We are presenting a case of asymptomatic huge subserosal pedunculated leiomyoma in a 37 years unmarried nullipara lady diagnosed as highly vascular abdominopelvic mass preoperatively as suggested by imaging studies but of uncertain origin, imposing surgical challenge due to diagnostic dilemma and its vascularity. Laparotomy revealed solid mass of 26 x 24 cm occupying whole abdomen covered with torturous dilated serpentine vessels derived from omentum. The mass was arising from the fundus of bulky uterus with small pedicle suggesting suserosal pedunculated leiomyoma. Myomectomy with infracolic omentectomy with round ligament plication was done and her post operative recovery was smooth. Here is a case report of 25 years G2P1 lady who presented with cervical pregnancy at 8 weeks and 5 days period of gestation and underwent manual vacuum aspiration under ultrasound guidance. Kc","doi":"10.3126/njog.v15i2.32907","DOIUrl":"https://doi.org/10.3126/njog.v15i2.32907","url":null,"abstract":"Aims: To find out the occurrence of genital tuberculosis in submitted samples of female genital tract, histopathological changes and age distribution. \u0000Methods: A retrospective study of 300 cases of extrapulmonary tuberculosis out of which 11 are of female genital tract based on laboratory record which was done from 2011 to 2018 at Patan Academy of Health Science, Lalitpur, Nepal. Descriptive analysis performed using MS Excel and SPSS 16. \u0000Results: Out of 30,000 biopsies 300 cases were of extrapulmonary tuberculosis. Out of 300 extrapulmonary tuberculosis females affected were 61%; out of these female patients 3.6% (n=11) were of female genital system. Maximum numbers of the cases lie in the age group 30-39 years. Lymph nodes comprised of 58%, followed by Gastrointestinal and Skin in 10% each. Out of all female genital tuberculosis 27% was seen in endometrium while majority (73%) was seen in tubo-ovarian complex; all of them had epithelioid granuloma and 54% had caseous necrosis. Ziehl Neelson stain done for acid fast bacilli was positive in 2 cases (18%). \u0000Conclusions: The female genital TB constituted 0.04% of total biopsies with 3.6% samples of female patients of total extrapulmonary tuberculosis. The commonest sites affected by tuberculosis were seen to be fallopian tube, ovaries and endometrium. Standardized and pre validated questionnaires “Brief Pain Inventory” and “Visual Analogue Scale” were used for subjective rating of pain. Result expressed in frequency and percentage. \u0000Results: Musculoskeletal pain syndrome was present among 99% women and most common complaint was low back pain (74%). Data were collected and analyzed. \u0000Results: Total 104 female patients were interviewed with their mean age of 30.1±12.7 (range: 11-70) years. 85.6% showed the regular brushing and 96.2% used the tooth brush and paste. 79% of the female brushes twice daily. Periodontal index and DMFT index showed good results. \u0000Conclusions: Despite of tooth brushing practice the prevalence of periodontal disease and DMFT were significant. This could be associated with many other factors. Data were collected from hospital record and descriptive analysis performed using MS Excel. \u0000Results: Total of 1536 women delivered in three month and caesarean section rate was 16.7% (257/1536). Previous caesarean section was the major contributor (46%) to the overall caesarean section rate. \u0000Conclusions: Maternity services were provided during COVID 19 pandemics and Lockdown. Pregnancy after 28 weeks without pre-existing medical disorder and fetal anomaly were studied. The data were analyzed to find out the association between adequate antenatal care utilization and severe maternal and severe perinatal morbidity by using chi-squared test. \u0000Results: Among 500 participants, 243 (48.6%) had adequate visits. This study is an attempt to assert the perinatal mortality status for the year 2016/2017 A.D. of Shree Birendra Hospital, a tertiary hospital. This study becomes imperative to comprehend the various shortcomings in the maternal and child health care, so as to remedy them effectively. \u0000Methods: This is a hospital record-based study conducted at Shree Birendra Hospital, Kathmandu, a tertiary care facility of the Nepal Army. The record of all perinatal death from 13th April 2016 to 12th April 2017 was reviewed. \u0000Results: Out of 1051 total births, there were 12 perinatal deaths with perinatal mortality rate of 11.42 per thousand births. Perinatal death was more prevalent in the age group 20-25 (58.33%), multiparous female (60%), period of gestation between 35-40 weeks (58.34%), male gender of baby (66.67%) but equal number of deaths was seen in both vaginal delivery and emergency LSCS. According to the Wigglesworth pathophysiological classification to classify perinatal deaths, conditions specific to neonate (33.3%) constituted as a significant cause perinatal death followed by stillbirths or neonatal deaths with congenital anomalies (25%) and conditions associated with prematurity (25%). \u0000Conclusions: Neonatal sepsis contributed to the major burden of perinatal mortality. Practicalities describing relevant routine factors that impact health care delivery and medico-legal litigation are outlined with suggestions on how they may be effectively implemented. \u0000Conclusions: The improved knowledge of healthcare professionals on the up- to-date Clinical Guidelines along with the law of Land concerning medico Legal issues will be constructive in health service delivery by reducing the amount of litigation in Obstetrics and Gynecology. Appropriate use of informed consent, counseling, documentation and proper record keeping is a top priority. Telephone consultation and Telemedicine may attract various liability issues. Hospital administration policy of risk management including regular audits, near-miss investigation, mortality-morbidity discussions, and timely reviews of Clinical guidelines will help reduce litigation in these fields.
