Thirty patients of age group 20-61years with abnormal uterine bleeding were included. Demonstrable pelvic pathology like cancer of cervix, vagina or endometrium and active pelvic inflammatory disease (PID) were excluded. All patients were assessed using hysteroscopy and findings were correlated with histopathology. \u0000Results: On hysteroscopy, endometrium was normal in 30%, hyperplastic in 30%, polyps found in 20% and atrophic in 10%. Endometrial polyp and submucous fibroid was seen in 6.7% cases and 3.3% cases each. On confirmation by histopathology the findings were: normal endometrium in 40% cases, hyperplastic in 40%, polyp in 16.7% and atrophic in 3.3%. Randomization of the cases was irrespective of age, parity, indication and elective or emergency and was performed by two surgeons. Total of 150 cases were considered in the study and 75 cases performed by each surgeon were selected. One surgeon performed the surgery as intrabdominal repair while the other exteriorized the uterus for closure. Descriptive parameter and t-test were used. \u0000Results: Intraoperative complications like intraoperative nausea, vomiting, pain were more commonly associated in cases in which uterus was exteriorized. In regards to severity of blood loss in either group there was no significant difference in either group (1.28±1.00, 1.23±0.98). One hundred thirty patients undergoing total laparoscopic hysterectomy were categorized in two groups with and without previous caesarean section. The data were entered into Microsoft Excel and analyzed by using statistical package for social studies (SPSS 23.0). \u0000Results: The overall complication rate was 3.87% (5/130) with 4.61% (3/65) in previous cesarean section and 3.07% (2/65) without it. Urinary tract injury was the common injury. Radhakrishnan","doi":"10.3126/njog.v15i1.29341","DOIUrl":"https://doi.org/10.3126/njog.v15i1.29341","url":null,"abstract":"Aims: To study the association of clinical profile of patients with Abnormal Uterine Bleeding with the different endometrial histopathological patterns Methods: A retrospective cross-sectional study was done in the department of Obstetrics and Gynaecology at a hospital in Karnataka, India from January 2017 to January 2019. Medical records were reviewed for all the cases of abnormal uterine bleeding who underwent endometrial biopsy. Results: There were 148 records of endometrial biopsies; >55% were in 41-50 years age group; 28% had postmenopausal bleeding; and 28% presented with menorrhagia. Menorrhagia and menometrorrhagia were equally common under 50 years of age. The various physiological patterns of endometrium (proliferative, secretory & menstruating endometrium) were noted in more than half the cases (51%). Proliferative endometrium was in 37%, followed by hyperplasia without atypia in 22% and only 3% showed malignancy; all of whom were in the postmenopausal group. Even in women with post menopausal bleeding, proliferative pattern was the commonest (33%), followed by atrophic endometrium and malignancy. Atypical hyperplasia was found to be most commonly associated with post menopausal bleeding. Conclusions: Both physiological and non physiological endometrial patterns were seen almost equally among the AUB cases. The knowledge of endometrial pattern in a case of AUB helps to decide a plan of management. In most of the cases fistula results mainly due to poor obstetric care in developing countries, where as in developed countries most of the urogenital fistulas are iatrogenic. It is mainly diagnosed by clinical examinations and intravesical dye test using methylene blue. Repair can be done via abdominal or vaginal route based on the findings. Pregnant women in between seven and ten weeks of gestation were enrolled. Transabdominal sonography was performed to measure yolk sac diameter and followed up till 12 weeks of gestation to see the outcome. Sensitivity, specificity, positive predictive value and negative predictive value were calculated for yolk sac diameter with regards to abnormal pregnancy outcome. \u0000Results: There were 80 cases enrolled and a significant positive correlation was found between yolk sac diameter and gestational age. Abnormal yolk sac diameter, irrespective of gestational age, had sensitivity of 92.95%, specificity 66.66%, positive predictive value 95.6% and negative predictive value of 54.54% in predicting abnormal pregnancy outcome. \u0000Conclusions: The measurement of yolk sac diameter between seventh and tenth weeks of gestation can predict first trimester pregnancy outcome. Methods: This is a cross-sectional study of treatment of stress urinary incontinence using trans-obturator tape at Kathmandu Model Hospital from February 2018 to December 2019. The outcome of the procedure in terms of improvement in quality of life and surgical complications were assessed. Results: Out of 16 cases of stress urinary incontinence half of them were post-menopausal. One each had intra-operative bladder injury, urinary retention and urinary tract infection; 14 cases improved completely by a week, one had some improvement and one didn’t improve. At six months follow-up one case had no improvement and one developed urge incontinence.
