{"title":"Management of Infected Orthopedic Joint Implants: Pertinent Information for Family Medicine Physicians","authors":"Gómez Mm, J. Manrique, Antonia F. Chen","doi":"10.4172/2327-4972.1000125","DOIUrl":"https://doi.org/10.4172/2327-4972.1000125","url":null,"abstract":"Periprosthetic joint infections (PJIs) are one of the most devastating complications after total joint replacement. An early diagnosis can improve the outcome of treating PJI, and the first steps for diagnosing PJI are often conducted by a family doctor or a general practitioner. \u0000When a patient presents with a suspicion for PJI, the steps that must be followed are based on the clinical practice guideline by the American Academy of Orthopedic Surgeons (AAOS). Clinical symptoms, such as pain, are the main complaint that alerts physicians to a potential PJI, along with other symptoms such as erythema, warmth and tenderness around the surgical wound. These patients should receive x-rays of the affected joint, and laboratory tests such as erythrocyte sedimentation rate and C-reactive protein should be drawn. Physicians should actively avoid starting antibiotic therapy, obtaining nuclear medicine imaging, sending patients to infectious disease, or prescribing physical therapy. Population 2.8 million. Rural population 67% spread over 860 villages. \u0000Method: An interventional “screen and treat” population based program of village outreach by holding Eye Camps--in remote areas of the District. \u0000Participants: Candidates –lined up for screening at Eye Camps. \u0000Results: Of the 49’ 518 screened; 32’ 615 men, women and children had refractory errors (mainly myopia and presbiopia). This is 66% of the total. 1’350 persons had cataract related blindness. Methods: Eight different types of alcoholic beverages consumed in different parts of Bangladesh were collected and studied in the laboratory of Bangladesh Council of Scientific and Industrial Research (BCSIR). Before sending to the laboratory, samples were stored in a refrigerator at temperature 4-8 degree Celsius. In all samples, strength of ethanol content was studied. Among the samples, Dochuani and Tari was tested for heavy metal, Chubichi and Pochani studied for total viable micro-bacterial contamination. Results: In this study one sample was from Khagrachari (Hilly area) not been reported as manufacture site by the Department of narcotics control of Bangladesh before. Out of eight samples, one was of a Brand company (Keru & Co) and others homemade. Highest concentration, 81.56% was observed in Spirit followed by 37.7% in Dochuani and lowest 2.2% in Tari. Insignificant amount of heavy metal detected in Dochuani and Tari. There was no viable micro-bacterial contamination in samples tested. Conclusions: Without knowing the strength, people are using different types of homemade alcoholic beverages as such in a risk of health hazards as well as death. The pilot study HWI-01 \"Antibiotic vs Ibuprofen for uncomplicated urinary tract infection\" was a noncommercial, double-blind clinical trial, assessing the clinical equivalence of a symptomatic treatment with ibuprofen compared to antibiotic treatment for uncomplicated urinary tract infection (UTI), and was conducted in 2007/08 according to current good clinical practice-guidelines in German family practices. Due to extensive regulatory requirements, a clinical drug trial is a great challenge for participating physicians and their teams. To optimize the planning and implementation of further randomized- controlled drug trials in a family medicine setting, views and experiences of participating family practitioners (FP) were explored subsequently in this qualitative interview study. Methods: After close- out of the HWI-01 study, semi-structured interviews were conducted with the FPs who acted as trial investigators. The interview guideline included the areas of general motivation for participation, experience with patient recruitment as well as with study procedures in the practice. Interviews were digitally recorded and transcribed verbatim. The evaluation was carried out by content analysis. Results/ Conclusions: Interviews were conducted with 20 family physicians in Lower Saxony, Germany. Aspects concerning physicians´ motivation to participate, patient recruitment and practical aspects of trial implementation could be elucidated. Treatment in women with PCOS is commonly symptomatic. Positive effects of acupuncture on menstrual frequency and androgenic hormone levels have been demonstrated, as well as in some diseases with endocrinological disorders such as PCOS. Here, a patient who had presented with menstrual irregularity and infertility and had regular menstruations and pregnancy after acupuncture treatment is presented. \u0000Case Report: A 32-year-old female presented to receive acupuncture treatment for menstrual irregularity, obesity, and infertility. She was diagnosed with PCOS ten years ago. The patient had menstrual irregularity for five years and amenorrhea for