Laaser","doi":"10.4119/SEEJPH-3813","DOIUrl":"https://doi.org/10.4119/SEEJPH-3813","url":null,"abstract":"Background: This study reviews the ability of the Russian Federation to reduce the high mortality until 2030 evenly across the country and in accordance with the Sustainable Development Goals (SDG). Methods: We adopted the method suggested by Haenzel for estimating Premature Years of Life Lost for the age group <70 years and applied a projected reduction of 33% by 2030 as proposed for SDG 3.4. To calculate the potential time gap we used the model of the United Nations Development Programme and standardized the rates by the OECD 1980 Standard Population employing the direct method. A positive time-gap indicates that the respective country is “On Track” to achieve the target on time or even earlier; a negative value indicates that it may still be “Likely” or even “Unlikely” to achieve the target within the target timeframe i.e. in 2030. A country is still considered likely to achieve the target as long as a negative value for time-gap does not make up for less than -25% of the remaining time. For the determination of the gap in 2024 and 2030 and the analysis of the trajectory of life expectancy data, we made use of the database of the World Bank and identified the peak data before and after the crisis during the 1990ies. Results: We analysed the eight Federal Districts of the Russian Federation with regard to their premature mortality as there are: North Caucasus, South, Privolzhskiy (Volga), Far East, Uralskiy, Siberian, Central, and North West Federal Districts. In addition, we determine whether each of the eight districts is on track to reach the SDG target by 2030. All districts reach the target in advance of 2030, the District of North Caucasus 10.6 years earlier and the North West District still 5 years in advance of 2030. For comparison, the progress of the EU-27 has been calculated which - at a considerably lower level - shows smaller rates of reduction and therefore a delay of -2.2 years in 2030. If Russia keeps the present level of effort the reduction by one third of the level of premature mortality as in 2013 will be in reach already in 2024 i.e. 5.9 years in advance of the SDG3 target for 2030. This target is achieved quite evenly also throughout the 8 districts of the Russian Federation between 10.6 and 5.0 years in advance and in selected districts/republics with the highest and lowest mortality rates. Conclusion: After the steep decrease of life expectancy during the 1990ies the Russian Federation returned to the original trajectory. In this paper we determine the overall progress of the Russian Federation and its North West Federal Okrugin in particular, with regard to the reduction of mortality. \u0000Methods: For the purpose of inter-country comparison and progress over time we make use of age-standardised Potential Years of Life Lost (PYLL) applied to quantifiable strategic targets, the Sustainable Development Goal 3 on Health and Social Well-being and the European Union Strategy of the Baltic Sea Region. A gap analysis is performed to determine whether the target achievement is in delay or on track. \u0000Results: With reference to the baseline of 2009 – corresponding to the most relevant recent period 2009-2020 respectively 2009-2030 – the Russian Federation as a whole is on track achieving the two strategic targets in advance by 2.7 years. For the North West Federal Okrug around St. Petersburg and Kaliningrad bordering the Baltic Sea the target achievement is estimated to be 4.8 and 10.8 years in advance of the deadlines 2020 and 2030. In comparison to the Baltic Sea states the Russian Federation takes a middle position after Estonia, Latvia and Finland. The early target achievement is confirmed if the period 2003-2020 respectively 2003-2030 is considered. \u0000Conclusion: Although the region is progressing there may be a slowdown towards 2030. Increasing involvement of patients in quality improvement of health care is a global trend. Appeals of the citizens to the state authorities are the easiest way to protect the rights of the citizens in the field of health care. The increased social activity of the citizens and transparency of the state authorities have determined the increase in the number of complaints and appeals of the citizens to the state authorities and medical organizations regarding provision of medical care. Handling appeals and complaints is a systemic problem of the Russian healthcare. Organization of monitoring over incoming information will make it possible to organize systematic work to protect the rights of the citizens in the field of health care, to improve health care quality and accessibility. The purpose of the study is to analyze organization of activities for handling appeals in public health protection at the regional level in order to develop proposals aimed at improving efficiency of appeals’ handling. Material and Methods. Material: official data of the Ministry of Health of the Chelyabinsk region. Research methods: mathematical and statistical; analytical (quantitative, structural, comparative). Results. A growth trend in the number of public appeals to the health executive authorities both at the federal and regional levels has been identified, including dynamics in the increase in the number of appeals related to the provision of medical care, financing and staff remuneration in medical organizations. Regional features of the structure of public appeals submitted to the Ministry of Health of the Chelyabinsk region have been determined. Results of the analysis of the appeal handling in medical chats in the Telegram messenger in 2022, as part of the “ZdravControl” project implementation have been presented. Conclusion. Results of the analysis of the public appeal handling in the field of public health protection at the regional level substantiate the need for developing a set of organizational measures to improve organization of care delivery in the region, and raising public awareness about their rights in the field of public health protection. The lack of positive dynamics in the rates of morbidity and mortality from cancer in the eastern regions of Russia in recent decades has been the basis for studying epidemiology of malignant neoplasms of the urogenital and urinary system among