Study participants included pregnant women visiting the antenatal clinic during 2nd and 3 rd trimester conducted from 21 st September 2018 till 20 th September 2019. Data were collected by using an interviewer-administered questionnaire. Ethics approval was granted by the research Ethics Board of health, Bhutan. \u0000Results: Total 450 parturients participated in this study; 61.4% of the participants knew that labour epidural services were available and 37.5% got information through media. A total of 57.3% of respondents had severe fear about labour pain; 62.4% had severe fear about delivery complication; 85.2% thought that epidural labour analgesia was a good method of pain relief; and 67% were willing to accept it for their current pregnancy. WHO recommended CS rate of 10-15% at populational level would not be the ideal rate at the hospitals level due to the differences on population they have been serving. At the hospital level, a perfectly effective system is necessary to understand the trends and causes of rising trends of CS as well as to implement effective measures where necessary to control the same. Hence, WHO recommended the Robson classification, which is also called the 10-group classification of CS (TGCS) as a global standard tool to assess, monitor and compare CS rates within healthcare facilities over time, and between health facilities. The Robson classification, proposed by Dr Michael Robson in 2001, is a system that classifies all women at admission at a specific health facility for childbirth into 10 groups based on five basic obstetric characteristics (parity, gestational age, onset of labour, foetal presentation and number of foetuses). This classification is easy and simple and mutually exclusive, highly reproducible, easily applicable, and useful to change clinical practice. It has many strengths such as simplicity, flexibility (further subdivisions can be made to increase homogeneity within groups). This classification helps to identify and analyse the contribution of each group to overall CS rates. The sample was taken from an antenatal outpatient department of Nobel Medical College Teaching Hospital. A total of 300 pregnant women was selected by using non- probability consecutive sampling technique. Data was collected by using standard tool, Pregnancy Stress Rating Scale (PSRS) and Brief Cope. Data analysis was done by descriptive and inferential statistics. \u0000Results: The study revealed that 40.7% of the respondents experienced a high level of stress and 84.0% of respondents had used adequate coping strategies where “self- distraction” was the most used coping strategy by the respondents with a mean score (2.99±.56). Stress and coping strategies had positive mild correlation (r=0.040). \u0000Conclusions: More than one-third of the respondents had experienced a high -level stress and the majority of the respondents had used adaptive coping strategies. Tripathi","doi":"10.3126/njog.v15i2.32905","DOIUrl":"https://doi.org/10.3126/njog.v15i2.32905","url":null,"abstract":"Aims: To compare the incidence of intra uterine fetal death and its causes at an interval of five years. \u0000Methods: This retrospective study was conducted to compare the incidence of intrauterine fetal death and its causes during 2010 and 2011 versus 2016 and 2017. \u0000Results: A total of 4219 deliveries conducted in the year 2010 and 2011, of them 90 (2.13%) were intrauterine fetal death. Likewise altogether 5873 deliveries conducted in the year 2016 and 2017, of them 105 (1.79%) were intrauterine fetal death. Mean age of mothers with fetal death were similar (25.47±5.64 vs 25.76±5.7 years; p>0.05). Common presenting complaints were reduced/absent fetal movement, abdominal pain, pre-labor rupture of membranes and antepartum hemorrhage. The majority of women were at low risk. Fetal characteristics of the intrauterine fetal deaths were almost comparable except fetal weight. Dead fetuses weighed lower significantly in the year 2016 and 2017 (2182.78±821.04 vs 1943.62±912.19 gram; p=0.05). In both periods, majority of stillborn babies appeared to be non-dysmorphic. Initial ultrasound or clinical diagnoses of adnexal masses were referred for the CT scan. Incidental adnexal findings on abdominal-pelvic CT scan performed for other diagnosis were also included.Descriptive parameters were calculated. \u0000Results: Total 46 cases were studied where mean age was 41.3 years with range 10-74 years. Most common age group with adnexal masses were in between 30 and 50 years (56.6%); 86% had benign features and rest were associated with either ascites (66%) or peritoneal deposits (13%). Complex cysts (60%) was the most common consistency with simple cyst (26%) followed by solid (6%). CTG was done before each transvaginal administration of misoprostol in 113 primigravida patients at Paropakar Maternity Women’s Hospital in Kathmandu. Variables studied were indications of induction, mode of delivery, Apgar score and neonatal admissions. \u0000Results: 11.5% of CTGs were non-reassuring who underwent Cesarean Section and had significantly low Apgar at 1 minute but only 46% of neonates required resuscitation. Liquor was thick meconium in 61.5% of non-reassuring CTGs. Total twelve (10.6%) of neonate needed resuscitation of which six (6%) neonate in reassuring CTG where as 46.1% of neonate needed resuscitation in non-reassuring. NICU admission rate was 7.96% in total and 33.3% of neonate with non-reassuring CTG who needed resuscitation. This study was carried out in the Department of Obstetrics and Gynecology, Gauhati Medical College and Hospital, Gauahati from August 2012 to July 2013. Blood samples were drawn from the antecubital vein and serum homocysteine levels were measured. \u0000Results: The mean serum homocysteine levels in preeclampsia cases were 27.7 ± 6.4 μmol/l compared to serum homocysteine levels in normal pregnancy 9.23 ± 3.4 μmol/l (P<0.01).There was a significant relationship between the systolic and the diastolic blood pressure and serum homocysteine levels. Cases with ovarian mass were taken pre-operatively with serum tumor marker (CA-125) and ultrasound report, and histopathology report post- operatively. Pregnancy and diagnosed malignancy were excluded. Sensitivity, specificity, positive and negative predictive values of RMI-3 were calculated at different cut-off values using Receiver operator characteristics (ROC) curve. \u0000Results: 36 cases of ovarian tumor from 15 to 60 years (mean=35) were studied. There were 31(86.1%) premenopausal and 5 (13.9%) in menopausal state; 26 (72.2%) were married and 10 (27.8%) unmarried; 19 (52.8%) were multiparous, 9 (25%) were nulliparous and 8 (22.2%) uniparous; 34 (94.4%) presented with pain in lower abdomen; 16 (44.4%) had lump in lower abdomen; 8 (22.2%) had bloody vaginal discharge. Eight out of 36 (22.2%) had malignant histopathology. Taking histopathology to diagnose ovarian malignant tumor RMI 3 score >200 has sensitivity, specificity, positive and negative predictive value of 75%, 92%, 75%, 92% respectively.