the last one year, and discontinued using combined oral contraceptives due to weight gain. Her body mass index (BMI) was 32.9 kg/m2. Her laboratory values were FSH: 4.34 mIU/ml, LH: 5.15 mIU/ml, and TSH:1.87 uIU/ml. Patient was given 24 acupuncture treatment sessions twice a week in three months. Bilateral we selected LI4, ST36, SP6, GB34, CV5, CV6, CV12, GV4, Yintang, Taiyang acupuncture points, also ear shenmen, ear gastric point. The patient started to have regular menstruation during her follow-up. She lost 8 kg in three months and her BMI was 29.7 kg/m2. She became pregnant after two months and could not continue acupuncture treatment. \u0000Discussion: Acupuncture is thought to provide ovulation through its effects on the neuroendocrine system, metabolism, and ovarian blood flow in anovulatory conditions, such as PCOS. In addition, it provides good results for infertility through its effects on immune function and mood. {"title":"Fatal Nitrofurantoin-Induced Pulmonary Toxicity: A Potentially Preventable Medical Error","authors":"C. It is known for its safety, efficacy, tolerability and cost-effectiveness. Rarely, nitrofurantoin can cause acute and chronic lung and liver diseases. Acute pulmonary toxicity due to nitrofurantoin is more common than its chronic form and is probably a hypersensitivity reaction to the drug. In contrast, the chronic pulmonary toxicity appears to be due to cumulative drug exposure leading to chronic inflammation and fibrosis and is therefore an irreversible process. What predisposes to the rare and severe form of acute pulmonary toxicity is unknown. This report describes a rare presentation of a case of HSTL with left shift leukocytosis, thrombocytosis, mild hepatomegaly along with an initial presentation of acute fulminant hepatic failure and bone marrow picture further confusing with chronic myelomonocytic leukemia(CMML). Contrary to the initial suspicion of hepatic infiltration of CMML, ultrasound guided liver biopsy showed leukemic infiltration into hepatic sinusoids with minimal cholestasis. Specific immunostaining of bone marrow revealed predominance of CD3+ T cells with negativity for other cell surface markers. Genetic analysis of bone marrow demonstrated trisomy 8 and isochromosome 7q, thus strongly suggesting hepatosplenic T-cell lymphoma (HSTL). This study aimed at assessing the extent of the use of CAM among adult patients attending the Family Medicine Clinic of University of Uyo Teaching Hospital, Uyo, Nigeria. \u0000Methods: This was a cross sectional descriptive study involving 574 consenting adult subjects aged 18-69 years, recruited over a 6-month period, using a systematic sampling method. An Interviewer administered Questionnaire was used for data collection. Data was analyzed using the 17th version of the Statistical Package for Social Sciences. \u0000Results: Of the 574 subjects studied, two thirds were female 62.9% (n=361). A total of 19.5% men (n=112) and 44.3% (n=254) women respondents respectively, were found in this study to have used CAM (p=0.000). Married respondents used CAM remedies more frequently than the non-married (p=0.000). There were no statistically significant relationships between place of residence (p=0.007), level of education (p=0.052) and CAM use. Low and middle level income earners constituted the majority of CAM users here in this study (p=0.000). Herbal remedies were the most frequently used preparations among the respondents accounting for 61.7%. There was statistically significant relationship between CAM use and the presence of hypertension (p=0.001), diabetes mellitus (p=0.001) and osteoarthritis (p=0.001) among respondents in this study. \u0000Conclusion: CAM use is common among middle aged respondents especially the women, married and in middle and low income groups. Lack of standardization of herbal preparation as well as the potential for hazardous drug-drug interactions constitutes the major drawback regarding this choice of therapy. Diagnosis is often complex and in some cases is purely casual discovery. In cases in which these are symptomatic is postural abdominal pain located it´s first expression. After the anamnesis facing possible predisposing factors such as trauma or the concurrence of processes involving increased abdominal pressure and palpation is the main diagnostic instrument. Ultrasound or tomographies are useful to diagnose when tenderness is not enough. The request for additional tests without a diagnosis or suspicion can result in false negatives that slow down the diagnostic process. Strangulation and subsequent surgical intervention was the reason that changes the diagnostic process after months of study and high direct and indirect costs. In recent years the approach by laparoscopic way has shown comparable performance and shorter hospital stay. Successful diagnostic and therapeutic action, by doctor the emergency room, helped the patient returning to work in just two weeks; recovering completely the quality of life that had before trauma.
