{"title":"Delirium en pacientes de la Unidad de Cuidados Intensivos de una institución de salud en Montería, Colombia","authors":"Jorge Luis Herrera Herrera , Edinson Oyola López , Yolima Judith Llorente Pérez","doi":"10.1016/j.sedene.2019.04.003","DOIUrl":"https://doi.org/10.1016/j.sedene.2019.04.003","url":null,"abstract":"<div><h3>Introduction</h3><p>Delirium, in addition to causing suffering to patients, relatives and caregivers, is associated with increases in morbidity, mortality and hospital complications, which leads to high costs for the health system.</p></div><div><h3>Objective</h3><p>To determine the incidence of delirium and related factors in patients admitted to an intensive care unit of a health institution in the city of Monteria, Colombia.</p></div><div><h3>Methods</h3><p>A descriptive, longitudinal study with a quantitative approach that includes the correlation of variables of interest with the incidence of delirium. For the collection of information and subsequent analysis, the CAM-ICU scale –which allowed the identification of the presence of delirium– and the APACHE II severity scale were used. Likewise, a data card was prepared based on clinical and epidemiological variables of interest.</p></div><div><h3>Results</h3><p>The patients were mostly men (50.5%) with an average age of 60 years. The incidence of delirium was 22% with hyperactive presentation in 62% of cases. Sociodemographic and clinical variables were collected using body mass index (BMI), CONUT index, Mini Nutritional Assessment (MNA), Barthel's index, Rankin scale, volume-viscosity clinical examination method, on admission, on discharge, and at 3<!--> <!-->months.</p></div><div><h3>Results</h3><p>On admission to hospital, 64.3% (92) of the patients had some degree of malnutrition according to the CONUT index, and 13.4% (22) were at risk of malnutrition according to the MNA. The median (interquartile range) of dependency was 100 (90;100). The median disability was 1 (0;2). Forty point four percent (67) of the patients had dysphagia.</p><p>At 3<!--> <!-->months, using the abovementioned scales, 58.2% of the patients had some degree of malnutrition, and 17.1% (28) were at risk of malnutrition. The median dependency was 95 (80;100). The median disability was 1 (1;3). Twelve percent (20) had dysphagia. There was a statistically significant relationship between dysphagia and level of malnutrition according to the CONUT index on admission (<em>P</em> <!-->=<!--> <!-->.021) and at 3<!--> <!-->months (<em>P</em> <!-->=<!--> <!-->.045), and according to the MNA at 3<!--> <!-->months (<em>P</em> <!-->=<!--> <!-->.009).</p></div><div><h3>Conclusion</h3><p>In stroke patients, there is an association between dysphagia and malnutrition, both on admission to hospital and at three months. Fampridine is recommended to control this symptom. Its clinical benefits are assessed by timed and subjective tests, such as the 12-item MS walking scale (MSWS-12). Determining patients’ responses using the MSWS-12 enables nurses to identify, evaluate and monitor NANDA diagnoses relating to mobility.</p></div><div><h3>Aim</h3><p>To analyse the effectiveness of the MSWS-12 in relation to NANDA diagnoses in monitoring subjects with MS who are responders to fampridine.</p></div><div><h3>Methods</h3><p>Observational study, descriptive and retrospective study from October 2017 to September 2018 In Girona's Unitat de Neuroimmunologia i Esclerosi Múltiple Territorial. A total of 17 fampridine responders were included in the study. Variables: age, sex, EDSS and baseline MSWS-12 scores, at 15 days, and at 6 months.</p></div><div><h3>Results</h3><p>An increased total MSWS-12 score was observed between 15 days and 6 months of: impaired walking <span><math><mover><mtext>x</mtext><mo>¯</mo></mover></math></span> = 1.9 (<em>p</em>=.076); impaired standing <span><math><mover><mtext>x</mtext><mo>¯</mo></mover></math></span> = .2 (<em>p</em>=.637); fatigue <span><math><mover><mtext>x</mtext><mo>¯</mo></mover></math></span> = .9 (<em>p</em>=.209); risk of falling <span><math><mover><mtext>x</mtext><mo>¯</mo></mover></math></span> = .6 (<em>p</em>=.270).</p></div><div><h3>Conclusion</h3><p>Impaired walking and fatigue had a greater tendency to worsen over time. Risk of falling and impaired standing had a more stable score. As an alternative, a trained nurse can perform this evaluation.</p></div><div><h3>Objective</h3><p>To describe the non-motor evaluation of the patients from the Cohort of Patients with Parkinson's Disease in Spain, 2015 study (COPPADIS-2015) conducted by movement disorder specialist nurse from our centre.</p></div><div><h3>Methods</h3><p>Patients and caregivers from the COPPADIS-2015 study were included. The assessment included: 1) cognition (PD-CRS and puzzle test); 2) non motor symptoms as a whole (NMSS); 3) mood (BDI-II); 4) sleep (PDSS); 5) neuropsychiatric symptoms (NPI, QUIP-RS); 6) pain and fatigue (VAS); 7) gait problems (FOGQ); 8) disability (ADLS); 9) quality of life (PDQ-39SI, PQ-10, WHOQL-8); 10) caregiver status (ZCBI, CSI, BDI-II, PQ-10, WHOQL-8).</p></div><div><h3>Results</h3><p>102 patients (65.3<!--> <!-->±<!--> <!-->8.2 y; 52% males) and 62 caregivers were evaluated between January 2016, and October, 2017. Mean time of non-motor evaluation was 73.7<!--> <!-->±<!--> <!-->20.5<!--> <!-->minutes. {"title":"Impacto del deterioro cognitivo en la recuperación de la capacidad funcional, la institucionalización y la mortalidad de los ancianos intervenidos de fractura de cadera","authors":"Francisco Javier Amarilla Donoso , Rosaura Toribio Felipe , Marcelina Rodríguez Ramos , Raúl Roncero García , Jesús María Lavado García , Fidel López Espuela","doi":"10.1016/j.sedene.2019.01.001","DOIUrl":"https://doi.org/10.1016/j.sedene.2019.01.001","url":null,"abstract":"<div><h3>Objectives</h3><p>To determine whether the baseline cognitive state influences functional recovery 6 months after hip fracture.</p><p>To discover the relationship of functional results with comorbidity and cognitive impairment (CI)</p></div><div><h3>Methods</h3><p>This was a prospective cohort study, performed in a hospital in the province of Cáceres, which included 184 patients over 65 years of age who underwent hip fracture between November 2016 and October 2017.</p><p>We included clinical, sociodemographic, personal history, and functional dependence variables (Barthel Index, Lawton and Brody Scale) on admission and at 6 months.</p></div><div><h3>Results</h3><p>Of the total 184 patients, 50% knowingly had CI.</p><p>The mean age was 85.48 years (SD: 5.82), 70.9% were women. There were no significant differences in age between the 2 groups (<em>P</em> <!-->=<!--> <!-->.081).</p><p>In the pre-post study of each cohort, there were significant differences in all the functional variables studied referring to BADL and IADL at pre-fracture and at 6 months post-fracture (<em>P</em> <!--><<!--> <!-->.05).</p><p>Institutionalization after discharge was 35.3% in the group of patients without CI, compared to 75% of patients with CI.</p></div><div><h3>Conclusions</h3><p>A significant decrease in BADL and IADL was observed in both groups.
