This study investigates the relationship between asthma control and sensitization to inhalant allergens. \u0000Methods: A cross-sectional study was conducted over a one year period since January 2011. Asthmatics were sequentially selected from allergy clinic at King Abdulaziz University Hospital in Jeddah. Evaluation of asthma control was based on GINA guideline. Assessment of the sensitization towards inhalant allergens was measured in-vivo by wheal size and number of positive reactions on the standard skin prick test (SPT). SPSS was used to analyse any statistical correlation. \u0000Results: A total of 110 asthmatics with a mean age of 34 ± 14 years were included, 63.6% being females. Asthma was predominantly uncontrolled in 68 cases (61.8%), partly controlled in 26 cases (23.6%) and controlled in 16 cases (14.5%). SPT to common inhalant allergens was positive in 76 asthmatics (77.6%); of which 54 (55.1%) were uncontrolled, 13 (13.3%) partly controlled and only 9 (9.2%) were controlled asthmatics. The predominant allergens were Dermatophagoides pteronyssinus in 57 cases (54.8%), Dermatophagoides farinae in 49 cases (47.1%), cat epithelia in 35 cases (33.7%) and cockroach in 23 cases (22.1%). Uncontrolled asthma was correlated significantly with positive SPT (p=0.038). \u0000Conclusions: More than half of the uncontrolled asthmatics population is sensitized to indoor inhalant allergens. This clinically favours the assessment of allergic status in any asthmatic, whose symptoms are not controlled. This is one of the early studies exploring the potential link between asthma control and sensitization to inhalant allergens.","PeriodicalId":356612,"journal":{"name":"Family Medicine and Medical Science Research","volume":"424 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2015-12-14","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"116005084","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Privacy and security have always been an issue in telemedicine. In order to overcome this problem, cloud has been adopted to store and securely access data. Cloud offers a way to allow medical data and images to be transferred from patient to medical clinicians providing security. But individual patient data is not provided with privacy when it is outsourced to public cloud. In this paper, using a case study of screening the masses for early detection of non-communicable diseases at Sri Kshetra Suttur, privacy is built into telemedicine or mobile health care system with the help of the private cloud. Efficient key generation, Encryption, Decryption and analysis of health data misuse by authenticating authorized Clinicians to access patient records using Paillier Cryptosystem and Searchable Symmetric Encryption are some of the salient features introduced in this paper.","PeriodicalId":356612,"journal":{"name":"Family Medicine and Medical Science Research","volume":"62 15 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2015-11-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"114852890","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Severe acute malnutrition(SAM) affects approximately 20 million children under five years of age, contributes to more than 1 million child deaths worldwide each year and being treated with community based management of acute malnutrition (CMAM) program. So that this study aimed of assessing the effect of CMAM integrated nutrition education on infant and young child feeding (IYCF) knowledge and practice of mothers/caregivers. Method: A quasi experimental study design was employed on a total of 100 mother/caregivers and acutely malnourished child pairs enrolled in outpatient treatment program (OTP) of CMAM. A nutrition education intervention comprising 8 specific messages held twice a month for 6 continuous months. A pre-tested semi-structured questionnaire was used to collect three phase (Baseline, Follow-up and End line) data. Collected data entered to SPSS and descriptive statistics was computed. Paired t-test and independent sample t-test were used to check within and between group change on mean knowledge and practice score of mothers/caregivers. Result: At the end of the intervention period mean knowledge score of mothers/caregivers was statistically significantly (p-value<0.001) improved within intervention group and between intervention and control group. At baseline (p-value=0.42) and follow up (p-value=0.44) between groups mean practice score of mothers/caregivers was not statistically significant different. Between group mean practice score comparison come up with statistically significant (p-value<0.001) difference. By the end of the study within group comparison for mean practice score showed significant difference both for intervention and control. Conclusion: IYCF focused nutrition education improved knowledge and practice of mothers/caregivers of children on medical and nutrition treatment. Further study can be done with large sample size, longer intervention period and more strong design to check consistency of result. Since mothers’/caregivers’ knowledge and practice of IYCF is crucial to sustain positive clinical outcomes of OTP management of acute malnutrition stakeholders better