Recently, many studies have been devoted to the study of type 2 diabetes mellitus (DM2) worldwide. In most countries of the world, the increase in the prevalence of DM2 among children and adolescents is accompanied by an increase in childhood obesity. The presence of obesity or overweight in children can complicate the diagnosis of various types of DM. Detection and treatment of DM2 is extremely important for the society due to the wide range of severe diabetes complications.The aim of this work was to analyze the frequency of DM2 according to the Register of patients with DM, and to determine the state of glycemic control in children with DM2. Materials and methods. An analysis of glycemic control in children with DM2 who received various treatment regimens: with diet and lifestyle modifications, or with additional oral hypoglycemic agents (metformin monotherapy, or combination with insulin therapy). The glycemic control was assessed by measurement of glycated hemoglobin (HbA1c) level. Results. According to the data Register in Ukraine, the prevalence of DM among children has a tendency to rise — from 8.6 per 10 thousand children in 2005, to 13.14 — in 2019 year. The prevalence of DM2 in 2019 was 0.47 % of all cases of DM, and its prevalence contained 0.062 per 10 thousand children under 19 years of age with significant differences in the frequency of its diagnosis in different regions of Ukraine. We analyzed the state of glycemic control in children with DM2 aged from 9 up to 19 years old (Me 16.2 [15.5; 18.0]), with a  disease duration 0.5—12 years (Me 4.5 [1.0] ; 7.5]). The age of diagnosis of DM2 was 2-17 years old (Me 11.7 [8.5; 15.0]), and 91.3 % of patients had obesity. Diet and lifestyle modifications were used in 34.8 % of patients, metformin monotherapy in 52.2 %, and metformin and insulin combination therapy in 13 % of patients. HbA1c values ranged from 5.0 to 11.4 % (Me 7.2 % [5,8; 8,0]). The proportion of children who had ideal or optimal glycemic control (HbA1c 7.0—7.5 %) was 52.2 %, suboptimal (HbA1c 7.6—9.0 %) — 39.1 %, high-risk glycemic control (HbA1c> 9.1 %) — 8.7 % of children. The most unsatisfactory HbA1c levels were registered in children who received metformin in combination with insulin (Me 10.6 % [10,15; 11,4]). Chronic complications of DM2 have not been reported.Conclusions. We found a low prevalence of DM2 among the pediatric population in Ukraine, more likely due to the low quality of its detection among children diagnosed with DM, as well as among children among high risk groups. Most children with DM 2 were obese. The majority of patients with DM2 (52.2 %) received metformin monotherapy, more than a third did not receive oral hypoglycemic agents. In most children with DM2 (52,2 %), the state of glycemic control corresponded to the optimal (< 7.5 %), but higher levels of HbA1c were found children who received metformin in combination with insulin.","PeriodicalId":243962,"journal":{"name":"Ukrainian Journal of Pediatric Endocrinology","volume":"1999 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2021-04-13","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"116685018","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Anonymous survey have been conducted of 1505 students of eleven general secondary education institutions with a health promoting school for students in Dnipro in February‑March 2019. The study involved 422 6th graders (197 boys and 213 girls), 482 7th graders (271 boys and 333 girls) and 784 9th graders (243 boys and 216 girls).Results. The study analyzed the frequency of consumption of certain foods, which in our opinion provide information about the diet of this age group. Frequency of daily consumption of sweets 28 %, cakes 18 %, meat 50 %, sausages 30 %, fish 10 %, milk 43 %, fresh vegetables 62 % and fruits 55 %, and, of course, products that can be potentially harmful for use by teenagers: crisps or crackers 7 %, hot dogs 4 %, ketchup and sauces 12 %. It was also found that the single‑parent family was a risk factor for more intensive consumption by adolescents of «fast food» (F = 9.8; p = 0.02), food for taken out (F = 17.3; p < 0.001), crisps, crackers (F = 12.2; p = 0.001) and quick meals (F = 18.9; p < 0.001). Mononuclear families are only 19.4 — 24.0 % %. The presence of several children in the family influenced the parents’ greater attention to the family’s consumption of fresh vegetables (F = 7.5; p = 0.006) and fruits (F = 7.3; p = 0.007). The three‑generation family paid more attention to regular consumption of meat (F = 4.1; p = 0.04) and fermented milk products (F = 4.0; p = 0.04).Conclusions. The family, which provides for the livelihood of adolescents, has played the most active role in the formation and promotion of eating behavior. The structure of food preferences of primary school students remains quite stable from the sixth to the ninth grade and differs significantly depending on the sex and composition of the family, which indicates the dependence of eating behavior of adolescents from the family. Even when parents want to create a health‑friendly educational environment, only up to 70 % of students receive fresh vegetables and fruits every day and a limited amount of complete protein (no more than 20 % fish and 60 % meat). In our opinion, medical and preventive care programs should be aimed on parents, taking into account their socio‑economic aspects and education.","PeriodicalId":243962,"journal":{"name":"Ukrainian Journal of Pediatric Endocrinology","volume":"67 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2020-12-24","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"121827563","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Materials and methods. This article includes clinical data, laboratory parameters, treatment and followupdata of the patient with APS1. Also, a review of the latest data in world literature was made. Results and discussion. The described patient has two main criteria for APS1: hypoparathyroidism and chronic mucosal candidiasis, as well as some additional ones such as: autoimmune thyroiditis with hypothyroidism, type 1 diabetes, malabsorption syndrome. Periodic seizures and gait abnormality may be due to a chronic inflammatory demyelinating polyneuropathy. The diagnosis of APS1 is to detect the genetic anomaly underlying the syndrome: mutations in the AIRE gene and identification of organspecific antibodies. International studies have shown that 100 % of patients with APS1have autoantibodies that interact with interferon omega, so tests for such autoantibodies can help to get quick diagnosis. The treatment consists of correction of endocrine insufficiency with hormone replacement therapy, and specific treatment of other diseases common to APS1. Conclusions . APS1 is extremely diverse in its symptoms and the progression of the disease causes the expanding of clinical spectrum of its components. Therefore this syndrome must be diagnosed in the early stages, taking into account high morbidity and mortality. The main purpose of treatment is to preserve the patient’s life and maintain its quality good. Awareness of this rare autoimmune polyendocrine disorder can help a doctor to recognize it and treat timely, preventing potential deathrelated illnesses.","PeriodicalId":243962,"journal":{"name":"Ukrainian Journal of Pediatric Endocrinology","volume":"34 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2019-07-26","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"133959102","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
