Chiari malformations are almost always present at birth, though symptoms may not develop until later in childhood. In adult years, problems can include persistent headaches, neck pain, and weakness and/or numbness and tingling in the arms and legs, which makes it a disease of interest to present as a clinical case with a nursing model care plan.</p></div><div><h3>Case</h3><p>This is a 43-year-old female patient with type I Chiari malformation. She has neck and shoulder pain, stiffness in the hands, as well as hypotonus and hypotrophy in the upper limb. The patient underwent foramen magnum decompression surgery.</p></div><div><h3>Care plans</h3><p>We used Roy's adaptation model for the nursing care plan. The main needs we find are respiration (risk for ineffective breathing pattern), mobility (impaired physical mobility), sensation (acute pain), protection (risk for infection, and risk for bleeding), fluid and electrolyte (risk of fluid and electrolyte imbalance), and self-concept (anxiety).</p></div><div><h3>Conclusion</h3><p>The Roy's adaptation model approach was useful in this case as it focuses on the patient's adaptability and is suitable for use in perioperative nursing management of type I Chiari malformation in adults. The symptomatology associated with PD is diverse. The clinical manifestations of patients include motor and non-motor symptoms. One of the non-motor symptoms is weight loss and a high risk of malnutrition. It is important to consult neurology regarding the diagnosis and follow-up of patients with PD.</p></div><div><h3>Objective</h3><p>To show the relationship between dietary habits and signs and symptoms in Parkinson disease.</p></div><div><h3>Method</h3><p>The methodology used consisted of a systematic review of narrative analysis on the relationship between food and the symptoms associated with PD. The databases consulted through the critical reading programme (critical appraisal skills programme, Spanish) were: MedLine, Web of Science, Scopus and PubMed. Among the inclusion criteria: studies in Spanish and English were accepted. The time range was from 2014 to 2019. The search terms used were: “Parkinson's Disease”, “Healthy Diet”, “Feeding Behaviour” and “Signs and Symptoms”.</p></div><div><h3>Results</h3><p>A total of 85 published study documents were identified, of which 8 met the inclusion criteria. Three documents were included from the literature review of grey literature and 11 selected documents were analysed.</p><p>Approximately 60% of patients with PD suffer from constipation. A high fibre diet and proper hydration are essential. Modified texture diets are the first strategic line in the management of dysphagia and will have to be considered in the treatment of PD. Vitamin D intake was found to be decreased and supplementation should be considered. Other non-motor symptoms in early stages, such as depression and hyposmia, may affect dietary choices and be responsible for the observed nutritional status decline.</p></div><div><h3>Conclusions</h3><p>Nutritional care should be considered as an integral part of the treatment of patients with PD, because the nutritional status of patients with PD is a relevant component of their quality of life. Combined with conventional physiotherapy (CP), dance as a therapeutic intervention has shown its functional benefits in other neurological diseases, such as Parkinson disease.</p></div><div><h3>Goals</h3><p>To determine gait dysfunction in a patient, to implement and carry out a physiotherapy and dance intervention programme and assess the results with a gait analysis platform.</p></div><div><h3>Subject and methods</h3><p>Evaluation pre and post intervention of the following variables: range of motion (goniometry), muscle strength (Daniel's scale), spasticity (Modified Ashworth scale), balance (Berg balance scale (BBS)) and spatio-temporal gait patterns (GaitRite®); intervention of 15 one-hour sessions (3 weeks) combining CP and Spanish Dance (SD) in a 49-year-old woman with a relapsing-remitting MS diagnosis.</p></div><div><h3>Goals</h3><p>To determine gait dysfunctions in this patient, to implement and carry out a physiotherapy and dance intervention programme and assess the results with a gait analysis platform.</p></div><div><h3>Results</h3><p>The spasticity decreased and the strength of the ankle in flexors and extensors increased. The balance and impaired spatio-temporal parameters of gait improved significantly (an increase of gait velocity, step length and monopodal support phase and increase of 25 points on the BBS).