Chorangioma of placenta with high risk pregnancy: A case series 高危妊娠胎盘脉管瘤:一个病例系列

Tumors larger than 5 cm are associated with maternal and fetal complications. We report four cases of chorangioma that were identified in mostly primigravida (75%) with a mean gestational age of 33 wks (Range 28-33 wks). All except one case, complete placenta was received with mean weight of 550 g (Range 400-800 g) and tumor size of 9.4 cm (Range 0.5-23 cm). Microscopy showed capillary proliferation resembling fetal vessels in enlarged villi covered by trophoblastic epithelium. Three cases had preterm delivery. One also presented with abruptio placenta and intrauterine fetal death. Because of limited documentation of cases and dismal prognosis of these pregnancies, especially with large chorangiomas, it’s of utmost importance to correctly diagnose them with the aid of color Doppler flow. In this series, 75% of cases (3/4), babies survived probably due to small size of tumor and attainment of fetal maturity. Keywords : Chorangioma, placenta, placental tumor, vascular tumor Key Messages : Even though chorangioma is the most common tumor of placenta, tumors arising from placenta are rare. Because of dismal prognosis of these pregnancies with large lesions, it’s important to correctly diagnose them with the aid of color Doppler flow. Approximately, 60‑80% of women with polycystic ovarian syndrome (PCOS) demonstrate elevated circulating androgen levels. Aim : This prospective observational randomized study was to investigate the likelihood of using the salivary luteinizing hormone (LH), free testosterone (FT) and dehydroepiandrosterone sulfate (DHEAS) levels instead of serum values in diagnosing biochemical hyperandrogenemia in women with PCOS. Subjects and Methods : This study was conducted on 75 women having PCOS in addition to 20 normal fertile women (control group). Venous blood and salivary samples were taken in the 3 rd day of the cycle to measure LH and FT and DHEAS levels. Results : Biochemical hyperandrogenemia prevails in 40% of women with PCOS. Salivary levels of LH, FT and DHEAS correlate with their corresponding serum values, with a higher sensitivity of salivary more than serum approach. Conclusion : Saliva provides a sensitive, simple, reliable, non‑invasive and uncomplicated diagnostic approach for biochemical hyperandrogenemia. Endometrial tissue can be rarely identified on abdominal wall following numerous surgical or procedure scars, including cesarean section and hysterectomy. Cytological diagnosis of scar endometriosis is difficult, as it can be easily misinterpreted as benign spindle cells tumor. We report three cases where patients presented with a chief complaint of nodule in parietal abdominal wall adjacent to previous cesarean section scars and associated with history of cyclical pain. Provisional clinical diagnosis of desmoid tumor was made in one patient while the other two were diagnosed as endometriosis and they were referred for Fine needle aspiration cytology which revealed endometrial‑like epithelial cells, stromal cells and hemosiderin‑laden macrophages. Death from breast cancer is usually a result of chemotherapy‑resistant metastatic disease. Eribulin mesylate is a recent addition to the therapeutic armamentarium for treating locally advanced or metastatic breast cancer (MBC) in patients who have received at least two prior chemotherapy regimens for late‑stage disease. This synthetic analog, derived from a marine sponge macrolide halichondrin B, inhibits microtubule stability by blocking microtubule growth without affecting microtubule shortening. The US Food and Drug Administration has approved eribulin mesylate as a third‑line treatment for MBC refractory to anthracyclines and taxanes based on a Phase III clinical trial showing significantly increased overall survival compared to treatment of investigator’s. Asthenia, fatigue, neutropenia, alopecia, nausea, anorexia, and neuropathy are the most frequent adverse effects associated with this drug. The aim of this review was to highlight the importance of this drug in the management of breast cancer. Medline, Excerpta Medica database, cochrane database, medscape, Elsevier Scopus, and clinicaltrials.gov were searched using terms “eribulin,” “E7389,” “halichondrin,” “metastatic breast cancer.” Journal articles published from 2007 to 2012 discussing pharmacology and/or clinical trials were screened. Aim: To ascertain the level of awareness about folic acid, its use during peri‑conceptional period and the predictive factors of folic acid awareness among pregnant women in our setting. Subjects and Methods: Confidential, anonymous questionnaires were administered to consecutive 543 pregnant women seeking routine antenatal care at Bingham University Teaching Hospital, Jos between January and June 2012. Questions included enquiry on their demographic features, obstetric history, pregnancy intention, awareness and intake of folic acid during the peri‑conceptional period, and knowledge of its natural sources. Descriptive statistics and multivariate logistic regression to identify predictive factors of folic acid awareness were then performed. Results: Out of 543 pregnant women surveyed, 64.6% (351/543) reported that they were aware of folic acid as a vitamin supplement. However, only 7.4% (26/351) consistently took the vitamin during the protective periconceptional period. The common sources of information about folic acid were health workers (68.5%) and the media (14.6%). Only 26.5% (93/351) of them could correctly identify a natural source of food rich in folic acid. Multivariate logistic regression showed that maternal age >30 years (P=0.01) and higher educational status (P=0.001) were predictive factors for folic acid awareness. Conclusion: Folic acid awareness as a vitamin supplement was relatively high among the women surveyed but its intake during peri‑conceptional period was worryingly low. Antenatal and Population health educational strategies and fortification of staple foods with folic acid may improve its intake among women of child‑bearing age in our setting. Objectives: To determine the mode of presentation and management of abdominal pregnancy, from cases in the Usmanu Dan-Fodiyo University Teaching Hospital (UDUTH), Sokoto. Materials and Methods: A retrospective review of the case records of all abdominal pregnancies managed in UDUTH, Sokoto, between January 2000 and December 2010. Results: During the 10-year study period, there were eight cases of abdominal pregnancies, 285 extrauterine pregnancies, and 25,506 total deliveries. All the patients were not booked for antenatal care and their ages ranged between 20 and 39 years (mean=28.1±3.4 years). The majority (88%) were grand multipara. The main presenting symptoms were persistent abdominal pain, vaginal bleeding, and prolonged pregnancy. Seven cases (87.5%) were diagnosed accurately by an ultrasound scan. There were seven perinatal deaths (87.5%) and one live birth. There was one maternal death (12.5%), which occurred in the patient whose placenta was left in-situ. Conclusion: Presence of persistent lower abdominal pain, vaginal bleeding, and prolonged pregnancy should raise the suspicion of abdominal pregnancy. However, the true underlying causes remain to be fully elucidated. Aim: This study aimed at searching for any relation between first trimester uric acid concentration and the development of GDM. Subjects and Methods: The study was conducted on 250 first trimester pregnant females at risk of diabetes mellitus attending the outpatient clinic of Tanta University Hospital. All cases underwent estimation of first trimester-fasting blood sugar and maternal serum uric acid concentration. Between 24 and 28 weeks' gestation random blood sugar and glucose challenge test were done. Positive cases were confirmed by 3 h glucose tolerance curve. Results: The results demonstrated an association between first trimester maternal serum uric acid concentration obesity and GDM. Approximately, 41.4% (60/145) of non-diabetic women were at first quartile, while 44.8% (47/105) of the diabetic women were at fourth quartile. Conclusion: We concluded that the cut-off level of maternal serum uric acid of 4 mg/dl in the first trimester was associated with developing GDM. The classical sterile male-techniques were based on the sterilization of male insects by irradiation, which were found to cause the rise of dominant lethal mutations that affect either entire chromosomes or individual chromatids.
