An additive hyperglycaemic response, secondary to the metabolism of intravenous fluids, can thus prove detrimental to the well-being of the patient, if ignored. Methods: We compared the effects of 4% gelatin and dextran-40 on blood glucose levels during surgery under subarachnoid block and their potential to induce hyperglycaemia. Sixty American Society of Anesthesiologists grade I and II patients were randomised into two groups, 30 patients in each. Group 1 patients were preloaded with 4% Gelatin (10 mL/kg) and Group 2 patients were preloaded with dextran-40 in normal saline (10 mL/kg), over a period of 30 min. Just prior to preloading, base line capillary blood glucose (CBG) level was noted this is followed by subsequent readings at 20 min interval until 100 min from base line reading. All patients received normal saline (0.9%) as a maintenance fluid. Under strict aseptic precautions, subarachnoid block using 15 mg of hyperbaric bupivacaine at L3-L4 or L4-L5 level was given after preloading. Results: The CBG levels in both the groups at 20, 40, 60, 80 and 100 min from base line were within physiological limits. In group 2, the maximum CBG level was found at 60 min from onset of preloading (P = 0.017), when compared with corresponding CBG level in Group 1. Alexander Marcet (17701822)2 developed chemical tests for examination of calculi while William Prout (1785-1850)3 developed a simple test for diagnosing diabetes and diseases of the urinary tracts. The present study evaluated the usefulness of urinary beta-N-acetyl D-glucosaminidase (u-NAG), alkaline phosphatase (u-ALP) and lactate dehydrogenase (u-LDH) as markers of tubular injury and urinary microalbumin (u-MA) as a marker of glomerular injury for early diagnosis of CIN in patients undergoing coronary interventions. \u0000Methods: One hundred and twenty patients scheduled for elective coronary angiography (CAG), with or without angioplasty with baseline serum creatinine less than 1.2 mg/dL were recruited. Serum creatinine, u-NAG, u-ALP, u-LDH and u-MA were analyzed at 0, 4 and 24 hours after administration of low-osmolal, non-ionic contrast medium. \u0000Results: CIN developed in 27 (22.5%) patients. A significant increase in u-ALP, u-LDH and in u-MA was seen in both CIN and non-CIN groups. However, no significant difference was observed in these markers between the two groups. Weak D refers to reduced expression of D antigen on the red blood cell that requires an extended testing with indirect antiglobulin test (IAT) to get detected. Clinical importance of weak D arises when labelling the donor and patient, as the donor is labelled as D positive, patient as D negative. \u0000Methods: In our center all blood donor and patient samples are tested for ABO and Rh D by conventional tube technique using two anti-D reagents; anti-D immunoglobulin M (IgM) monoclonal and a blend of anti-D IgM and immunoglobulin G (IgG). The blood samples which were negative for agglutination by immediate spin method were further tested for weak-D using IgG anti-D in the IAT phase with low ionic strength solution (LISS)/Coombs’gel card. \u0000Results: A total of 46,654 blood samples were tested (22,326 donors and 24,328 patients) during the period January 2012 to August 2014. Among these 43,771 (93.82%) were Rh D positive and remaining 2,883 (6.18%) were Rh-D negative. A total of 30 individuals (16 donors and 14 patients) were weak D positive constituting 1.04% of Rh-D negatives and 0.06% of total individuals screened. \u0000Conclusions: This study shows the prevalence of weak D antigen in our population who are representative of Rayalaseema region of Andhra Pradesh. It is an autosomal dominant disorder and results from mutations in adenomatous polyposis coli (APC) gene. Patients with GS if not treated early will invariably develop colonic cancers at a much younger age than those with sporadic colonic carcinoma. These patients also develop other malignant tumours like duodenal cancers, gastric cancer, hepatoblastoma, papillary carcinoma of the thyroid and multifocal cholangiocarcinomas. With early diagnosis and treatment of colonic polyposis, adenocarcinoma of the duodenum has become the leading cause of death in FAP patients. The mean age at which duodenal carcinoma is diagnosed in FAP is 45-52 years. False positive serological tests lead to wastage of large number of blood units. Nucleic acid amplification test (NAAT) technology has greatly enhanced the accuracy in identification of transfusions transmitted infections. \u0000Methods: The present study was undertaken to study the seroprevalence of hepatitis C virus (HCV) and asses the concordance between seropositivity with the presence of HCV ribonucleic acid (RNA) and to know the distribution pattern of HCV genotypes in healthy blood donors. \u0000Results: Among the 9287 donors screened (88.3% males), 7153 (77%) were voluntary donors while 2134 (23%) were replacement donors. Among blood donors, 27 (0.3%) samples tested HCV seropositive. Among 27 anti-HCV positive samples only 11 (41%) were found positive for HCV RNA. There is an urgent need to understand ageing as a holistic process. The changes occurring in ageing kidneys resemble to those occurring in chronic kidney disease. However, many differences exist between them. Old age is a risk factor for acute kidney injury and increased risk of urinary tract infections and asymptomatic bacteriruria, Serum creatinine is a suboptimal indicator of renal function in the elderly because loss of muscle mass in elderly decreases creatinine production. Serum cystatinc measurement, with reference values adjusted for age represents a promising marker to measure renal function in the elderly. The default management strategy for renal failure in elderly is more of conservative in developing countries when compared to developed countries where there is changing trend from conservative to initiation of renal replacement therapy. The decision when to initiate renal replacement therapy is more challenging. The idiopathic cases of SEP, which lack any identifiable cause from clinical, radiological and histopathological findings, are also reported under the descriptive term “abdominal cocoon syndrome”. Patient with SEP present with intestinal obstruction. Persistent untreated SEP may advance to bowel gangrene or intestinal perforation, which are life threatening conditions. \u0000We report the rare occurrence of SEP in a 45-year-old male presenting with signs of intestinal obstruction. Imaging findings revealed abdominal cocoon with bowel gangrene leading to perforation and the same was confirmed at surgery. Surgical excision of the fibrotic sac encasing the bowel, resection of gangrenous bowel segment and end ileostomy were performed. Histopathology of the excised membrane confirmed the diagnosis of SEP. Two months ago she had conceived spontaneously. Pelvic ultrasonography revealed cervical pregnancy of 8 weeks duration. Parenteral methotrexate failed to terminate pregnancy and serum beta- human chorionic gonadotropin levels continued to rise. In order to achieve haemostasis, catheter angiography and bilateral uterine artery embolization were done followed by curettage of cervical canal. The patient recovered completely and was discharged after 2 days.
