At the present stage, many countries are on the threshold of a global \"epidemic\" of diabetes mellitus. Chronic hyperglycemia leads to damage and failure of various organs and tissues. Therefore, diabetes mellitus, and especially its most common form - type 2 diabetes (DM2), is a serious medical and social problem. An effective approach to therapy of DM2 is highly relevant and depends on many factors and pathogenetic mechanisms. But despite the great progress in this direction, there are still many unresolved issues concerning individualization of treatment, creation of new or selection of already available drugs. This study is devoted to the dynamic analysis of prescription forms prescribed and dispensed to patients suffering from DM2 in the Kabardino-Balkar Republic for 2020-2023. The study found an increase in the total volume of drugs dispensed, the financial volume of expenditures on pharmaceutical care for diabetics with DM2 and costs per patient per year. A significant increase in the number of DM2 patients during the study period is shown. The analysis of prescriptions of oral medicines revealed the continuing trends of predominance of sugar-lowering therapy in medical organizations of the Republic of traditional approaches. Innovative drugs are available only to a small number of patients. Herbal expectorants with a reflex action can significantly increase the volume of bronchial secretions that young children are unable to cough up on their own, which can lead to a significant impairment of the drainage function of the lungs. A combination of herbal expectorants with secretolytics will be very effective. The creation of combination drugs that simultaneously affect various pathogenetic mechanisms of cough is a promising direction in pharmacy. Therefore, the purpose of this study is to develop the composition of lozenges with expectorant and mucolytic effects that do not cause disruption of the drainage function of the lungs. During the study, were produced 9 ready compositions of a medicine, the quality of which was assessed in accordance with the requirements of the general pharmacopoeial article of the State Pharmacopoeia of the XIV edition according to the indicators: description, disintegration, crushing strength and grateness. Cisplatin continues to be used to treat a wide range of malignancies, the main ones being lung (49%), testicular (2%), ovarian (7%), uterine cancer (13%), head and neck tumors (16%), and bladder cancer (13%). The results of an analysis of the global market for these drugs and a forecast until 2026, as well as the Russian market for 2022, are presented. The prospects for the development and registration of new drugs and new promising combinations involving drugs from this group are described. Analysis of recommendations from the professional oncology community indicates continued widespread use of existing platinum drugs. There has been a trend towards an increase in global sales, largely due to their increase in the Asia-Pacific region, countries of Central Asia and Africa. On the Russian market, whose capacity is 1.3% and 1.9% of sales of all anticancer drugs in value and volume terms, domestic manufacturers are satisfactorily represented. New promising treatment regimens are being developed, including those that include proprietary drugs (primarily PARB inhibitors); There remains interest in the development of innovative platinum drugs. We took into account the existing need to improve the reproductive health system for girls and adolescents, analyzed data on the needs and capabilities of participants in sex education for children and adolescents, and developed an electronic “School of Reproductive Health.” This article is devoted to the features of the school structure, its technical and information content. To ensure the effective operation of the school, the main groups of participants in the process of sex education have been identified and the tasks for each of them have been carefully described. An analysis of the results of using the school was carried out using a sociological survey of 115 parents of children and adolescents who attended the electronic school for reproductive health. Based on the results of the survey, it was determined that the structure of the school turned out to be absolutely user-friendly, and the recommendations received at the school were useful for the vast majority of respondents. More than 95% of users noted that the e-school increases or rather increases the level of their knowledge in matters of maintaining reproductive health and conducting sex education. As a result, according to the surveyed users, the electronic school of reproductive health can completely (51.3%) or partially (36.5%) replace the need to visit a pediatrician or obstetrician-gynecologist for preventive purposes to explain issues of reproductive health and sex education. The average score of readiness to recommend visiting the described information resource is 9.5 out of 10 points. Materials and methods of research. As a source of primary information, we used data on cases of medical care provided to the population of older age groups (60 years and older). The probability of treatment in each one-year age range, starting from the age of 60 years, was studied. 