Complete blood count and bone marrow studies including flowcytometry revealed acute myeloid leukaemia (AML) with dysplastic changes and according to the WHO criteria published in 2018, it was categorised as AML-M2 not otherwise specified. The patient received azacytidine as a sole line of therapy because of his age and general condition. Ten months later, serum ferritin and prolactin level were elevated and the cytopenia became more severe, prompting new bone marrow studies for proper evaluation. Bone marrow metastasis from the existing non-hematopoietic malignant tumour were detected. A diagnosis of metastasis from adenocarcinoma of the lung was made by immunohistochemistry which showed positivity for cytokeratin 7(CK7), pancytokeratin (CKAE1AE3) and thyroid transcription factor (TTF). Additionally, CEA and CA 19-9 were elevated. To the best of our knowledge, this is the first case of a dual diagnosis of an haematological and a solid tumour in a patient, originating from the Makkah area, Saudi Arabia to be published. Keeping in mind that the patient was treated with azacytidine therapy, which was not licensed for use until now, for the development of secondary malignancies. This topic was rarely investigated in Saudi Arabia. Aims: This study investigated OG gender preference among female resident in Jeddah, Saudi Arabia and factors that influence their choice. Methods: In this cross-sectional study, 596 female participants answered a self-reported questionnaire. Participants were recruited from shopping malls in four different areas in Jeddah using convenient sampling. The participants were asked or questioned about their OG gender preference and factors affecting it. Results: OG gender preference was found to be an important issue among females in Jeddah, as 66.6% of the participants do ask about the OG gender before their visit, and 39.4% would feel uncomfortable if the preferred OG gender is not available. The results showed that 57.89% preferred female OG, 20.47% preferred male and 21.64% had no preference. Participants answered that for presence at delivery, 50.34% preferred female OG, 27.35% preferred male OG and 22.32% had no preference. Also, in the case of OG emergency and critical surgery, 9.2% reported they would refuse surgery if the preferred OG was not available and seek an alternative solution such as changing hospital. The most important factors that influenced the participant’s choice were: the ability to handle OG emergencies situations with confidence (53.40%), OG knowledge about women health (45%), and OG sympathy (42%). Comparison Methods: This was a prospective case-control study; a total of 551 women were recruited from two Maternity and Children Hospitals (Makkah and Jeddah cities) in the Western province of the kingdom between March 2013 and June 2016 Serum and products of conceptions were collected prospectively from 201 women diagnosed with first-trimester spontaneous abortion. Another set of serum samples and placental tissues were collected from 350 healthy pregnant women at the time of normal vaginal delivery. CMV IgM and IgG antibodies were measured serologically by ELISA while viral DNA was detected in tissue specimens by PCR. Socio-economic data were analysed to identify potential risk factors of abortion and/or CMV infection. Results: The overall prevalence was 88.4%, 13.6% and 10.1%, for IgG+, IgM+ and PCR+, respectively. IgG+ frequency was comparable between cases (89%) and controls (88%). Both exclusively IgM+ (7.9% vs. 0.6%) and dual IgM+/IgG+ (22.4% vs. 3.4%) antibodies were significantly more prevalent in abortion (P < 0.01). PCR+ was also significantly higher (P < 0.01) in abortion (25.4% vs. 1.4%). Women with exclusive IgM+ (OR 17.1; 95%CI [3.38 – 86.68]; P = 0.001) and those who were positive for all tests (OR 29.3; 95%CI [6.52–131.71]; P < 0.001) had a significantly increased risk for abortion. Conclusions: The majority of the study sample of pregnant women in Saudi Arabia were previously exposed to CMV. First trimester CMV infection appears to be an independent risk for spontaneous abortion and viral spread to the intrauterine cavity was associated with the highest risk for miscarriage.
