Although many scientific advances have been achieved over the last few months, the knowledge about how the etiologic agent SARS-CoV-2 interacts with the host's immune system, regulating the pathogenesis and the outcome of the disease, is not yet fully understood. Objective: This is a narrative literature review to discuss the available evidence on modes of transmission and the main immunopathological mechanisms involved in SARS-CoV-2 infection. Material and method: A search in the indexed journals was carried out in the PubMed/MEDLINE database, during the period from January to May 2020. The following descriptors were used in the research: Covid-19, SARS-CoV-2, transmission, immunity, immunopathogenesis, and pathogenesis. Results and discussion: Transmission of SARS-CoV-2 occurs through contact with airway secretions from a symptomatic patient. However, scientific evidence encourages transmission by asymptomatic and/or oligosymptomatic patients. Immunopathological studies suggest that the hypercytokinaemia promotes lung tissue damage and, subsequently, organ and system involvement, leading to decompensation, organ dysfunction and death. Regarding the humoral response, the high titers of B lymphocytes and antibodies, traditionally, correlated with the protection of the host, may be associated with the severity of the disease by SARS-CoV-2. Conclusion: The host's immune response is determinant in the pathogenesis of Covid-19. Objective: The objective of this study was to evaluate the performance of point-of-care rapid tests (RT) used for HIV diagnosis as compared to conventional tests in blood samples from a population of transvestites and transsexual women. Methods: A total of 1,385 samples with HIV RT results were submitted to Instituto Adolfo Lutz for evaluation by conventional laboratory tests. Results: HIV positivity was confirmed by conventional laboratory tests in 100% of the 240 (17.3%) samples with reactive RT. However, 1,145 (82.7%) samples with non-reactive RT results were evaluated; three (0.3%) tested positive and one (0.1%), indeterminate for HIV. Although a proportion of discordant results were found, the point-of-care RTs showed a high confirmation rate when compared to the serological tests. Material and method: A retrospective, analytical and cross-sectional test was performed. The sample consisted of 39 medical records of women who underwent chemotherapy in the period from January to December 2016. The effects of hematological toxicities were evaluated according to erythrogram, leukogram, and platelet parameters, in relation to biochemical toxicities, using levels of serum marker (urea, creatinine, transaminases). Results: It was evidenced that among the chemotherapy protocols used, cisplatin showed a leukogram and plateletgram of 1.06 and 2.09 chances, respectively, for the development of leucopenia and thrombocytopenia during treatment. {"title":"TRH stimulation test in patients with repeatedly elevated TSH and normal\u0000 FT4","authors":"P. We aimed to evaluate the thyrotropin-releasing hormone (TRH) stimulation test in patients with repeatedly elevated TSH (up to 10 mIU/l) and normal FT4, but without apparent thyroid disease. Women with TSH > 4.5 and ≤ 10 mIU/l (in two measurements) and normal FT4 were selected. Women with a known non-thyroid cause of TSH elevation, those treated with anti-thyroid drugs, amiodarone, lithium, and those with a history of thyroidectomy, neck radiotherapy and 131 I treatment were excluded. Seventy women had negative antithyroperoxidase antibodies. Ultrasonography revealed a eutopic thyroid, usual echogenicity, and a volume ≤ 15 ml, and they underwent the TRH stimulation test during initial evaluation. After stimulation with TRH, TSH > 30 mIU/l was observed in 38 women (expected response), while 32 women had TSH < 20 mIU/l (inadequate response). Age, basal TSH or thyroid volume did not differ between both groups, but FT4 concentrations were significantly lower in the first group. Follow-up was available for 66/70 women. Seven women developed a need for levothyroxine, all of them in the group with an adequate response to TRH [7/36 (19.4%) versus 0/30].
