{"title":"Valoración de la disfagia con el test método exploración clínica volumen-viscosidad en pacientes ingresados tras un accidente cerebrovascular","authors":"Estela Hernández-Bello , Lorena Castellot-Perales , Concepción Tomás-Aznar","doi":"10.1016/j.sedene.2018.08.001","DOIUrl":"https://doi.org/10.1016/j.sedene.2018.08.001","url":null,"abstract":"<div><h3>Objective</h3><p>The objective of this study is to describe the frequency of dysphagia and the associated factors in stroke patients hospitalised in the Neurology Unit of the <em>Hospital Clínico Universitario Lozano Blesa</em>, in Zaragoza, as well as to analyse the PPV and NPV of the volume-viscosity swallow test (V-VST).</p></div><div><h3>Methods</h3><p>A cross-sectional descriptive study was conducted on stroke patients in order to assess the detectability of dysphagia using the V-VST and to track their progress for 7 days.</p></div><div><h3>Results</h3><p>The large majority (87.7%) of the patients did not have dysphagia. The study population included 81 subjects, of which 65.4% were men with a mean age of 72.84 years. The stroke was ischaemic type in 59.3% of cases, with no previous history of stroke in 86.4%, and with a slight dependence in48.1% measured with Barthel index. The test was performed in the first 24<!--> <!-->h in 79% of the population, with preventive dietary measures being introduced in 56.8% of patients. Some signs of lack of security were observed in 7 patients (8.6%) in the first week. Significant statistical relationships were found between the dysphagia and the dependence, signs of lack of security during the intake in the following days, and the type of diet. Neurostimulation therapy is shown as a widely-accepted treatment for patients who have completed the pharmacological treatment options with no results.</p></div><div><h3>Objectives</h3><p>To show the importance of early detection of post-surgical complication in spinal cord stimulation patients with spinal electrode as well as an adaptation and personalization of nursing care in the event of post-surgical complications.</p></div><div><h3>Method</h3><p>A case of a woman suffering long-term chronic neuropathic pain following a serious traffic accident and subsequent spinal trauma. A study of a number of possible medical options was undertaken to decide the best outcome for the patient; finally, fitting a spinal electrode implant was decided as the best option, in order to start spinal cord stimulation therapy. Post-surgical paraplegia and cerebrospinal fluid leak were detected and incorrect placement of the electrode was observed on magnetic resonance imaging. However, this is not usually addressed in clinical practice.</p></div><div><h3>Objective</h3><p>To evaluate coping skills, psychosocial adjustment and quality of life of patients with PD and their family/carers from a quasiexperiment at baseline time.</p></div><div><h3>Method</h3><p>Quasi-experimental study carried out in Primary Care centres to evaluate the impact of a psychoeducational intervention in contrast with an informative intervention. The sample comprised 80 patients with PD and 80 family carers, divided into a control group and an experimental group. The psychosocial adjustment scale PAIS-SR, the coping scale Brief COPE and the quality of life scales PDQ-39 and SQLC were used in the data collection. The analysis of sociodemographic data and Student's t tests was performed using SPSS 23.0.</p></div><div><h3>Results</h3><p>The patients and family/carers from the control group and the experimental group noticed a mild impairment in their quality of life and some difficulties in their psychosocial adjustment to illness. Both groups used coping skills with a medium-low frequency. Acceptance was the most used coping skill by patients and family/carers. Conventional treatment is effective in the first years of the disease, but other kinds of therapy should be considered upon the onset of motor and non-motor complications that are clearly impacting quality of life. At present we have three advanced therapies which have proven beneficial for these types of patients, namely: deep cerebral stimulation, subcutaneous apomorphine infusion pumps, and continuous intestinal levodopa infusion to the duodenum (Duodopa®).</p></div><div><h3>Objectives</h3><p>To assess the impact of advanced therapies on the quality of life of patients with Parkinson's disease, measured using the PDQ39 scale.</p></div><div><h3>Methods</h3><p>Observational, prospective, non-randomized study. Three measurements (n<!--> <!-->=<!--> <!-->20) using the PDQ39 scale were taken: the first before initiating advanced therapy, the second at 3-5 months, and the third at 6-10 months.