The paper uses methods of copying information from primary and reporting medical documents and logical analysis of regulatory documents. In the conditions of the COVID-19 pandemic, activities of MSE facilities are guided by the Temporal procedure for examination. The Procedure guides establishment of disability, development of rehabilitation or habilitation programs for the disabled (disabled children), as well as determination of the degree of loss of professional ability to work as a result of industrial accidents and occupational diseases, and development of rehabilitation programs for victims of industrial accidents and patients with occupational diseases. The examination is carried out in absentia, without presence in person. Also, the Order of the Ministry of Labor of the Russian Federation No. 979n dated 12/30/2020 approved the \"Procedure for organization and performance of federal institutions of medical and social expertise\", stipulating the rules, sequence and timing of the provision of MSE services to the citizens. The Order of the Ministry of Labor of the Russian Federation No. 106n dated 05.03.2021 approved the \"List of indications and contraindications for providing the disabled people with technical means of rehabilitation\", which has introduced a number of changes to the technical means of rehabilitation of the disabled people. The main feature of the MSE procedure was the automatic extension of disability for 6 months if there is no referral for examination (form 088/y) from a health care facility. In the framework of the Temporal Procedure, disability was automatically extended for 29,624 citizens in 2020 (38.0% of the total number of examinations), and 31442 citizens for the six months of 2021 – (70.0% of the total number of examinations). {"title":"MALIGNANT NEOPLASMS OF FEMALE REPRODUCTIVE ORGANS IN DYNAMICS in 2011-2020, ON THE EXAMPLE OF NOVOKUZNETSK","authors":"N. Zhilina, S. Malignant neoplasms are among the leading socially significant diseases. In recent years, the mortality rate from malignant neoplasms among females aged 16-54 in Russia has taken first place, slightly ahead of the traditional leader - mortality from diseases of the circulatory system. Therefore, early detection of oncopathology, including reproductive organs, means preservation of life, health and performance capability, and ultimately, the quality of female life, including young and middle-aged women. Purpose: to identify risk zones of oncopathology of female reproductive organs to ensure timely and adequate control actions and women's health improvement. Material and methods. The study was based on statistical reports of the Regional clinical oncology dispensary (Novokuznetsk branch) for 2011-2020. The reporting information has been translated into a database format (DB) with a 1-year observation unit. The database was created in the licensed statistical package IBM SPSS Statistics-19. The following methods of statistical analysis were used: calculation of median values of indicators, upper and lower quartiles; the Kendall’s Tau correlation method (Ʈ), the Mann-Whitney criterion (U). Results. The risk zones of oncopathology of the female population of Novokuznetsk as a whole and by age groups were identified. In terms of morbidity and mortality per 100 000 population, malignant neoplasms of the breast have the highest morbidity indicator, while ovarian cancer has the highest mortality rate during the year. At the same time, the highest detectability of stages III and IV of oncological diseases is in ovarian cancer. The risk zones of oncopathology by age are as follows: cervical cancer in females aged 40-44, breast cancer in females aged 60-64 and 65-69, and endometrial cancer and ovarian cancer in females aged 65-69. In these ages, the highest levels of the studied morbidity in the dynamics in 2011-2020 were revealed. Cervical cancer and breast cancer have a negative trend in crude incidence: a statistically significant growth of both absolute and relative (per 100 thousand population) indicators was revealed. As to ovarian cancer localization, there is a tendency towards a decreasing absolute indicator. Conclusions. The main study results can be used to conduct preventive examinations and medical examinations of women with a focus on identifying risk zones of the reproductive organ cancer by age group and localization. {"title":"WORKING TIME OF THE OBSTETRICIAN-GYNECOLOGIST AT THE OUTPATIENT LEVEL","authors":"M. N. The study of the costs of an obstetrician-gynecologist’s working time on an outpatient basis is a very urgent task, since it is the first stage in determining the standards for the number of these doctors, which needs to be revised based on the modern conditions for the provision of medical care. Purpose: to determine the costs of working time of an obstetrician-gynecologist on an outpatient basis, both for different types of admission of one patient and individual labor operations. Materials and methods: Materials: data of photo-timing observations, primary medical documentation (forms №№025/у «Medical record of a patient receiving medical care on an outpatient basis» and №111/у-20 «Individual medical record of a pregnant and postpartum woman»). Methods: photo-timing measurements, statistical, analytical, expert assessments, mathematical statistics, descriptive. Results. The time norms for one patient's appointment at the first level of outpatient medical care in modern conditions are: for medical and diagnostic purposes, primary - 17.88 minutes, repeated - 10.68 minutes; for prophylactic purposes, primary - 10.68 minutes, repeated - 1.5 minutes; for pregnancy women, primary - 22.89 minutes, repeated - 12.98 minutes. The average duration of admission of one patient by an obstetrician-gynecologist is 15.02 minutes. The time spent on performing the main activity amounted to 56.9%, while the implementation of the necessary additional activities takes 43.1% of the total time of the patient's appointment, of which 39.5% is represented by work with documentation. Conclusions. The duration of one patient's admission to an obstetrician-gynecologist at the first level of outpatient care is 15.02 