López Castilla, Juan Luis Becerra Cuñat","doi":"10.1016/j.sedene.2020.03.001","DOIUrl":"https://doi.org/10.1016/j.sedene.2020.03.001","url":null,"abstract":"<div><h3>Objectives</h3><p>To study the prevalence of mood disorders in patients with epilepsy and their relationship with seizure control.</p></div><div><h3>Methods</h3><p>We conducted an observational and cross-sectional study of epileptic patients treated in our centre from January-2017 to June-2019. We classify patients into 2<!--> <!-->groups according to crisis control: good control (≤1 crisis / month) and poor control (> 1 crisis / month) and we compare the demographic variables (age, sex, employment status and civil status), clinics (epilepsy type, crisis type, years of evolution of epilepsy), therapeutic (antiepileptic, antidepressant and anxiolytic drugs), presence of depression (NDDI-E scale), anxiety (GAD7 scale) and quality of life (QOLIE10). The data was collected by doctors and nurses after informed consent was signed.</p></div><div><h3>Results</h3><p>152 patients participated, 53% were men with a mean age of 44 ± 11 years. Sixty-eight percent (n = 103) had good crisis control and 32% (n = 49) poor control. Thirty-six point 6<!--> <!-->percent had depression (NDDI-E> 15) and 42% anxiety (GAD> 10). Sixty percent of patients with depression and 54% with anxiety did not receive antidepressant and / or anxiolytic treatment. The factors associated with poor crisis control were the presence of depression (OR 2.3, <em>P</em>=.02) and a worse quality of life (OR 1.8, <em>P=</em>.01).</p></div><div><h3>Conclusions</h3><p>The presence of altered mood in patients with epilepsy is frequent. Moral-Munoz","doi":"10.1016/j.sedene.2019.12.001","DOIUrl":"https://doi.org/10.1016/j.sedene.2019.12.001","url":null,"abstract":"<div><h3>Introduction</h3><p>Pusher syndrome is characterized by the patient pushing towards the hemiparetic side with the contralateral side, in a sitting position, standing and during movement, surpassing the gravitational mean line, which produces inappropriate adjustment and postural control.</p></div><div><h3>Objective</h3><p>To determine the effectiveness of physiotherapy treatment in post-stroke pusher syndrome and to extract guidelines for its correct identification and interdisciplinary approach.</p></div><div><h3>Method</h3><p>A systematic review was carried out (2000-2019), using the specialized databases MEDLINE, Scopus, PEDro and Web of Science. The descriptors used in the search strategies were “pusher syndrome”, “pushing behaviour”, “controversial pushing”, treatment, “physical therapy”, physiotherapy.</p></div><div><h3>Results</h3><p>Although the evidence is still limited, the intensive intervention of Physiotherapy in the patient with pusher syndrome post-stroke seems to improve the clinical symptoms in terms of severity and postural control. Data was collected from written self-assessments (pretest; n<!--> <!-->=<!--> <!-->83) and exam papers (postest; n<!--> <!-->=<!--> <!-->82) completed by third-year nursing students enrolled in the 2016-17 academic year in one nursing school in Spain.</p></div><div><h3>Results</h3><p>The analysis revealed 3<!--> <!-->conceptions in relation to understanding stroke care delivery when apathy occurs: a) apathy as a multi-causal response; b) apathy as a reactive psychological response; c) apathy as a result of brain injury.</p></div><div><h3>Conclusions</h3><p>The analysis of conceptions in this research has shown that the percentage of students arguing apathy based on one single factor did not vary from pretest to postest. The relationship between functional loss and reactive psychological answers was the abounding argument. Identification of conceptions will allow nurse educators to design evidence-based teaching-learning strategies focused on difficulties in stroke care delivery among undergraduate nursing students. The patients were grouped into 3 groups according to the rehabilitation treatment received: non-rehabilitation (NR), light rehabilitation (RL) and intense rehabilitation (RI).</p><p>Sociodemographic data, clinical variables (on stroke), hospital stay, rehabilitative treatment, and functional status (Barthel index) were collected.</p></div><div><h3>Results</h3><p>350 patients, with a mean age of 75.83 (± 8.02) years. The hospital stay was longer in the group of RL (19.7 [±<!--> <!-->8.69]), RI (17.67 [±<!--> <!-->10.05]) and of those who did not undergo rehabilitation (10.97 [±<!--> <!-->6.96]) (<em>P</em> <!-->=<!--> <!-->.001).</p><p>A significant increase (<em>P</em> <!--><<!--> <!-->.001) was observed in the Barthel index scores from admission to 8<!--> <!-->weeks after the stroke.</p><p>Age (<em>P</em> <!-->=<!--> <!-->.003) and hospital stay (<em>P</em> <!-->=<!--> <!-->.013) were shown as risk factors for functional dependence. {"title":"Plan de cuidados de enfermería en el síndrome epiléptico relacionado con infección febril: a propósito de un caso","authors":"María Palanca Cámara , Rosa Güell Baró","doi":"10.1016/j.sedene.2019.10.001","DOIUrl":"https://doi.org/10.1016/j.sedene.2019.10.001","url":null,"abstract":"<div><h3>Introduction</h3><p>Febrile infection-related epilepsy syndrome (FIRES) is a serious epileptic encephalopathy of unknown aetiology, which leads to refractory epileptic status and neuropsychological deterioration. It appears mainly in children and has 3 phases: initial, acute and chronic. In the chronic phase, educational intervention is important to reduce and address exacerbations. Its prevalence is low (1/100,000 children in Europe) which makes it a disease of interest to present as a clinical case with a nursing care plan.</p></div><div><h3>Case</h3><p>This is a 16-year-old male patient who debuted at age 7 and has intellectual impairment and abehavioural disorder. Approximately every 21 days he has exacerbations requiring hospitalization, a situation that we address in this case.</p></div><div><h3>Care plans</h3><p>The main needs we find are breathing (risk of aspiration), food (risk of electrolyte imbalance), elimination (urinary and faecal incontinence), mobility (deterioration of physical mobility), communication (deterioration of verbal communication) and safety (tiredness of the caregiver's role, risk of falls, risk of infection, risk of deterioration of skin integrity).</p></div><div><h3>Conclusions</h3><p>In the acute phase, the need for safety is the most affected, requiring the necessary prevention measures. On the other hand, hospitalization is a good time to work with the family, optimizing the resources provided (home service, ketogenic diet, among others). The most severe presentation is systemic loxoscelism that can compromise the patient's life if it is not diagnosed and treated in time. There are no reports about how it presents in patients with an epilepsy diagnosis. We present the case of a 28-year-old male patient, with a prior history of epilepsy in treatment with anticonvulsants. He developed systemic loxoscelism after a spider bite in the posterior middle third of the right arm.