</p></div><div><h3>Results</h3><p>Men who receive treatment with advanced therapies are younger and have fewer years of progression of the disease than women. There were significant differences between the three measurements in terms of emotional well-being, stigmatization, cognitive state and overall assessment of PDQ39. Similarly, significant differences were found in pairs in the long term in these parameters. In this article, a bioelectrical quantification method is used for the study of attention, based on a frequency exponent.</p></div><div><h3>Objectives</h3><p>To analyse the attention of a selected sample of 36 volunteers from the Armed Forces in a state of rest and during the performance of military simulation tasks. To establish the potential existence of differences between the male and female groups in different EEG types (rest with closed eyes and 2<!--> <!-->specific attention tasks).</p></div><div><h3>Methods</h3><p>The spectral power was calculated and an exponent obtained that models this magnitude by means of a function of coloured noise type. The third minute of the signals of each subject was used in 3 types of EEG and 12 channels. The data were analysed using a statistical test.</p></div><div><h3>Results</h3><p>Significant differences were observed between males and females in the basal EEG (rest with closed eyes) in the parietal, occipital, temporal, and central regions (1.62 was the value of the male cortical average versus 1.39 for the female), and during the task (j2) in frontal, occipital and parietal regions (1.34 for the male cortical average versus 1.16 in the female).</p></div><div><h3>Conclusions</h3><p>The female group shows greater cerebral bioelectrical complexity than that of men in the same type of EEG. Venous thromboembolism is a serious condition in which small clots form in the deep veins of the legs, called deep vein thrombosis (DVT), and these clots are carried through the blood circulation to the lungs, leading to pulmonary embolism. Intermittent pneumatic compression (IPC) has been proven to prevent deep vein thrombosis in 29.9% of stroke patients.</p></div><div><h3>Objectives</h3><p>1. Discover the effects of intermittent pneumatic compression protocol implementation in patients with severe stroke in relation to comfort and safety, and determine how long implementation would take and the work load of this new protocol. 2. Compare functional evolution after 3 months in patients on whom an IPC system was applied with the previous sample at the beginning of the system application.</p></div><div><h3>Method</h3><p>Observational, transversal and descriptive study which evaluated the viability and safety of the IPC system in acute stroke patients in the Stroke Unit of the Hospital Vall d’Hebron. IPC was applied for 24<!--> <!-->hours on patients with ischaemic stroke treated with reperfusion and on patients with intracranial haemorrhage during 72<!--> <!-->hours.</p></div><div><h3>Results</h3><p>172 patients were included in the sample. The mean age was 71<!--> <!-->±<!--> <!-->14 years. 48% were women. 79.2% were diagnosed with ischaemic stroke. In the application phase the nurses reported that prior to training they had taken 15<!--> <!-->±<!--> <!-->2,3<!--> <!-->minutes to use the system and that after training application only took 6<!--> <!-->±<!--> <!-->1,5<!--> <!-->minutes. Practically all patients (98.8%) who were given IPC suffered no discomfort and did not present with any complications derived from its use. During this period no cases of DVT were detected.</p></div><div><h3>Conclusions</h3><p>Implementation of the IPC systems protocol has been proven to have a preventative and efficient effect in the reduction of embolic complications, which is safe for the patient and with no discomfort in its use. The nurses’ work load is not increased with appropriate training. The aim of this study is to determine these alterations in a 29-year-old patient with primary progressive multiple sclerosis, and to describe and implement a physiotherapeutic intervention (40<!--> <!-->min/day, 2 days/week for 5 months), and objectify the results through instrumental gait analysis. An increase was obtained in the muscular balance (Daniels Test) in the right lower limb, and in balance using the Berg Balance Scale (6 points), as well as a decrease in spasticity in the Modified Ashworth Scale in the right triceps surae. There was also an improvement in the spatiotemporal gait parameters, an increase in average speed by 0.11 m, a decrease of 4.7 steps/min of the cadence, a decrease in step width by 0.03 m, and an increase in stride lengths (0.08 m right, 0.09 m left). The data suggested a functional improvement, even in the case of a progressive disease.
