Omphalocele pathogenesis is not clear but familial and genetical factors plays important role to development of omphalocele. Twenty years old, 19 weeks 4 days pregnant, primigravid patient presented. In her ultrasonographic assessment omphalocele was detected and fetus was death. Pregnancy was terminated and genetic studies were performed from fetal blood and tissue biopsy. Karyotype from intracardiac blood sample was found 46, XX. QF-PCR analysis performed from the DNA obtained from fetal skin biopsy; 4:1 peak ratio observed in all the STR regions. This is the first case shows 4:1 peaks at QF-PCR. This finding was incompatible with karyotype. So mosaicism doubt occurred. FISH from skin biopsy showed diploid/triploid mosaicism. Fetuses with omphalocele must be evaluated for additional anomalies and genetic consultation. Our case indicates high frequency of mosaicism and the importance of examination from different tissues with different types of genetic methods. Triploid cell line is found only in fibroblasts in about 70% of cases, so this mixoploidy syndrome is likely underdiagnosed. {"title":"The Relationship between Xenoestrogen Exposure and Early Puberty among Young Females Living in Jeddah, Saudi Arabia","authors":"A. Xenoestrogen have been claimed to affect the process of puberty among young females. \u0000Purpose: This study aimed to investigate the relationship between exposure to various xenoestrogen products and the factor of early puberty among younger females in Jeddah, Saudi Arabia. \u0000Method: This cross-sectional study was conducted in Jeddah for the time frame of one month. The study selected 568 younger females from different areas of Jeddah. Pubertal staging was done using the Tanner staging. The relevant data was collected through a questionnaire. Pearson’s Correlation Coefficient did data analysis. \u0000Results: There was a significant association among the use of children toys, age of breast (p-value = 0.040), and pubic hair development (p-value=0.028). The consumption of children plastic bottles also showed a significant correlation with early pubic hair development (p-value=0.048). The increased frequency usage of water plastic bottles, plastic packaging materials, women cosmetics and food preservations showed an inverse effect on pubertal signs. \u0000Conclusion: This study confirmed the association between using xenoestrogen products and early puberty in young females living in Jeddah, Saudi Arabia. The study has evaluated that different association between the use of xenoestrogen products and the occurrence of early puberty was observed among the selected population. A questionnaire consisted of 63 items in 4 sections was used to assess sociodemographic properties, cardiovascular risk factor analysis, LC awareness, effects of HT. \u0000Results: A total of 301 patients with a mean age of 59.6 ± 10.9 years were included. The rate of blood pressure (BP) under control was 48%. Awareness rates of 6 basic LC were diet at 18%, alcohol at 48%, cigarette smoking at 50%, exercise at 45%, obesity at 65%, salt restriction at 91%. From the total patient group 44% were aware of 4 or more LC, frequency of LC awareness was greater in males (54%) than females (40%) (p=0.02). LC awareness was seen not to affect BP control (p=0.4). The patients were found to have a high level of awareness about target organ damage in HT. While patients were aware of the necessity of a salt-free diet, they did not know how much salt should be restricted each day and while they were aware that they should exercise, they did not know what kind of exercise or for what duration. Patients who applied 4 or more of LC were determined at a rate of 71%. Other measures included Body Mass Index, body fat percentage, and aerobic threshold (VO2). \u0000Results: Significant relationships were found between sleep patterns based on gender, age, and day-to-day activity associations. \u0000Conclusions: It was determined that children who were more physically active slept less at night. A second aim was to study whether there is any connection between deviant levels of TSH and mental stress or between diagnosed thyroid disease with normalized levels of TSH and mental stress. Design: Prospective study. Setting: Goteborg, Sweden, population about 4,30,000. Subjects: This study is based on material from the Population Study of Women in Gothenburg. The participants are two representative samples of 38- and 50-year-old women who took part in examinations 1980-1981 and 2004-2005. Main outcome measures: Prevalence of thyroid disease as well associations between mental stress and thyroid disease, studied cross-sectionally. Results: The prevalence of thyroid disease has increased in both 38- (with 39%) and 50-year-old (with 56%) women between 1980-1982 and 2004-2005. There was a significant increase in the prevalence, from 0.9% (CI 0.2-2.6) to 3.4% (CI 1.7-6.2) of 50-year-old women with suppressed TSH. No connection was found between high mental stress and deviant levels of TSH. In the 38-year-old women there was a significant connection between high mental stress and diagnosed thyroid disease. Conclusion: The results indicate that the prevalence of suppressed TSH in Swedish women has increased since the early eighties, and that mental symptoms can exist also in successfully treated thyroid disease. Case 1 rejected exposure. Although Case 2 engaged in Cognitive Behavioural therapy, it had a side effect that his obsession intensified. Case 1 could shorten her bathing time and engaged in occupation of taking care of animals. After Case 2 engaged in works with other patients, he could live with obsession and was aware of anger towards his parents. We report a case of 40 year old patient on hemodialysis who had 2 uncomplicated pregnancy 6 years apart with delivery of full term healthy babies. The fertility and conception in patients with renal failure is reviewed. The reported pregnancies and their outcome from different countries in patients on dialysis showed that majority result in abortion, still birth or preterm babies. The care of a pregnant dialysis patient should involve increased dialysis hours and special care to dialysis bath and heparin use. Pulmonary hypertension represents one such comorbidity which, when present, worsens prognosis and confounds approach to treatment. Sildenafil is a pulmonary vasodilator, which has recently generated considerable interest for use in patients with diffuse parenchymal lung disease and concomitant pulmonary hypertension. We wondered whether acute administration of oral sildenafil affects the diffusion capacity measurement, which is a tool for serially monitoring patients with diffuse parenchymal lung diseases. \u0000Methods: 15 patients with diffuse parenchymal lung disease and pulmonary hypertension (WHO Class III) had diffusion capacity measurements and 6 minute walk test performed before and after receiving sildenafil 20 mg orally. CT scans of the chest were scored to determine burden of interstitial lung disease. \u0000Results: The mean change in DLCO after administration of oral sildenafil was -0.26 ± 0.94 ml/min/mmHg (p=0.30) and -0.41 ± 2.84 % predicted (p=0.58). The average 6 minute walk distance at baseline was 363.28 ± 141 meters. After oral administration of sildenafil, 6 minute walk distance was 369 ± 116 meters (p=0.63).