</p></div><div><h3>Conclusion</h3><p>Gait disturbance consistent with data from previous research was found and significant improvements have been achieved thanks to the intervention performed. Moreover, the dependence of the patient will increase.</p></div><div><h3>Objectives</h3><p>To assess the efficacy of specific interventions designed to train non-professional caregivers of patients diagnosed with amyotrophic lateral disease.</p></div><div><h3>Method</h3><p>Descriptive cross-sectional study of an intervention designed for non-professional caregivers of patients with ALS. Pilot study. The target audience of this study are non-professional caregivers of patients with ALS. The level of dependence of the patients is measured with the Barthel index, the Revised Scale of Functional Assessment of Amyotrophic Lateral Sclerosis and Cognitive Behavioural Screening in ALS. The needs of these patients’ caregivers were evaluated according to the Kreutzer questionnaire. A multidisciplinary intervention designed to modify this situation was designed and reevaluated.</p></div><div><h3>Results</h3><p>Finally, seven caregivers were included in the pilot study. They were women, with an average age of 56 years, partners of the patients, with parallel economic activity and with an average level of education. The patients were men with a mean age of 63 years and 1-3 years of disease duration; they presented a moderate-mild Barthel, cognitive impairment, gross motility and respiratory function. The most demanded needs were for «medical/health information» and «involvement in treatment and care».</p></div><div><h3>Conclusion</h3><p>Specific interventions to meet the needs of non-professional caregivers are highly effective. Therefore, to improve our difficulties in education, it is necessary to identify the knowledge needs of patients in relation to their disease.</p></div><div><h3>Objective</h3><p>To explore the expectations of epilepsy patients regarding the health education received.</p></div><div><h3>Method Design</h3><p>descriptive qualitative study from a hermeneutical perspective. Subjects: Intentional sampling of 18 patients with epilepsy volunteers to participate in health education sessions between January and July 2019. Data collection and analysis: Documentary technique through analysis of the wording to the question about their expectations in education sessions.</p></div><div><h3>Results</h3><p>We obtained 10 codes that are codified in 3 subcategories of expectations in education: learning and disease management, problems with mood and interaction and sharing with other people with epilepsy. Ocassionally, medical-care team requirements might not meet MS patientsʹ needs.</p></div><div><h3>Objectives</h3><p>To learn the experience of patients and their caregivers of the medical care received in the Gregorio Marañón Hospital Demyelinating Unit.</p></div><div><h3>Method</h3><p>We designed a focus group with 26 participants incidentally recruited from the Gregorio Marañón Hospital Demyelinating Unit: 8 with low physical disability, 8 with high physical disability and 10 caregivers. Information was collected regarding 3 main topics: information, clinical care and medical team.</p></div><div><h3>Results</h3><p>High satisfaction was found among all participants with most of the medical care and with the professionals of the MS unit. They agreed with the interventions they were offered but they considered that physical rehabilitation and psychological support must be offered, not only to MS patients but also to their caregivers and family. This study translated the Brazilian version of the booklet “Laura's diary: knowing hydrocephalus and its treatment” into Spanish as spoken in Spain.</p></div><div><h3>Methodology</h3><p>This methodological study conducted in Spain (Universidad de Santiago de Compostela) and Brazil (Botucatu Medical School) undertook the following translation steps: translation, using two Brazilian translators fluent in Spanish; synthesis, involving a Spanish nurse with experience in education; and back-translation, using a Brazilian translator and nurse fluent in Spanish.</p></div><div><h3>Findings</h3><p>The two versions of the initial translation were similar, with few amendments necessary during the consensus meeting between the researchers to achieve synthesis of the translations. The two versions of the back-translation were very similar to the original.</p></div><div><h3>Conclusion</h3><p>A free Spanish booklet was presented to minimize the negative effects of surgery, empower families, and aid future research between Brazil and Spain. Mismanagement of a disease can affect a child's quality of life.
