[Risk factors associated with oral leukoplakia: a cross-sectional study of 430 patients]. [与口腔白斑相关的危险因素:430例患者的横断面研究]。

{"title":"[Risk factors associated with oral leukoplakia: a cross-sectional study of 430 patients].","authors":"X N Chen, Z R Wang, T H Jin, Z T Zhou, C X Li, L J Shi","doi":"10.3760/cma.j.cn112144-20241215-00483","DOIUrl":"10.3760/cma.j.cn112144-20241215-00483","url":null,"abstract":"<p><p><b>Objective:</b> To analyze the epidemiological information of oral leukoplakia and explore the relationship between clinical characteristics and different degrees of epithelial dysplasia. <b>Methods:</b> A total of 430 patients with oral leukoplakia confirmed by pathology in the Department of Oral Medicine, Shanghai Ninth People's Hospital, Shanghai Jiao Tong University School of Medicine from February 2013 to May 2021 were enrolled. Epidemiological information was recorded, and univariate and multivariate Logistic regression analysis were performed on age, gender, smoking, drinking, lesion size, lesion site and lesion type to evaluate the correlation between the risk factors and the degree of epithelial dysplasia in oral leukoplakia. <b>Results:</b> A total of 430 valid samples were included in this study, among which 23 patients were diagnosed with oral leukoplakia with concurrent cancer. The age of all patients was (52.9±12.9) years (22-85 years); 213 were male and 217 were female. One hundred and seventy-six cases (40.9%) had lesions on the lateral and ventral tongue, and 254 cases (59.1%) had lesions in other locations. Three hundred and two cases (70.2%) had homogeneous lesions, and 128 cases (29.8%) had heterogeneous lesions. The degrees of epithelial dysplasia included none [11.9% (51/430)], mild [51.9% (223/430)], moderate [24.4% (105/430)], severe [6.5% (28/430)], and concurrent cancer [5.3% (23/430)]. It is commonly seen in patients with temporomandibular joint osteoarthritis. Studies have shown that BME is closely related to joint pain, disc displacement and bony degeneration, but its specific mechanism is not yet fully understood, potentially involving abnormal mechanical loading, inflammatory responses and imbalances in bone metabolism. Although BME is considered an early sign of osteoarthritis, BME persists in approximately 71.4% of patients after pain resolution , suggesting the complexity of its association with clinical symptoms. In recent years, advanced MRI techniques (such as T2 mapping and fat suppression sequences) have improved the detection rate of BME and facilitated the assessment of its dynamic changes. However, the exact role of BME in temporomandibular disorders remains controversial. The aim is to provide a reference for the rational selection of CBCT scanning FOV for TMJ imaging in clinical practice. &lt;b&gt;Methods:&lt;/b&gt; A total of 46 patients with temporomandibular disorders [22 males, 24 females; median age 24.5 years (range from 12 to 84 years)] were enrolled from the Department of Stomatology,the First Medical Centre, Chinese PLA General Hospital, between January 2023 and January 2025. All patients underwent CBCT scanning of the temporomandibular joints with both large and small fields of view using the same CBCT device, Newtom 5G (QR S.r.l, Italy), including small-field-of-view (sFOV 6 cm×6 cm, 46 cases, 92 sides) and large-field-of-view (lFOV 15 cm×12 cm for 26 cases with 52 sides or 18 cm×16 cm for 20 cases with 40 sides). A self-matched design was used to compare imaging parameters (scan time, exposure time, reconstruction voxel, radiation dose, data volume), image quality, and lesion detection efficacy (articular surface flattening, surface erosion, osteophyte, subcortical sclerosis, subcortical cyst, intra-articular calcification). Additionally, differences in joint spaces, condylar head height, and condylar height were analyzed. &lt;b&gt;Results:&lt;/b&gt; In the sFOV group, scan time (72 s &lt;i&gt;vs&lt;/i&gt;. 24 s) and exposure time (14.6 s &lt;i&gt;vs&lt;/i&gt;. 4.8 s) were significantly longer than those in the lFOV group, with smaller reconstruction voxels (0.15 mm &lt;i&gt;vs&lt;/i&gt;. 0.3 mm). Radiation dose in the sFOV group [(199.94±5.52) mGy·cm] was significantly higher than that in the lFOV 15 cm×12 cm subgroup [(96.20±25.34) mGy·cm, &lt;i&gt;t&lt;/i&gt;=-20.29, &lt;i&gt;P&lt;/i&gt;&lt;0.001] and the 18 cm×16 cm subgroup [(101.73±13.49) mGy·cm, &lt;i&gt;t&lt;/i&gt;=-31.34, &lt;i&gt;P&lt;/i&gt;&lt;0.001]. In terms of data volume, the sFOV group [(274.18±1.74) MB] was larger than the lFOV 15 cm×12 cm subgroup [(208.83±20.13) MB, &lt;i&gt;t&lt;/i&gt;=-16.75, &lt;i&gt;P&lt;/i&gt;&lt;0.001], while the lFOV 18 cm×16 cm subgroup [(386.39±1.63) MB] was significantly larger than the sFOV group [(274.83±1.78) MB, &lt;i&gt;t&lt;/i&gt;=214.49, &lt;i&gt;P&lt;/i&gt;&lt;0.001]. sFOV images showed clearer anatomical margins, distinguishable trabecular bone, and fewer artifacts. The detection rate of subcortical cyst in the sFOV group (50.0%, 46/92) was significantly higher than that in the lFOV group (36.96%, 34/92, χ²=5.61, &lt;i&gt;P&lt;/i&gt;=0.018), with a moderate agreement Kappa coefficient of 0.48. Detection rates of articular surface flattening (80.43%, 74/92) and osteophyte (55.43%, 51/92) were identical between groups, with a perfect agreement &lt;i&gt;Kappa&lt;/i&gt; coefficient of 1. Early prediction and appropriate treatment of TMD are therefore critically important. Research on TMD prediction models has evolved from traditional statistical methods to machine learning and subsequently to deep learning, each offering unique advantages and limitations. Traditional statistical methods can effectively identify independent risk factors influencing treatment outcomes but generally rely on substantial prior knowledge and assumptions. Machine learning techniques can process large-scale, high-dimensional data and autonomously learning patterns and regularities within datasets. However, they exhibit strong dependence on data quality and limited model generalization capabilities. Deep learning approaches excel at automatically extracting temporal patterns and trends from time-series data while effectively capturing complex nonlinear relationships, yet they require extensive training datasets and suffer from interpretability challenges due to their inherent black-box testing. Hyperglycemia is the characteristic metabolic change in DM patients and those in the pre-diabetic stage, playing a determinative role in many complications related to DM. A number of clinical studies had revealed an association between hyperglycemia and OPMD, as well as its malignant transformation.
