To better understand the mechanism of periodontal diseases, <i>in vivo</i> and <i>in vitro</i> models are usually required. With the rapid development of tissue engineering, <i>in vitro</i> models with the advantages of easy observation, high controllability, low cost, and high efficiency has become a unique choice for current research. <i>In vitro</i> models of periodontitis are no longer limited to cellular models, researchers are increasingly inclined to develop simple, effective, and realistic models to simulate the periodontal microenvironment. {"title":"[A preliminary exploration of an intelligent system for personalized tooth morphology reconstruction based on deep learning].","authors":"M Q Yu, D Chen, Z Y Wang, F Liu, Y Y Zhang, Y P Li, J F Shen","doi":"10.3760/cma.j.cn112144-20250331-00110","DOIUrl":"10.3760/cma.j.cn112144-20250331-00110","url":null,"abstract":"<p><p><b>Objective:</b> To integrate implicit templates with deep learning techniques, a novel neural network, the tooth-deformable deep implicit network (T-DDIN), was constructed to achieve high-precision shape completion of tooth defects in a personalized manner. <b>Methods:</b> A total of 550 intraoral scan models were collected from patients treated at the Department of Orthodontics and Department of Prosthodontics, West China Hospital of Stomatology, Sichuan University (500 for training and 50 for testing), between March 2022 and March 2024. T-DDIN reconstructed defective tooth morphology using an implicit template and a latent encoding prediction network. During model evaluation, Class Ⅱ cavity defects and occlusal wear defects were simulated in the test set. Morphological restoration was performed using both traditional computer aided design (CAD) methods and the T-DDIN deep learning approach. The two methods were compared based on three-dimensional deviation, occlusal adjustment volumes, cusp angle deviation, and restoration time. <b>Results:</b> The T-DDIN group demonstrated significantly lower three-dimensional deviation for Class Ⅱ cavity defects and occlusal wear restoration [(0.14±0.05) and (0.16±0.09) mm], occlusal adjustment volumes [(0.44±0.03) and (0.49±0.03) mm<sup>3</sup>], and difference value of the tooth cusp angles (5.69°±1.90° and 6.04°±0.53°) compared to the traditional CAD group (both <i>P</i><0.001). No significant differences were observed within the T-DDIN group between the two defect types in terms of three-dimensional deviation (<i>P</i>=0.098) or occlusal adjustment volume (<i>P</i>=0.154) or difference value of the tooth cusp angles (<i>P</i>=0.196). However, in the traditional CAD group, three-dimensional deviation, occlusal adjustment volume and difference value of the tooth cusp angles was significantly higher in occlusal wear restorations than in Class Ⅱ cavity defects restorations (<i>P</i><0.001). This paper systematically reviews the latest advancements in materials, design technologies, and clinical applications of digital occlusal splints, analyzes core issues in current clinical practice such as insufficient standardization and lack of long-term efficacy evidence, and proposes a dynamic diagnosis-and-treatment protocol based on four-dimensional virtual dental patient technology. Combined with the team's developed functional anterior repositioning splint system and clinical cases, the innovative advantages in treating anterior disc displacement with reduction of the temporomandibular joint are highlighted: achieving precise jaw position regulation, functional restoration, and serialized treatment through multimodal data fusion. {"title":"[Study on mechanical stability of maxillary LeFort Ⅰ type osteotomy after fixation with mortise and tenon and absorbable plate].","authors":"Y K Zhang, Z X Shen, R Zhang, L J Dong, Y Tian, L Lu, M L Yang, G Q Yan","doi":"10.3760/cma.j.cn112144-20241206-00462","DOIUrl":"10.3760/cma.j.cn112144-20241206-00462","url":null,"abstract":"<p><p><b>Objective:</b> To evaluate the mechanical stability of the bone block after LeFort Ⅰ osteotomy with maxillary advancement using absorbable plates fixed with tenon-and-mortise structures. <b>Methods:</b> This study developed three finite element models: one for the maxillary LeFort Ⅰ osteotomy with anterior advancement fixed with absorbable plates (Model 1); another for the maxillary LeFort Ⅰ osteotomy with anterior advancement fixed with absorbable plates assisted by