (35.2%) Vs 12/54 (22.2%); p=0.201]. Nine patients with RA already known to have hypothyroidism were receiving levothyroxine treatment. Among the remaining RA patients (n=45), a significantly higher prevalence of autoimmune thyroid disease (AITD) (10/45 Vs 4/54; 2=4.437, p=0.045) and subclinical hypothyroidism with anti-thyroid peroxidase (anti-TPO) antibody positivity (4/45 Vs 0/54; 2=5.002, p=0.040) were observed compared with healthy control subjects. The prevalence of MetS was higher in patients with RA than in control subjects; however, this difference was not statistically significant [31/54 (57.4%) Vs 25/54 (46.3%); p=0.336]. Conclusions: A significantly higher prevalence of AITD and subclinical hypothyroidism with anti-TPO antibody positivity in patients with RA suggests that these patients would benefit from screening for AITD. Methods: Patients with painful bone metastases were randomly assigned to receive: Arm A: 8 Gray (Gy) in single fraction (n=15), Arm B: 20 Gy in 5 fractions at the rate of 4 Gy per fraction (n=15) and Arm C: 30 Gy in 10 fractions at the rate of 3 Gy per fraction (n=15). The arms were compared in terms of pain relief, performance improvement, analgesic requirement and duration of overall response at 1week, 1month, and 3 months after treatment. Results: Pain relief in Arms A, B and C at 1 week were 60%, 53.3% and 60%; at 1 month were 71.4%, 73.3%, 73.3%, at 3 months were 78.5%, 80% and 80% respectively. Performance improvement in Arm A, B and C at 1 week 60%, at 1 month 65%, 66% and 66%, at 3 months 78.5% ,80% and 80% respectively. Duration of overall response in all the patients who improved after therapy is 3 months There was no statistically significant difference between three arms with respect to pain relief, performance improvement, analgesic requirement and duration of overall response. Conclusions: The 8 Gy Single fraction is as efficacious as multi-fractionated regimens.
