The predominant cause of death after TBI is brain edema which can be quantified by non-invasive diffusion-weighted magnetic resonance imaging (DWI).</p><p><strong>Purpose: </strong>To provide a better understanding of the early onset, time course, spatial development, and type of brain edema after TBI and to correlate MRI data and the cerebral energy state reflected by the metabolite adenosine triphosphate (ATP).</p><p><strong>Material and methods: </strong>The spontaneous development of lateral fluid percussion-induced TBI was investigated in the acute (6 h), subacute (48 h), and chronic (7 days) phase in rats by MRI of quantitative T2 and apparent diffusion coefficient (ADC) mapping as well as perfusion was combined with ATP-specific bioluminescence imaging and histology.</p><p><strong>Results: </strong>An induced TBI led to moderate to mild brain damages, reflected by transient, pronounced development of vasogenic edema and perfusion reduction. Heterogeneous ADC patterns indicated a parallel, but mixed expression of vasogenic and cytotoxic edema. Cortical ATP levels were reduced in the acute and subacute phase by 13% and 27%, respectively, but were completely normalized at 7 days after injury.</p><p><strong>Conclusion: </strong>The partial ATP reduction was interpreted to be partially caused by a loss of neurons in parallel with transient dilution of the regional ATP concentration by pronounced vasogenic edema. The normalization of energy metabolism after 7 days was likely due to infiltrating glia and not to recovery. This complication is extremely rare, presents dramatically, and is usually fatal. The patient was successfully treated with a stent graft deployment in order to achieve immediate hemostasis maintaining blood flow to the leg. To the best of our knowledge, two prior cases of this kind of herniation have been described. One of these cases was with associated symptomatology and other abnormalities, and the other was characterized as idiopathic. Herein, we report a case of a 14-year-old girl with HGA whose whole-body magnetic resonance imaging (MRI) disclosed an unusual picture characterized by small, widespread punctuate millimetric nodules, hypointense on T1-weighted and hyperintense on STIR sequences. However, many of these patients have a metallic stent in the bile duct, which is recognized as a contraindication for IRE ablation. We report a case in which an IRE ablation in the region of the pancreatic head was performed on a patient with a metallic stent which led to severe complications. After the treatment, the patient suffered from several complications including perforation of the duodenum and transverse colon, and bleeding from a branch of the superior mesenteric artery which eventually lead to her death. This is a well-known technique that can be used in the treatment of, for example, inoperable pancreatic cancer. In this case, the patient had a metallic stent in the bile duct, which is strictly regarded as an absolute contraindication in doing such procedures. This procedure was, nevertheless, performed at a private institution, and when the complications became obvious, the patient was transferred to a university clinic and treated, unfortunately, with a fatal outcome. \u0000 \u0000As interventional radiologists, we all realize the importance of knowing when to do, or more importantly, when not to do/perform special procedures. It is (or should be) common knowledge to most of us that procedures as described in this manuscript should not be done due to the severe, and in this case fatal, outcome connected to the metallic stent implanted in the area of the tumor to be treated. Instead, this procedure was done, with the described fatal outcome as a result. \u0000 \u0000The radiological community has to take responsibility if someone is doing procedures that are regarded as contraindicated, in this case by both the producer of the equipment and by most colleagues performing such procedures. When some of our colleagues, regardless of warnings, decide to perform such a procedure, this should be focused upon in the scientific literature and make clear to all of us the responsibility of choosing appropriate treatment methods. This procedure was done in a private clinic and one can only speculate if the reason for doing this was based on economy or lack of knowledge. We all have a responsibility of securing a safe treatment to our patients as interventional radiology now has become a very important part of the broad specter of methods in treating patients. \u0000 \u0000Reporting treatment results are important; however, reports of treatment failures are equally important, and not at least to focus on indications, and not at least contraindications of interventional procedures. The present article is a good example of the importance of reporting treatment failure and most importantly when not to perform certain kinds of procedures. The focus should be kept on strict information of indications/contraindications and treatment successes/failures.