minutes. A significant part of the time of admission of the studied specialist is spent on performing additional types of activities, where work with documentation prevails in the structure of the latter. The norms of time, workload and standards for the number of doctors should be periodically revised in order to correspond to the modern conditions of medical care. Application area of the results. The results of the study can be used in practical health care by the management team for making managerial decisions in the field of staff rationing. Cardiovascular diseases, according to the World Health Organization, are one of the leading causes of mortality among the population of developed countries of the world, including the Russian Federation. The development of cardiovascular diseases is closely related to behavioral risk factors such as smoking, unhealthy diet, insufficient physical activity and a number of other factors. The planning of preventive programs at the population level and the organization of effective preventive activities of the primary health care unit requires detailed and reliable information on the prevalence of risk factors for the development of cardiovascular diseases. Currently, there is a need to raise awareness of patients about the risk factors for the development of diseases; to increase the patient's responsibility for maintaining his health; to form skills and abilities in patients to reduce the adverse effects of behavioral risk factors The purpose to analysis of risk factors for the development of cardiovascular diseases in people of working age in the practice of medical office operating of the arterial hypertension prevention. Materials and methods. Retrospective analysis of the complex of automated integrated assessment of the state of the cardiovascular system with statistical processing of the data obtained. For qualitative indicators, the statistical criterion for the significance of differences is χ2 (chi-squared). Descriptive statistics are given in the form of N(n%), that is, the number of objects with this feature and the percentage of the total number of objects in the sample. Results. Results. In the office of prevention of arterial hypertension, the average polyclinic of the GBUZ \"City Polyclinic No. 91\" of the city of St. Petersburg, for 5 years (2014-2018), 983 appeals of the adult population were noted. In 807 cases, there are persons aged 18 to 65 years with risk factors for the development of cardiovascular diseases. Of these, men applied 382 times, and women 425 times, which was (47.4%) and (52.5%), respectively. According to the frequency of appeals, it was noted that people over 41 years of age were more likely to apply than younger people. Analysis of the dynamics of risk factors for the development of cardiovascular diseases in people of working age revealed that hypercholysteremia, hereditary predisposition to hypertension, are most common. Conclusions. The appeal rate of persons over 41 years of age is higher than that of younger persons. The frequency of smoking, alcohol consumption, physical inactivity, overweight and obesity is increasing in 2/3 of patients. Among the studied patients of the prevention of hypertension, hypercholesterolemia was significantly more common in women, and at the age of 18 to 40 years. To ensure efficiency in achieving targets in healthcare reforming and modernization, the requirements for quality managerial personnel are being increased. A wide variety of organizational and legal forms of the central health executive authorities of the constituent entities of the Russian Federation, their often insufficient staff size and imbalance in the staffing of structural units necessitate development of unified methodological approaches to the formation of the structure of the central health authorities of the constituent entities of the Russian Federation and substantiate methodology for calculating the staff size. The purpose of the study: to develop a methodology for creating the optimal structure of the health executive authority of a constituent entity of the Russian Federation and methodological recommendations for calculating their staffing. Results. Analysis of the organizational and legal structure and functional responsibilities of the state authorities in healthcare in 13 constituent entities of the Russian Federation has identified significant differences and different competencies, although each constituent entity of the Russian Federation solves general health issues within the competences, therefore, the scope of activities of all levels of government should be coordinated. Staffing of the regional executive authority (REA) should primarily depend on the population size of the constituent entity of the Russian Federation, its density, number of medical organizations, their total bed capacity, planned capacity of units providing outpatient care, and the number of medical personnel. A coefficient has been derived for calculating the staffing level of the state health authority in the constituent entities of the Russian Federation. In accordance with the general calculation criteria, several models of the REA structure were proposed, depending on the population size of the constituent entity of the Russian Federation. For REA with the number of deputies four and over, two versions of the model with different hierarchical subordination were proposed. Conclusion. In 2020, many Russians faced an increased need for medicines to treat COVID-19 and its complications, as well as economic hardship due to the crisis caused by the pandemic. The purpose of the study. To identify key factors that determined the Russian households’ spending on medicines and dietary supplements during the pandemic. Material and Methods. The study used Russian Longitudinal Monitoring Survey - National Research University “Higher School of Economics” samples of households and adults for the year 2020. We employed regression models to estimate the probability of purchasing medicines and dietary supplements, and family expenses on medicines and dietary supplements, depending on medical factors, healthy lifestyle factors, socio-economic and