residents of a particular territory. The purpose of the study is to estimate the major morbidity and mortality parameters in patients with uro-oncologic pathology in the Far East (2008- 2021). Material and methods. Rates of morbidity, mortality and their derivatives, reporting forms of cancer statistics and regulatory documents of healthcare bodies have been analyzed. Results. In 2021, the average incidence rate of prostate cancer equaled to 30.92±1.75о/оооо, kidney cancer – 10.82±0.28о/оооо, and bladder cancer - 6.83±0.18 (2018: 29.81±2.05о/оооо; 10.83±0.34о/оооо; 6.97±0.19, respectively). The number of patients with stage I-II prostate cancer (+33.9%), kidney cancer (+11.3%), and bladder cancer (+14.3%) increased over the study period with a simultaneous decrease in the number of neglected forms of the disease. The share of actively detected patients increased across all tumor localizations, as well as the contingent accumulation index by the end of the year. Morphological confirmation of the diagnosis corresponded to the Russian average (prostate cancer - 96.6%, kidney cancer - 89.9%, bladder cancer - 94.4%). The share of cancer survivors for 5 and more years was lower than in Russia for prostate cancer (44.1%), kidney cancer (58.7%) and bladder cancer (56.6%), however, the mortality rate within one year after the diagnosis has improved. The average mortality rates of prostate cancer equaled to 11.80±0.20о/оооо, kidney cancer – 3.80±0.08о/оооо, and bladder cancer – 2.79±0.08о/оооо (2018: 11.60±0.19о/оооо, 3.85±0.09о/оооо, 2.89±0.07о/оооо, respectively). The quality of oncologic service activity was denoted by the index of registration reliability, which did not exceed 0.40 in the Far Eastern Federal District. Discussion. The present study has confirmed that the oncologic service of the region needs standardization of all types of medical care, improvement of material and technical facilities, provision of medical therapy departments with modern drugs against the background of the improved standard personnel policy. Conclusion. Preventive activities aimed at early cancer detection and well-organized follow-up of patients with background and precancerous conditions remain priority areas of work of medical preventive institutions in the Far East. The study on life expectancy of the population and its impact on the indicator of the leading causes of death is an important stage in the assessment and forecasting of socio-economic processes in individual administrative-territorial entities. Analysis of the dynamics in the indicator of average life expectancy provides the necessary information for developing an evidence-based demographic policy in the subject of the Russian Federation. Purpose of the study: to identify and explore the potential for increasing average life expectancy in the Irkutsk region. Material and methods. A continuous method was used to observe deaths by sex, age group, cause of death for 2005, 2010, 2019 in the Irkutsk region. We used methodology that included the indirect method of constructing tables of average life expectancy (ALE) or survival tables. We determined intensive indicators of mortality by class of causes and by individual nosological form. Based on the decomposition method we carried out calculations of ALE and the elimination reserve of the number of years of increase in life, subject to a 50% decrease in mortality rate with regard to classes 2,9,20 of the International Classification of Diseases No. 10 revision. Results. Dynamics in life expectancy among male and female population demonstrate the following: increase in life expectancy at birth among males equals to 6.6 years, and 4.2 years in females. Accordingly, the gap in life expectancy between women and men has narrowed from 14.2 years to 11.8 years. Assessment of the contribution of individual causes of death to the overall mortality and life expectancy among males and females shows that the following three classes play the leading role: diseases of the circulatory system, neoplasms, and external causes of death. Calculation of the demographic gain (elimination reserve) shows that the class of diseases of diseases of the circulatory system is of highest significance for increasing life expectancy, while control over cerebrovascular diseases and acute myocardial infarction makes the highest contribution to increasing life expectance in the Irkutsk region Conclusion. The presented results made it possible to study age and gender characteristics of the most important demographic indicator of average life expectancy in male and female population. The traditional healthcare system is losing its relevance. Digital transformation of healthcare dictates new conditions to medical organizations, imposing new requirements on healthcare administrators. The purpose of the study is to analyze the attitude of heads of medical organizations in the Arkhangelsk region towards the digital model of healthcare organization. Material and methods. A sociological survey was conducted among heads of the state medical organizations in the Arkhangelsk region. The survey was conducted in the period from March 2021 to August 2022. Representatives of 46 medical organizations took part in the survey including 28 urban and 18 rural organizations. The total number of the respondents added up to 181. A semi-structured questionnaire was used as a research tool, the design was a cross-sectional study. Results. The conducted study shows that 82.3% of the healthcare administrators consider informatization of medical organizations to be successful and show a high level of confidence (95.6%) in information retrieved from medical information systems. In urban areas, the respondents noted a higher level of workplace automation (78.0%), greater need for a personal computer in everyday work (94,9%) and higher frequency of its use (96.6%), and the highest proportion of connection to the medical information system of all departments of a medical organization (94.1%). Healthcare administrators are unanimously positive about replacing the paper medical history/ outpatient records with an electronic equivalent. The main problems related to automation of a medical organization are as follows: insufficient financing of informatization in a medical organization (50.8%), requirements to duplicate electronic