give due emphasis to mothers/care givers IYCF focused nutrition education in line with the clinical management.","PeriodicalId":356612,"journal":{"name":"Family Medicine and Medical Science Research","volume":"4 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2015-11-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"130487338","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The purpose of this study was to fill the research gap by describing care providers’ experiences of factors that influence counselling and supporting family members in out-of-hospital emergency care. Methods: The data were collected by semi-structured interviews of emergency care providers (N=15) in Finland in 2014 and analysed using content analysis. Results: Factors that influence the counselling aimed at family members in out-of-hospital emergency care can depend on the patient, family member, emergency care provider or context. Patient-dependent factors involve the patient’s personality, severity of physical and psychological symptoms, criticality of the situation, the patient’s memory, responses, behaviour and cognitive abilities and emotional resources. Family member-dependent factors include the family members’ needs and responses, fears, anxiety, feelings of not having control, needs for support, cognitive and emotional preparedness to commit themselves to help and support the patient and willingness to engage in supporting the patient. Care provider-dependent factors include the care provider’s personality, clinical expertise, theoretical and practical competence, work and life experience and attitudes. Context-dependent factors include time, degree of urgency, safety of the environment and the circumstances. Conclusions: Care providers must gain an overall idea of the family’s function and their ability to endure stress in acute situations, so that counselling can be founded on the family’s strengths and limitations regarding the patient’s follow-up care and coping at home. Ability to differentiate between various factors in counselling situations helps care providers concentrate on factors that they can affect. The results can be used in the education of health carestudents and professionals and in developing the quality of counselling.","PeriodicalId":356612,"journal":{"name":"Family Medicine and Medical Science Research","volume":"57 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2015-11-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"126957290","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Few studies have evaluated MI training efforts with providers who work with pediatric patients using objective treatment integrity tools. This pilot study aims to test the effectiveness of a four-hour continuing education workshop, plus performance feedback and coaching, on MI skillfulness among family and pediatric health professionals. Materials and methods: Participants completed standardized patient (SP) practice samples at baseline (BL), received written performance feedback, attended an MI workshop, provided a one-week SP practice sample (FU1), received written feedback and a coaching call, and provided a final SP practice sample at two months (FU2). All practice samples were coded using the Motivational Interviewing Treatment Integrity Code (MITI 3.1.1). Results: Participants showed improvements in MI skillfulness between BL and FU1 that were associated with moderate to large effect sizes. Despite feedback and coaching, some deterioration in skillfulness was seen between FU1 and FU2. Few providers met beginning proficiency or competency standards at either follow-up point. Discussion: While the tested training model led to improvement in skillfulness, and telephone feedback was feasible, additional training is likely needed to meaningfully impact behavior.","PeriodicalId":356612,"journal":{"name":"Family Medicine and Medical Science Research","volume":"13 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2015-11-20","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"122171925","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

A personalized disease risk assessment leads to recommendations for evidence-based interventions that can delay/prevent disease onset or reduce the severity of disease. As the sophistication of medical diagnoses develops and new interventions become available, the value of collecting and analyzing family health history (FHH) for maintaining patient wellness by determining ‘the right treatment, at the right time, for the right patient’ is more apparent than ever. FHH remains underutilized in primary care, however, because of numerous barriers. Ironically, the introduction of genetic tests and genomic methods that identify carriers who might be vulnerable to a variety of medical conditions and diseases has simply raised the importance of collecting and utilizing FHH to guide patient management in primary care.","PeriodicalId":356612,"journal":{"name":"Family Medicine and Medical Science Research","volume":"6 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2015-10-15","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"117040429","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