tenon-and-mortise structures (Model 2); and the last one for the maxillary LeFort Ⅰ osteotomy with anterior advancement fixed with titanium plates and screws (Model 3). Simulated occlusal forces were applied on the anterior and posterior teeth in each model. The displacement changes of the nasal-palatine point (NP) and posterior nasal spine point (PNS) in the finite element coordinate system were compared and analyzed. The Mises equivalent stress distributions of the metal and absorbable plates were also examined to assess the mechanical stability of the three finite element models. Clinical data from 45 patients with dentofacial deformities treated from January 2017 to January 2023 at the Stomatology Hospital of China Medical University were collected. The age of the patients was 21±3 years. Among these, 15 patients had absorbable plates for fixation, 15 had absorbable plates assisted by tenon-and-mortise structures, and 15 had titanium plates and screws fixation after maxillary advancement. All patients underwent preoperative (T0), postoperative 3 days (T1), and 6 months (T2) spiral CT scans. The CT data in DICOM format were input into digital software, which was used to calculate the distances from the NP and PNS points to the horizontal plane (HP), right sagittal plane (FZSR), and coronal plane (CP) at T1 and T2. The distances at T1 and T2 were statistically analyzed using the Wilcoxon signed-rank test with SPSS 20.0, and a P value of<0.05 was considered statistically significant. <b>Results:</b> The finite element analysis showed that in the absorbable plate-only fixation group, the maximum displacement of the NP point (mm) under anterior and posterior tooth force conditions were 0.6 and 0.12, respectively, and for the PNS point, the maximum displacements were 0.5 and 0.11. In the tenon-and-mortise-assisted absorbable plate fixation group, the displacement of the NP point was 0.40 and 0.02 mm, and the displacement of the PNS point was 0.5 and 0.015 mm. In the titanium plate-screw fixation group, the NP point displacement was 0.33 and 0.055 mm, and the PNS point displacement was 0.16 and 0.1 mm. The Mises equivalent stress on the absorbable plates with tenon-and-mortise structure was significantly lower than that in the absorbable plate-only fixation group, while the titanium plate experienced the highest Mises equivalent stress. Traditional preparation techniques, heavily reliant on operators' unaided visual estimation and empirical skills, suffer from inherent subjectivity and the inability to quantitatively verify preparation dimensions. These limitations render them inadequate to meet the sub-100-micron linear precision required for contemporary minimally invasive and esthetic restorations, which often feature maximum preparation depths below 1 mm. Emerging digital solutions, such as depth-specific preparation guides based on target restorative space analysis, have achieved reported preparation accuracies of approximately 50 μm by integrating arithmetic calculations and real-time verification of preparation depth as the pivotal geometric parameter. Further advancements, including real-time intraoral scanning-based preparation monitoring systems, are now being applied to ultra-thin and extreme-thin veneer restorations, offering a more precise and efficient paradigm for depth control. To comprehensively evaluate the efficacy of current digital tooth preparation protocols, this article systematically compares digital guidance technologies with conventional methods across three dimensions: time efficiency, economic cost, and clinical efficacy. It critically examines the synergistic use of visual magnification tools, emphasizing the necessity of concurrently addressing\" visual clarity\" and \"enhancement of manual skill stability and precision\". Notably, the target restorative space (TRS)-guided digital preparation technique under dental microscopy employs preparation depth as the master geometric parameter throughout multi-stage workflows. By combining microscopic visualization with rigid depth-specific guides, this method enables real-time verification of preparation depth, achieving unprecedented intraoperative precision. The integration of next-generation non-contact high-precision intraoral scanning systems further elevates accuracy from the sub-100-micron level to a tens-of-microns precision scale, paving the way for ultra-precise tooth preparation.