information in paper form (48.1%), lack of clear requirements for the implemented information systems (22.7%). Conclusion. The study of attitude of the heads of medical organizations in the Arkhangelsk region towards the digital model of healthcare organizational showed a greater readiness of urban organizations for a standard information interaction in healthcare in the region. To ensure successful work with the medical information systems, healthcare administrators need a supervised on-the-job training by a more experienced colleague. The rate of eye care seeking (that significantly differs across population groups) affects prevalence of corneal disorders and visual impairments. Data on the causes of low vision in corneal disorders are fragmental. Studies on prevalence of corneal blindness are scarce. The climatic and geographic diversity of Azerbaijan and different accessibility of eye care typical of various regions of the country make the study significant. Purpose: to compare prevalence of corneal disorders in cities and districts of Azerbaijan. Material and Methods: information was provided by statistical services of the administrative and territorial entities. Statistical forms for registration of final diagnoses served as the control document. Statistical forms encoded by ICD-10 were distributed by age, gender, and nosological variants of corneal disorders. Prevalence of corneal disorders per 100,000 population was calculated. Results: the overall prevalence of corneal disorders in Azerbaijan equaled to 340.4±2.0 per 100,000 population. The prevalence of corneal disorders in the administrative and territorial entities varied from 228.4±20.2 to 697.6±34.3 per 100,000 population. The number of females with corneal disorders was higher in all age groups (р˂0.05) except for ages over 70. The maximum relative gender risk was registered in the age group of 40-44 years. The age-related prevalence pattern of corneal disorders in males and females was similar (the maximum prevalence is registered in the age group of 65-69 years). The leading nosologies included corneal injuries and foreign bodies (20.3%), bacterial keratitis (20.2%), and corneal erosions (18.0%). Conclusions: in Azerbaijan, the prevalence of corneal disorders depends upon the place of residence. These diseases are more common in females and the elderly. The complexity of developing such text report templates lies in the diversity of instrumental methods, variety of diagnostic objectives and specific work characteristics of individual medical organizations. Development of tools for marking the unstructured radiological chest examination protocols makes it possible to improve the system of electronic document management in healthcare due to automation of data formalization processes as well as develop data sets for machine learning. The purpose of this study is to develop a system for automated marking of text reports of the unstructured radiological chest examination protocols using heuristic approach and machine learning algorithms. Material and methods. The study used patient data on radiological chest examinations of medical organizations connected to the Unified Radiological Information Service of the Unified Medical Information and Analysis System of inpatient and outpatient medical organizations of Moscow and the Moscow region. Semantic analysis methods, expert rules and machine learning algorithms were used for processing the unstructured text reports. Results. The study has identified language patterns associated with important pathological conditions and “norm” class as well as developed regular expressions for these classes. A dictionary of radiological concepts and abbreviations (397 items) was compiled, followed by the development of an algorithm for correcting grammar mistakes in the protocols. In collaboration with the expert group, the rules of multilabel classification of the radiological examination protocols were created and their efficiency was tested. When solving the multilabel classification problem using only the expert rules, the percentage of exact matches equaled to 84%. Inasmuch as classifiers for conditions such as “infiltration/consolidation” and “blackout focus” were not effective, we have adjusted the models of machine learning. Conclusion. The best classification results were demonstrated by the recurrent neural network with the long-short term memory architecture ensuring sensitivity of 89% and 99% for “infiltration/consolidation” and “blackout focus” classes, respectively. The regulatory framework should correspond to the real situation in the field of public health protection. At the present stage of health care development, compliance of regulatory legal acts with the real situation related to activities of medical organizations in the Russian Federation is becoming crucial. High prevalence of chronic non-communicable diseases and socially significant diseases, as well as a complicated epidemiological situation caused by the spread of the coronavirus infection, necessitates a clear regulation of actions of the primary health care workers, especially in terms of providing planned medical care in outpatient settings, including medical examination of certain groups of the adult population in terms of its oncological component. Purpose: to conduct a retrospective analysis of the orders of the Ministry of Health of the Russian Federation regulating procedures for medical examination of certain groups of the adult population and collection of statistical data through the form of industry statistical reporting, in regard to the oncological component of the medical examination. Material and methods. Content analysis of the orders of the Ministry of Health of the Russian Federation, regulating procedures for medical examination of certain groups of the adult population, and orders approving the forms of industry statistical reporting in the period from 2013 to 2021. Results. Introduction of each new order regulating the procedure for medical examination of certain groups of the adult population has been changing the oncological component of the medical examination and, ultimately, it was formulated in separate appendices of the orders of the Ministry of Health of the Russian Federation №124n dated 13.03.2019 and №404n dated 27.04.2021. Conclusions.
