Superficial sensations were intact. There was history of being treated for prolonged fever two years ago, for about three weeks. Based on history, clinical and microscopic examination, she was diagnosed to have post kala-azar dermal leishmaniasis. Treatment with parenteral sodium stibogluconate was initiated, to which she responded satisfactorily. In addition, health-care associated neurological infections may develop in critically ill patients admitted to an ICU for other indications. Central nervous system infections can develop as complications in ICU patients including post-operative neurosurgical patients. While bacterial infections are the most common cause, mycobacterial and fungal infections are also frequently encountered. Delay in institution of specific treatment is considered to be the single most important poor prognostic factor. Empirical antibiotic therapy must be initiated while awaiting specific culture and sensitivity results. Choice of empirical antimicrobial therapy should take into consideration the most likely pathogens involved, locally prevalent drug-resistance patterns, underlying predisposing, co-morbid conditions, and other factors, such as age, immune status. Further, the antibiotic should adequately penetrate the blood-brain and bloodcerebrospinal fluid barriers. The presence of a focal collection of pus warrants immediate surgical drainage. Following strict aseptic precautions during surgery, hand-hygiene and care of catheters, devices constitute important preventive measures. We present 50-year-old-woman who presented with a vaginal mass that was diagnosed to be a malignant round cell tumour which later was confirmed to be primary vaginal Ewing’s sarcoma/PNET on light microscopy and immunohistochemical staining. She was then treated with induction chemotherapy followed by local radiotherapy and further maintenance chemotherapy. This rare case of primary vaginal Ewing’s sarcoma/PNET emphasizes the need for combining morphological features with immunohistochemistry with a panel of antibodies in establishing the diagnosis of Ewing’s sarcoma/PNET at an uncommon site. Further, the case also highlights the use of induction chemotherapy followed by radiation therapy and subsequent maintenance chemotherapy as a treatment modality. Technical advances in automated cell separators have substantially improved the productivity and quality of the collection of apheresis platelets. \u0000Methods: We prospectively studied 90 healthy donors who volunteered for plateletpheresis. We compared the preand post-plateletpheresis haematological values, namely, haemoglobin (Hb), haematocrit, platelet count, mean corpuscular volume, mean corpuscular haemoglobin, mean corpuscular haemoglobin concentration, platelet distribution width (PDW), red blood cell count, white blood cell (WBC) count and mean platelet volume (MPV) in all of them. \u0000Results: Following plateletpheresis a significant decrease in pre- and post-donation Hb (g/dL) (14.8 ± 1.097 Vs 14.5 ± 1.4; p=0.002); haematocrit (%) (43.29 ± 6.62 Vs 41.64 ± 4.96; p=0.045); platelet count (χ 109/L) (280.34 ± 54.55 Vs 175.58 ± 44.56; p< 0.001); WBC count (χ 103/μL) (8.28 ± 1.88 Vs 6.95 ± 1.76; p< 0.001); and PDW (fL) (15.96 ± 1.38 Vs 4.96 ± 0.67; p=0.053) and a slight rise in MPV (fL) (8.61 ± 0.77 Vs 8.72 ± 0.83; p=0.067) were observed. The cleavage of amyloid precursor protein (APP) by -secretase generates peptide fragment A42, the main cause of memory and cognitive defects in AD. Therefore, elevated level of -secretase results in accumulation of insoluble form of A peptides (senile plaques) representing the protein as an attractive drug target of AD. Methods: Five recently reported -secretase antagonist (thiazolidinediones, rosiglitazone, pioglitazone, SC7 and tartaric acid) structural analogs were searched from ligand info database and prepared using LigPrep. The crystal structure of -secretase was optimized using Maestro v9.2 protein preparation wizard applying optimized potential for liquid simulations (OPLS)-2005 force field. Structure based virtual screening was performed for -secretase from prepared ligands using virtual screening workflow of Maestro v9.2 and was ranked based on XP Gscore. Results: Eight lead molecules were identified to have better binding affinity (lower XP Gscore) compared to five existing -secretase antagonists and appear to have good pharmacological properties. Binding orientations of the lead molecules were in well agreement with existing inhibitors. Lead1 showed lowest XP Gscore (-10.72 Kcal/mol). Molecular dynamics (MD) simulations of -secretase-lead1 complex was stable in all trajectories.