41 points were used to construct the dynamic curve. The calculation was made per 1000 population of the corresponding age and gender. The principle of constructing mathematical models was based on a functional approach. As a result of this approach, a system of linear and nonlinear equations was obtained, in each of which the dependence of the volume of medical care on predictor variables was expressed. In this case, age and gender were used as predictor variables. Results. Algebraic formulas were obtained for modeling the level of need for medical care provided in outpatient settings in relation to three blocks of causes (group A of causes). The impact on the level of need of other reasons can be determined using a correction factor. During the study, we used the methodology of R.A. Berezovskaya. The results of the study allow us to conclude that 36% of students in specialized medical education have a low cognitive level of health, i.e. children either do not know or are not fully aware of the need to take care of their health. The behavioral level of more than half of them is very high, which shows their desire to lead a healthy lifestyle. A small number of children (11%) have a low emotional level of health. The study of the value-motivational component of health allows us to conclude that health is not a value for many subjects. Currently, a number of aspects related to informing patients about unregistered medicines remain insufficiently regulated by current legislation. The purpose of the study is to analyze the legal regulation of informing patients about various aspects of the use of medicines unregistered in the Russian Federation. In the process of research, the texts of regulatory legal acts of the Russian Federation have been used, and systematic, grammatical and logical methods of their interpretation have been also used. It has been shown that currently Russian legislation establishes a mandatory requirement for medical organizations and attending physicians to inform patients about the possibility of prescribing unregistered medicines for them if there are medical indications. It has been established that the final decision on prescribing unregistered medicines to patients for vital indications is made by a council of the relevant specialized medical organization at the federal level with the preparation of a protocol, but at the same time, the responsibility for informing the patient about all aspects of pharmacotherapy rests with his attending physician. The main difficulties in obtaining complete and accessible information about unregistered medicines by attending physicians have been identified. This article presents the results of a study examining the dynamics of changes in the structural and functional parameters of the left heart in COPD patients who have had Covid-19 12 months after discharge from the hospital. Materials and methods. The prospective controlled trial included 73 patients with COPD who were divided into 2 groups: Group 1 (n=43) - patients with a history COVID-19; Group 2 (n=30) - patients without a history of COVID-19. The analysis included the results of a double echocardiographic examination of the left heart of the patients over a period of 12 months. Results and Discussion. It was found that in the acute phase of COVID-19, there were higher values LAV index, left ventricular (LV) end-diastolic volume (EDV) and LV end-diastolic size (EDS) (p>0.05) in-group 1. In12 months, both groups had mean values at the upper limit of normal. However, a significant increase in LV end-systolic size (p<0.05) and LV end-diastolic volume (EDV) (p<0.01) was observed in patients of group 1. LV EDS and LV EDV were also significantly higher in-group 1 (p<0.05). Initially, the normal type of LV diastolic function predominated in both groups. However, there were significantly more patients with LV diastolic dysfunction in-group 1 (χ2=4.59; p<0.05). At 12 months after COVID-19 in patients with COPD, there was an increase in E/a and the E/e' ratio (p<0.01). The relative risk of detecting LV diastolic dysfunction was 2.96 in-group 2. This fact is explained primarily by the fact that the development of hemorrhoids is facilitated by a sedentary lifestyle of the urban population, bad habits, irregular and unbalanced nutrition, and abuse of fast-food products. The most effective way to treat hemorrhoids is surgery. However, drug therapy is no less important, both at the stage of conservative treatment and in the postoperative period. Most often, hormonal and antibacterial drugs are used to treat hemorrhoids. However, as medical practice shows, their use does not always provide a long-lasting and sustainable antihemorrhoidal effect, and is also characterized by a number of side effects associated with the possibility of systemic absorption of the active components of the drugs. In this regard, the search and development of new safe and effective drugs for the treatment and prevention of exacerbations of hemorrhoids is an urgent task of modern pharmaceutical science. When designing drugs for the treatment of hemorrhoids, it is necessary to take into account the combined clinical picture of this disease, including such manifestations as swelling, inflammation, pain, itching and burning in the rectal area. Thus, it seems appropriate to create a multifunctional antihemorrhoidal drug, which includes components of anti-inflammatory, antimicrobial and anesthetic action. It is possible to ensure such a rational combination by combining synthetic drugs and complexes of biologically active substances of plant origin One of the compositions suitable for the development of an antihemorrhoidal drug is, in our opinion, the combined presence of benzocaine and dry herb extract. Benzocaine is well known as an affordable, effective and safe local anesthetic used for local pain relief. Hemp sapling herb, in turn, is often used in folk medicine as a wound-healing, anti-inflammatory, antimicrobial, decongestant for the treatment of bruises, burns, infected wounds and ulcers of various etiologies. The most rational form for the combination of benzocaine and dry sapling herb extract proposed above seems to be suppositories. In this regard, the purpose of this study was to develop the composition, technology and testing of anti-hemorrhoidal suppositories from the sapling grass with dry extract and benzocaine.
