Sachan","doi":"10.15380/2277-5706.JCSR.14.067","DOIUrl":"https://doi.org/10.15380/2277-5706.JCSR.14.067","url":null,"abstract":"Background: There is a paucity of observational data from India on initial catch up growth in hypothyroid children started on thyroxine replacement therapy. \u0000Methods: We retrospectively studied 44 children and adolescents (32 girls) with a diagnosis of primary hypothyroidism i.e., (thyrotropin > 15 μIU/mL and thyroxine < 55ng/mL). All were started on treatment with oral thyroxine. The dose was adjusted to maintain thyrotropin between 0.5-5 μIU/mL. Height was measured at baseline and at each follow-up visit and height standard deviation scores (HtSDS) were calculated. We studied the therapeutic benefit of thyroxine replacement therapy on growth in the initial couple of years of treatment. \u0000Results: Dose of thyroxine required to restore euthyroidism was 4.1±2.5 μg/kg body weight. The dose fell from 9.7±2.4 μg/kg body weight in infants to 3.0±1.5 μg/kg body weight in adolescence (p<0.001). Likewise, the dose per unit body surface area also fell consistently from 207 ± 70.3 μg/m2 in infants to only 89.3±17.9 μg/m2 in adolescents (p<0.001). The initial HtSDS was -2.0±1.5 and this improved by 0.4 to final value of -1.6±1.3 (p < 0.001) after an average follow up of 14.1±2.5 months. Altered lipid profile and increased lipid peroxidation activate endothelial dysfunction and atherothrombosis leading to PIH. Therefore, estimation of lipid profile with serum malondialdehyde (MDA) in pregnancy may be helpful in predicting the development of PIH and further progression. Material and methods: In this prospective case-control study, serum lipid profile and MDA were estimated in 70 PIH subjects with gestational hypertension, pre-eclampsia, eclampsia; and 70 normotensive pregnant women aged 18 40 years, with gestational age of over 20 weeks. Results: A statistically significant higher serum total cholesterol, very low density lipoprotein cholesterol (VLDL-C), low density lipoprotein cholesterol (LDL-C), triglycerides (TG), TC/HDL-C, LDL-C/HDL-C and MDA, and a significantly lower HDL-C was noted in PIH subjects as compared to control subjects. When compared with the severity of PIH, all the lipoproteins (except HDL-C) along with MDA were found to be higher in women with eclampsia when compared with gestational hypertension, pre-eclampsia and normotensive pregnant women. Conclusions: An abnormal lipid metabolism along with oxidative stress may add to the promotion of vascular dysfunction leading to PIH. Lipoproteins and MDA alter significantly in eclampsia. The aim of thisstudy was to find out the reasons for discarding blood and blood components. \u0000Materials and Methods: We retrospectively studied all whole blood and blood components collected during January 2013 to June 2014 at our tertiary care teaching hospital blood bank in South India. \u0000Results: Of the 5261 whole blood bags, 298 (5.7%) were discarded. Of these, 146 (49%) were discarded because of seroreactivity for transfusion transmitted infections (TTI). Of the 19586 blood components prepared, 1449 (7.4%) were discarded. Among blood components discarded, most common units were platelets (16.3%). The most common cause of discarding the blood components was due to expired date (36.9%). \u0000Conclusion: A properly conducted donor screening, notification and counselling of permanently deferred donors will help in discarding less number of bags which are positive for different TTI. These are very rare tumors with an incidence of 0.012% in general population. They present as slow growing, asymptomatic neck masses of varying duration. They occur in sporadic, hereditary and hyperplastic variants. The hereditary forms tend to be bilateral, functional and carry higher risk of malignancy. The angiography is the modality of choice for diagnosis and treatment planning. The surgical resection is the mainstay of treatment which can be performed with acceptable complication rates in experienced centres. It is astonishing to find vitamin D deficiency in India despite plentiful sunlight. This could probably because of urbanization, life style and dress code changes, and revision of vitamin D adequacy range. Recent studies in south India using in vitro ampoule model with 7- dehydrocholesterol have shown adequate formation of active form of vitamin D in mid-day sun. In India 70% of populations reside in rural areas. Only 35% of Indian populations have access to regulated milk supply. We as humans can get vitamin D from abundant sunshine, by exposing 18% of body surface area (without sunscreen) to mid-day sun for 30-45 min to cause 1 minimal erythemal dose (MED) which is equivalent to taking about 600 to 1000 IU of vitamin D. This is about the recommended daily dose by expert group on human nutrient requirements and the dose used in studies with fortified milk supplementation studies. Vitamin D synthesized in the skin lasts two-times longer in the body. In populations where there is limited exposure to sunlight, like dress-code limiting sun-exposure, usage of sunscreen with (SPF) greater than 8 etc., vitamin D supplementation may also be required. Sparse published data are available on dietary calcium intake of metropolitan city population from south India. \u0000Methods: We recorded the dietary intake of calcium and phytates of 524 healthy subjects from Bengaluru, a metropolitan city, by recalling the diet consumed in the previous 5 to 7 days and compared it with dietary intake of calcium of 325 healthy subjects from rural areas around Tirupati, and 508 healthy subjects from urban Tirupati. \u0000Results: The dietary calcium intake of the Tirupati rural subjects was the least compared to that of the urban and metropolitan city subjects. In the metropolitan and urban groups the diet was high in calories, milk, milk products and vegetables compared to rural subjects. Dietary calcium intake was significantly lower (p<0.0001) in the rural subjects compared to the urban and metropolitan city subjects. The dietary phytate were significantly different in both the rural, urban as well as the metropolitan city groups (p<0.0001). Despite the long list of several other blood groups discovered so far, the knowledge and distribution of ABO and Rh-D blood group are essential for effective management of blood bank inventory. \u0000 Methods: We retrospectively studied the distribution of ABO and Rh blood group antigens in donors presenting to our tertiary care teaching hospital blood bank in south India during the period January 2007 to August 2014. Blood group was determined by commercially available standard monoclonal antisera by test tube agglutination technique. \u0000 Results: A total of 49,110 donor samples were tested during the study period for ABO grouping and Rh-D typing. Out of these 96.9% were males. The frequency of O, B, A, AB and Bombay blood groups were 41.7%, 32.2% 20%, 6.1% and 0.03% respectively. Rh (D) positive and negative blood groups were seen in 92.8% and 7.2% respectively. The allele frequencies of the I A , I B and I O alleles were 0.1398, 0.2148 and 0.6454 respectively. Methods: Retrospective study of 6 patients, who presented with pyrexia of unknown origin (PUO) seen over a 7-year period who were diagnosed to have AOSD after a thorough work-up. Results: Their mean age was 24.6 (range 18-38) years; there were four males. Mean duration of symptoms was 7.8 (range 3-5) weeks. In addition to prolonged fever, patients presented with rash, arthropathy, hepato-splenomegaly and peripheral lymphadenopathy. Laboratory evaluation revealed neutrophilic leukocytosis, hepatopathy, serositis, raised serum ferritin levels; anti-nuclear antibody and rheumatoid factor were negative in all. One patient developed acute respiratory distress syndrome and died. The remaining five patients were treated with non-steroidal anti-inflammatory drugs, oral corticosteroids, and hydroxy chloroquine and responded well to treatment. Conclusions: Increased awareness and a high index of suspicion is required for the diagnosis of AOSD.
