Novel Drug Treatments for Diabetes Complications 糖尿病并发症的新型药物治疗

It has become the most prevalent disease in the Saudi population according to the last report from the national prevention program. Diabetes cases can be divided into three types: type I, which requires insulin therapy; type II, which requires the use of hypoglycemic drugs, with or without insulin, and is the most common type in Saudi Arabia; and gestational diabetes, which occurs during pregnancy. We must remember that diabetes can be very dangerous, causing complications in many parts of the body, like the nerves, heart, eyes, and kidneys. According to previous studies, these complications vary depending on the severity of diabetes. They may also affect the blood vessels of the retina, which can develop into diabetic retinopathy. Complications may also be fatal, in the cases of heart attacks, angina, and atherosclerosis. Additionally, they may cause poor circulation that leads to infection and gangrene, or cause neuropathy with numbness and a loss of pain sensations. Diabetic nephropathy is another harmful effect of diabetes that attacks the renal blood vessels. In this article, my aim was to review newly discovered drugs that target and decrease the complications produced by diabetes. Blood transfusion is vital in saving lives but is associated with transfusion-transmittable infections e.g.HBV which can lead to adverse consequences. Objective: To determine HBV infection prevalence between donors of blood banking in Makkah,Saudi Arabia. Methods: An institutional-based cross-sectional study was conducted among 2024 blood donors in two hospitals blood banks. Data were collected using laboratory blood screening tools. Data were analyzed using the SPSS statistical program. Statistically significance was considered at p < 0.05. Results: Our study was conducted on 4024 blood donor’s data. For Saudi donors, statistical analysis of HBV results has showed that 1% of donors were +ve for HBsAg and 6.8% of donors were +ve for anti-HBc. The results of NAT have showed that 1.1% of donors were positive and all were males. There is a high significance in age between 30-39 and 40-49 for NAT. The overall prevalence of HBsAg was 1% in Saudi donors. For non-Saudi donors, HBV results showed that 0.5% and 12.8% of donors were positive for HBsAg test and Anti-HBc test, respectively. The results of the Anti-HBc test were highly significant for the age range 30-39. The statistical analysis of NAT results showed that 0.8% of donors were positive and all were males. The overall prevalence of the HBsAg test among non-Saudi donors was 0.5%. Conclusion: The HBsAg and anti-HBc rates among blood donors have declined significantly. NAT is an important tool for viral detection which is expensive and not available everywhere. Some of these injuries occur at home, where parents are considered responsible for reporting them and taking the first action, which directly can influence the prognosis. The present study aims to quantify the level of parents’ knowledge and attitude towards emergency management of TDI and to predict the factors that affect their level of knowledge. Subject and Methods: 269 questionnaires were completed by parents who live in Makkah, Saudi Arabia. The questionnaire was divided into three parts, including questions on demographic data, knowledge, and attitude about emergency management, and self-assessed knowledge and attitude toward further education on TDI emergency management. Results: The average knowledge score was 2 out of 9 (SD±1.9). More than 78% of the participants were enthusiastic about further free education and believed that they don’t have enough knowledge about TDI. A significant positive association has been predicted, using a multivariable Poisson model, between the parental knowledge score and their willingness to attend free lessons, having a previous idea about TDI, higher educational background, confidence, and higher socioeconomic class (SEC). However, the history of a previous TDI was predicted to have a significant negative influence on the knowledge score (p values <0.05). Conclusion: Parents of children in Makkah city show a weak level of knowledge about TDI emergency management. A concerning attitude was shown as parents tend to proceed to manage TDIs against the recommended managements rather than seeking professional advice. This study aims to evaluate the values of biomarkers such as Neutrophil-Lymphocyte Ratio (NLR), Platelet-Lymphocyte Ratio (PLR), and Lymphocyte-Monocyte Ratio (LMR) in monitoring diabetes in a rat model subject to treatment with melatonin, vitamin-D and vitamin-E. Methods: Male albino rats (n=8-10 per group), normal and with diabetes were divided as follows into 12 groups: G1 normal fed, received no medications; G2 normal, treated with melatonin; G3 normal, treated with vitamin-E; G4 normal, treated with vitamin-D; G5 diabetic; received no medications; G6 diabetic, treated with insulin; G7 diabetic treated with melatonin; G8 diabetic, treated with melatonin and insulin; G9 diabetic, treated with vitamin-E; G10 diabetic, treated with vitamin-E and insulin; G11 diabetic, treated with vitamin-D and G12 diabetic, treated with vitamin-D and insulin. Two months post-treatment, hematological (NLR, PLR and LMR) and biochemical examination of glucose profile and oxidative stress status, were performed. Results: NLR is significantly decreased on comparing G3 and G4 with G1, and significantly increased on comparing G9 with G1. On comparing G3 with G5 and G6, NLR is significantly decreased, but on comparing G9 with G5 and G6, NLR is significantly increased. However, PLR and LMR showed no statistically significant changes in all rat groups. Various techniques and instruments have been used for endodontic retreatment. Recently, the XP-endo Finisher has been introduced in endodontic treatment to clean highly complex morphologies and difficult to reach areas. Aims: a) To compare the effectiveness of XP-endo Finisher and ProTaper universal retreatment system with or without organic solvent in removing gutta-percha. b) To compare the amount of apically extruded debris between the groups. Methods: 24-human- extracted, single-rooted teeth were divided into three groups of 8. The groups were classified as follows: Group I: using a ProTaper universal retreatment system with chloroform. Group II: using a ProTaper universal retreatment system without chloroform. Group III: using a ProTaper universal retreatment system followed by XP-endo Finisher without chloroform. The apically extruded debris was collected during