demographic factors. Results. The probability of buying medicines and dietary supplements and the household expenditures were higher in families with the disabled, individuals with multimorbidity, with self-reported poor health, and with pensioners. High expenditures on medicines and dietary supplements were observed in families which members visited doctors on a regular basis. Both the probability and expenditures on medicines and dietary supplements rose with the increase in average per capita income. The presence of children under 14 years in the family was associated with a decrease in expenditures on medicines. Smoking, overweight and sports were significant contributors to expenditures on medicines and supplements, although their relative contributions were quite small. Private health insurance did not affect expenditures on medicines. Residents of the Northwestern Federal District spent significantly more on medicines compared to residents of other districts. Conclusions. Expenditures on medicines and dietary supplements in 2020 were largely explained by the following two key factors - people suffering from two or more chronic conditions in the family and the average per capita income. Other household factors, although significant, contributed much less to the decision to spend on medicines and dietary supplements. Public provision of medicines especially during economic crisis can be improved by supporting poor households and including patients with multimorbidity into drug reimbursement programs and thus expanding the number of its beneficiaries. Scope of application. In this regard, health officials pay a special attention to the resource and regulatory support for care delivery to people older than working age, taking into account the changed conditions and capabilities of medical organizations. The purpose of the study was to assess normative documents and main indicators characterizing the number and performance of geriatricians providing medical care to the elderly and senile people. Material and methods: retrospective analysis of data from federal statistical observation forms No. 30 for the period from 2015 to 2020, data on the population aged 60 years and older, Rosstat data, orders of the Ministry of Health of the Russian Federation No. 38 and No. 297. Content analysis of regulatory documents, statistical (calculation of extensive and intensive indicators), descriptive statistics using MS Office Excel 2016, Statistica 6.1. The study results showed that, despite a gradual increase in the number of geriatricians and relevant visits by patients of this category, the full-time position staffing remains low, affecting a growth in the part-time coefficient, suggesting a need for training geriatricians. For example, the number of geriatricians in Russia as a whole in 2020 equaled to 0.09 per 10,000 population older than working age. The ongoing decline in medical position staffing against a background of a positive growth rate in the number of medical specialists is probably due to the gap between the increasing number of full-time positions and the staffing rate by individuals. Diabetes is currently the fifth leading cause of death worldwide. According to the International Diabetes Federation, every eleventh adult has diabetes mellitus (463 million people), 10% of the global health spending is spent on diabetes mellitus. In 2012, 1.5 million deaths from diabetes were registered worldwide. Purpose. To evaluate the structure and frequency of fatal complications of diabetes mellitus according to medical death certificates in the Tula region for 5 years (2015-2019). Material and methods. For this study, databases of medical death certificates of residents of the Tula region who died of diabetes mellitus in 2015-2019 were used; statistical analysis was carried out, a system of groupings was applied and an expert method was used. Results. The structure of fatal complications by four-character subcategories of the international classification of diseases and by type of diabetes mellitus has been analyzed. Two methods of analysis were used. The first method does not provide for the analysis of specific complications, while the second one is more accurate, making it possible to get a detailed structure of the causes of death from diabetes mellitus by chapter of the international classification of diseases. Conclusion. In diabetes mellitus, multiple complications top the structure of complications (72.9%), followed by other specified complications (7.5%) with neuropathies being in third place (6.2%). In the structure of complications by type of diabetes mellitus, diseases of the circulatory system prevail (54.0%), followed by diseases of the nervous system (29.4%), and diseases of the urinary system being in third place (11.1%). Scope of application. Against the background of the ongoing global pandemic of the new coronavirus infection, it is necessary to study peculiar features of the epidemic process of morbidity in the Republic of Bashkortostan. Purpose. The purpose of the study was to analyze morbidity, mortality, case fatality and organization of inpatient care across different age groups in the context of the new coronavirus infection in the Republic of Bashkortostan in 2020-2021. Material and methods. Data from the Federal Patient Registry for COVID-19 was used. Results. Plotting intra-annual dynamics made it possible to determine the maximum and minimum periods of morbidity and mortality. Sex and age characteristics, the number of hospitalized, the average length of patient stay in the hospital, case lethality rates were determined. Conclusions. In general, in the territory of the Republic of Bashkortostan, in 2020-2021, the trend towards increased incidence of the new coronavirus infection continued with a distinct significant increase in autumn. A characteristic feature of the epidemic process in the Republic of Bashkortostan in 2021 is the increased number of cases of the disease among working age population. There was a relationship between the patient's age and death at the hospital. There was a significant decrease in the mortality rate in 2021 compared to 2020 across all age groups.
