Materials and methods. A comprehensive survey of 198 children and adolescents 2—17 years of age (97 boys and 101 girls) with metabolic and inflammatory diseases of the biliary tract (MIDBT) was done. Physical development (PD) was assessed in all patientsby comparing the basic anthropometric parameters (height and weight) with age norms. Results and discussion. Individual analysis of anthropometric parameters of patients with MIDBT allowed to diagnose disharmonic PD (DPD) in 42.4 % of the surveyed. Most often, is were body mass deficiency (18.2 %), overweight (6.5 %) and obesity (8.1 %), and low height (7.1 %). Also,it was found that DPD is almost twice as often diagnosed in boys than in girls (62.6 vs. 32.7 %; p < 0.05). Regardless of the age of onset significant number of children were underweight. Overweight was diagnosed with approximately equal frequency. It is found that among patients with DPD, 81.8 % of girls and 62.7 % of boys had relapsing gallstone disease. Dynamic observation of patients allowed to determine the nature of gender­specific risk factors in children with different disease duration. Conclusions . Metabolic and inflammatory diseases of the hepatobiliary system is the important problem of рediatrics, because it affects the formation of the child’s health and development. Individual analysis of anthropometric parameters in patients with metabolic and inflammatory diseases of the biliary tract helped DPD in 42.4 % of patients. It was found that DPD is almost twice as often diagnosed in boys than in girls (62.6 vs. 32.7 %; p < 0.05), which was due to greater frequency of patients with short stature and excessive body weight. It was found that among patients with DPD, regardless of gender, there were more children with cholelithiasis (81.8 % of girls and 62.7 % of boys), who had a relapsing course of disease. Матеріали та методи. У хлопців 14—18 років із затримкою статевого розвитку досліджували вміст загального тестостерону у сироватці крові та добову екскрецію мелатоніну. Залежно від рівня загального тестостерону виділено групи: група 1 — 12,0—8,0 нмоль/л; група 2 — 7,0—4,0 нмоль/л; група 3 — < 4,0 нмоль/л. Вивчали функціональний стан серцево-судинної системи на підставі визначення показників частоти серцевих скорочень, артеріального тиску й обчислення функціональних індексів. Статистичний аналіз проводився з використанням пакета прикладних програм SPSS 17.0 методом варіаційної статистики із застосуванням параметричних та непараметричних методів. Критичний рівень значущості для перевірки статистичних гіпотез приймався не вищим за 0,05. Результати та обговорення. Доведено, що прогресування дефіциту андрогенів супроводжувалось погіршенням функціонального стану серцево-судинної системи на тлі зменшення екскреції мелатоніну (Pk-w < 0,05). Найбільші показники мелатоніну були притаманні хлопцям з показниками тестостерону, які перевищували 12,0 нмоль/л, — 53,4 (22,8; 69,6) нмоль/добу. У підлітків групи 1 рівень мелатоніну склав 47,2 (36,9; 68,7) нмоль/добу, групи 2 — 43,9 (23,7; 55,2) нмоль/добу, групи 3 — 36,9 (25,4; 57,1) нмоль/добу. Визначено негативний зв’язок мелатоніну із частотою серцевих скорочень (r = –0,40; p < 0,1), показниками систолічного (r = –0,32; p < 0,1) та діастолічного артеріального тиску (r = –0,38; p < 0,1). Під час проведення регресійного та факторного аналізу підтверджено вплив мелатоніну на стан серцево-судинної системи. Висновки. Materials and methods. A total of 185 girls with OM aged 13—17 years (142 with OM I and 43 with OM II) and 107 girls with a regular menstrual cycle (regMС — comparison group (CG)) were examined. We analyzed clinical and anamnestic indicators, data of ultrasound of the pelvic organs, LH, FSH, estradiol, and testosterone levels. Results and discussion. Family history of impaired function of the reproductive system in girls with OM II were less likely than with OM I (12.5 vs. 25.8 %; рφ < 0.002). Mothers of girls with OM gave birth more often at an early reproductive age than in CG (14.5 and 4.9 %; рφ < 0.001). Delayed menarche (15 years and later) with OM I was 5 times more frequent than in CG (24,1 vs 4,7 %; рφ< 0,001), while in OM II its frequency did not differ significantly from CG. Normal body mass index (BMI) in OM was less frequent than in women with reg MС (40.8 and 72.2 %, pφ < 0.001). In OM II, uterine hypoplasia was less common than in OM I (38.5 and 53.9 %; pφ < 0.05). Hypogonadotropinemia was more frequent with OM II than with OM I (21.6 versus 4.3 %; рφ < 0.001), and an increase in blood testosterone levels was more common with OM I than with OM II (24.1 versus 5.6  %; рφ < 0.01). Restoration of menstrual dysfunction was more frequent with OM II than with OM I (79.5 versus 63.4 %; рφ < 0.03). Conclusions . OM I is characterized by delayed menarche, hypoplasia of the uterus, family history of impaired function of the reproductive system and increased testosterone levels. Матеріали та методи. У 233 підлітків 14—17 років визначено стан тиреоїдної системи, характер статевого та фізичного розвитку. Виділено такі групи: контрольна група — практично здорові хлопці; підлітки з нормальним об’ємом щитоподібної залози та затримкою статевого розвитку (ЗСР); підлітки з дифузним нетоксичним зобом (ДНЗ) та нормальним статевим розвитком; підлітки із ДНЗ та ЗСР. У сироватці крові радіоімунним методом визначали рівень СТГ, ІРФ-1 та статевих гормонів, методом імуноферментного аналізу — тиротропіну, вільного тироксину та вільного трийодтироніну. Математичну обробку отриманих результатів проведено за допомогою пакетів програм SPSS Statistics 17,0, Excel з визначенням основних статистичних параметрів ряду. Результати та обговорення. Підлітки із ДНЗ та нормальним статевим розвитком мали рівень СТГ ((3,65; 1,00—8,15) МО/л) та ІРФ-1 ((290,80; 157,00—370,00) нг/мл), який достовірно не відрізнявся від контролю, що зумовлено коливанням показників у широких межах. У хлопців із ДНЗ та ЗСР частіше, ніж в однолітків із ЗСР, діагностували низькорослість, яка супроводжувалася дефіцитом маси тіла при відсутності достовірних відмінностей у клінічних ознаках гіпогеніталізму. Серед підлітків із ДНЗ, низьким зростом та ЗСР реєстрували найменший рівень базального ІРФ-1, СТГ та зменшення резервних можливостей соматотрофів. Тісний взаємозв’язок між функціональним станом системи СТГ—ІРФ- 1, тиреоїдними і статевими гормонами підтверджено під час регресійного аналізу, а характер взаємовідношень між ними залежав від перебігу пубертатного періоду та наявності зоба. Висновки. With the increase in the frequency of obesity in the population, the frequency of such comorbid diseases as type 2 diabetes, arterial hypertension, ischemic heart disease increases. In this regard, a great deal of attention from the specialists of various branches of medicine is given to the metabolic syndrome, the component of which is obesity. Objective — to determine the predictors of the development of metabolic syndrome in children aged 10 to 18 years in order to prevent the development of complications in this contingent. Materials and methods. The analysis of the medical records of 52 children has been carried out. Inclusion criteria: the age of children 10—18 years, the presence of excessive body weight and obesity due to excessive energy resources. Exception criterion: presence of secondary forms of obesity in a child. Excessive body weight was established according to the recommendations of The Endocrine Society (2017). The blood pressure was evaluated according to the AAPguidelines for hypertension in children. Results and discussion. The study found the predominance of boys over girls. Excessive body weight was established in 16 children, and 36 children were obese. In 28 examined children, increased systolic and diastolic blood pressure above 95 percentiles was detected. Of these, 23 children had obesity, and in 2 children with elevated blood pressure, glucose intolerance was diagnosed. Increased risk of elevated blood pressure in children with obesity was established, 3.8 times compared with the group of children with excess body weight. All surveyed people are offered a rehab program. Conclusions. Materials and methods. The study involved 47 adolescents with hypoandrogenism aged 13—18 years, who underwent a set of standard and special clinical and laboratory examination on the basis of the department of endocrinology SI «Institute of Children and Adolescents Health Care of the NAMS of Ukraine». Patients with hypoandrogenism were divided into groups depending on the index of masculinization. Mathematical processing of the study results was carried out using software