the retreatment and weighted. The roots were split longitudinally. The degree of dentinal tubules patency was evaluated by scanning electron microscopy. Results: The most intensely clear dentinal tubules were observed in group III, followed by group II and group I, respectively. Group I resulted in a debris extrusion, significantly less, as compared with the other groups (P < 0.05). While there was no notable difference between group II and group III (P > 0.05). Conclusion: Under the conditions of the present study, out of all the retreatment techniques that were used, the XPendo Finisher was the most effective in the removal of the gutta-percha in cervical, middle and apical thirds. This research aims to measure knowledge of and attitudevs towards TKA among health education students, as they are the health educators of the future. Methods: A cross sectional design was used to study 227 health education students from four Saudi universities that offered a health education program. A questionnaire was distributed to eligible participants during the 7-month study period (20 September 2017 until 17 April 2018). Data were analyzed using SPSS. A chi-square test to measure the association between the different groups in terms of categorical variables and a Spearman’s correlation test were conducted. Poor knowledge rated as 0-49%; fair knowledge 50-74%; and good knowledge 75-100%. A total score of more than 7.2 indicated positive attitude, more than 4.6-7.2 neutral attitude and less than 4.6 negative attitude. Results: Of all participants, 71.40% had poor knowledge of TKA, while 76.20% had a neutral attitude towards it. Participant’s level of knowledge was significantly associated with having previously heard about TKA and having a family member who had undergone surgery. Participants’ attitude was significantly associated with having previously heard about TKA. Conclusions: Overall, knowledge of TKA was poor, and attitude towards it was neutral. A correlation was found between participants’ level of knowledge and attitude. This step depends primarily on the CYP450 system, which plays a role in drug metabolism and changes their therapeutic responses. CYP450 enzyme system includes many notable points such as major families, their functions and also the factors that can alter their activity and cause significant interaction. According to previous studies, the most important factor affecting this enzyme system is the medications that can enhance or inhibit its activity. The aim of this article is to discuss the detailed mechanism of the interaction between CYP450 enzyme and other drugs. This mechanism is very important when prescribing various medications to avoid the problem of drug-drug interaction. We must remember that many drugs undergo metabolism through more than just isozyme interactions or may pass through dual metabolism pathways. Of these infections, Hepatitis B causes significant morbidity and mortality, and imposes a great burden on the Saudi Arabia healthcare system. Therefore, awareness of Hepatitis B virus (HBV) infection, routes of transmission, preventive practices and vaccination are very important for the protection of dental students. Aim: Determination of the level of knowledge, attitude and practice of preventive measures regarding the HBV as well as vaccine status among Umm Al-Qura University (UQU) dental students and interns. Method: A descriptive cross sectional study was conducted on 182 undergraduate students in fourth, fifth and sixth grades as well as on interns at Umm Al-Qura College of Dentistry. A self-structured questionnaire of 31 questions was distributed among participants to determine knowledge, attitude, preventive measures about hepatitis B during dental practice as well as the vaccine status. Data were collected, tabulated and statistically analyzed by SPSS v.20. Results: 182 questionnaires were answered, total males were 88 (48.4%) and females were 94 (51.6%). Among all students in 4th, 5th, 6th grades and interns, the overall knowledge was high (>70%). Also the scores for total attitude and knowledge about HBV infection were significantly higher in females than males (p<0.001). All participants were vaccinated against HBV and received 1-3 doses. However, 10.9% of them did not complete their doses (they received only 2 doses). Conclusions: UQU dental students and interns showed high knowledge and attitude about many aspects of HBV infection, but also there are some aspects in which they showed poor knowledge. It contained demographic data about the child and his mother, and included ten items about oral health practice, oral health attitude, and oral health knowledge that were answered by the child and his guardian separately. SPSS version 20 was used for the analysis of the data and descriptive statistics and chi- square tests were applied. Results: 405 children, both males and females, and their mothers have participated, 79% of them were Saudi mothers and 57% have had higher education. A significant difference was found regarding the correct tooth brushing frequency among male and female children. 67% had more than two episodes of dental pain during last year. No significant difference was found regarding the knowledge except the effect of the fluoride on teeth. More than 45% of the children who got good score their mothers are University graduates. Conclusion: Most of the children and their mothers have adequate oral hygiene knowledge, practice, and attitude. The disease transmits from animals to humans via the eating raw or insufficient cook meat or by contact with raw meat. The main aim of the present study was to define the prevalence and the associated risk factors of the disease in workers and animals in slaughterhouses in Makkah. The study was based on a cross-sectional study in humans and animals including, sheep, goats, cattle, and camels. Data were collected using a short questionnaire. Approximately 108 blood samples were collected from workers included the butchers and the workers and 160 blood samples of the slaughterhouse animals. Serum samples were kept in -80o C till examined by using ELISA IgM and IgG tests. The data analyzed by using SPSS version 25. The result showed that the prevalence in workers was 0.9% and 25% using IgM and IgG, respectively. In animals, all the results were negative using ELISA IgM and IgG tests. The result showed that there was a correlation between chronic infection and contact with a cat (p<0.015). The study focussed on the T. gondii in slaughterhouse workers which found that both acute and chronic toxoplasmosis.