packages SPSS Statistics 17.0, Excel. Results and discussion. It was found that complaints of abdominal pain and dyspeptic symptoms were observed in 62 % of cases. The conventional biochemical testing of liver function determined that the average levels of total bilirubin and liver enzymes were within normal range, suggesting that boys with hypoandrogenism probably had no significant violations of the functional state of the liver yet. Analysis of the lipid profile of children and adolescents with hypoandrogenism revealed that 75 % of patients had pathological changes in lipid levels, including 85 % with increased total cholesterol, 34 % with decreased HDL cholesterol and increased TG, 34 % with increased LDL cholesterol, 36 % with increased atherogenic coefficient. Atherogenic­chan­ges in the lipidogram increased with age. The study of indicators of free radical oxidation process revealed that in boys with hypoandrogenism the level of the final product of lipid peroxidation — malondialdehyde remained at the level of the control group, and the level of carbonated proteins tended to decrease with the increasing in the degree of delayed puberty. A significant reduction of the glutathione peroxidase level respectively to the deficiency of androgens was found. Thus, in adolescents with hypoandrogenism, a tendency towards the formation of the state of oxidative stress was revealed, which is confirmed by an increase of the integral coefficient of oxidative stress. Reducing the activity of the most powerful part of antioxidant system, in particular enzymatic, can lead to a «second hit» in the formation of non­alcoholic fatty liver disease and contribute to the transformation of liver steatosis into steatohepatitis. The results of ultrasound examination indicated the presence of functional disorders of the biliary tract in all boys with hypoandrogenism, and in one third of patients there were clear signs of steatohepa­tosis and gallstone disease. Conclusions. The results of the study indicated the formation of comorbid pathology of the hepatobiliary system in boys with hypoandrogenism. In 30 % of patients, ultrasound signs of steatohepatosis, atherogenic dyslipidemia and inhibition of antioxidant defense in the form of a significant decrease in the activity of glutathione peroxidase were detected. Materials and methods. A survey of 29 adolescents aged 13—18 years with HA was conducted. The comparison group consisted of 84 boys of similar age with normal sexual and physical development. The concentration of testosterone, indicators of free radical oxidation, antioxidant system and lipid metabolism were measured. The results were aggregated using the Wilcoxon—Mann—Whitney criteria, the Fisher angular transform, and the Pearson coefficient. Results and discussion. It has been established that the majority of adolescent boys with HA have type IIa dyslipidemia,which is recorded 2.5 times more oftenwith a high level of oxidative stress. Various interrelationships between pro­ and antioxidant systems with indices of blood lipid spectrum in adolescent boys with HAare revealed, the pattern depends on the degree of oxidative stress. In boys, with normal (physiological) value of the oxidative stress coefficient, multiple intersystem interactions were found indicating the main role of the antioxidant defense system in regulating lipid metabolism, which indicates a suffici­ently high compensatory and adaptive capabilities of the body to maintain the functional state at the proper level. With an increase in the degree of oxidative stress, there are other relationships between the studied parameters with the connection of the mechanisms of the regulatory influence of testosterone and free radical oxidation. In boys, with normal (physiological) value of the oxidative stress coefficient, multiple intersystem interactions were found indicating the main role of the antioxidant defense system in regulating lipid metabolism, which indicates a sufficiently high compensatory and adaptive capabilities of the body to maintain the functional state at the proper level. With an increase in the degree of oxidative stress, other relationships between the studied parameters occur, withemergence of regulatory influence of testosterone and free radical oxidation. Conclusions.
