{"title":"[Effects of lipopolysaccharide-activated interferon gene stimulator signaling on periodontal ligament cells].","authors":"Y Zhou, D L Wang, H Y Cai, R Zhang, Y Xu","doi":"10.3760/cma.j.cn112144-20250211-00043","DOIUrl":"10.3760/cma.j.cn112144-20250211-00043","url":null,"abstract":"<p><p><b>Objective:</b> To investigate the effects of lipopolysaccharide (LPS)-activated stimulator of interferon genes (STING) signaling on the biological behavior of periodontal ligament cells and its mechanism of action. <b>Methods:</b> Human periodontal ligament cells (hPDLC) were divided into the PBS group and the LPS group by stimulated with phosphate-buffered saline (PBS) and LPS derived from <i>Porphyromonas gingivalis</i> (ATCC 33277) for 12 hours, respectively. The intracellular distribution of 8-hydroxydeoxyguanosine (8-OHdG), a marker of DNA damage, and the activation level of STING signaling were detected by immunofluorescence. The source of intracellular double-stranded DNA was detected by live-cell probes. The levels of osteogenic-related proteins, such as special protein 7 (SP7), bone morphogenetic protein 2 (BMP2), cyclic guanosine monophosphate-adenosine monophosphate synthase (cGAS), and STING were detected by Western blotting. The cell supernatants of the PBS group and the LPS group were collected, and the levels of inflammatory cytokines, such as interleukin (IL)-1β, IL-6, and interferon (IFN)-β, were detected by double-antibody sandwich enzyme-linked immunosorbent assay (ELISA). A total of 12 cgas knockout mice and 12 littermate wild-type mice were constructed. The maxillary second molars of the mice were ligated with silk or sham surgery, respectively. After 7 days of modeling, the mice were divided into littermate control sham surgery group, littermate control periodontitis group, cgas knockout sham surgery group, and cgas knockout periodontitis group, with 6 mice in each group. Micro-CT was used to collect image data, and three-dimensional reconstructions were performed on the maxillary samples of each group. The distance from the cemento-enamel junction to the alveolar bone crest (CEJ-ABC), bone volume fraction (BV/TV), and bone mineral density (BMD) in the model area were statistically analyzed using CTAn and CTVOX software. Frozen sectioning was used to obtain sections of the maxillary molars of each group of mice, and the signal intensities of cGAS and STING proteins were detected by immunofluorescence. <b>Results:</b> Immunofluorescence results showed that the fluorescence signal intensity of 8-OHdG outside the nucleus in the LPS group (4.09±0.24) was significantly higher than that in the PBS group (1.00±0.10) (<i>t=</i>20.33, <i>P<</i>0.001). The co-localization signal of mitochondrial marker TOM20 and 8-OHdG (8.56±0.53) were significantly higher than that of PBS group (1.00±0.09) (<i>t=</i>24.37, <i>P<</i>0.001). Live cell DNA probe detection showed that the signal intensity of double-stranded DNA in LPS group (3.23±0.12) was significantly stronger than that in PBS group (1.00±0.17) (<i>t=</i>18.30, <i>P<</i>0.001). Immunofluorescence demonstrated a significant increase in STING expression in hPDLC of the LPS group (<i>t=</i>6.42, <i>P<</i>0.001), and it was colocalized with the Golgi marker GM130. Teeth were randomly divided into modeling group and validation group with 7∶3 ratio according to the random number table. The modeling group was divided into direct implantation group and auxiliary bone grafting group on the basis of whether auxiliary bone grafting was performed 6 months after ARP. Univariate and multivariate analyses were conducted to identify factors influencing auxiliary bone grafting with implant placement following ARP. Nomogram was constructed using R software. Receiver operator characteristic (ROC) curve and calibration curve were drawn to evaluate model differentiation and consistency. The decision curve analysis (DCA) was used to assess the clinical application value of the model. &lt;b&gt;Results:&lt;/b&gt; Age (&lt;i&gt;OR&lt;/i&gt;=1.06, &lt;i&gt;P&lt;/i&gt;=0.001), maximum attachment loss (AL) (&lt;i&gt;OR&lt;/i&gt;=1.75, &lt;i&gt;P&lt;/i&gt;&lt;0.001), reason of tooth extraction (&lt;i&gt;OR&lt;/i&gt;=12.73, &lt;i&gt;P&lt;/i&gt;&lt;0.001), smoking [&lt;10 cigarettes/d (&lt;i&gt;OR&lt;/i&gt;=7.59, &lt;i&gt;P&lt;/i&gt;&lt;0.001);≥10 cigarettes/d (&lt;i&gt;OR&lt;/i&gt;=28.12, &lt;i&gt;P&lt;/i&gt;&lt;0.001)] and stage of periodontitis [stage Ⅱ (&lt;i&gt;OR&lt;/i&gt;=2.57, &lt;i&gt;P&lt;/i&gt;=0.430); stage Ⅲ (&lt;i&gt;OR&lt;/i&gt;=21.00, &lt;i&gt;P&lt;/i&gt;=0.007); stage Ⅳ (&lt;i&gt;OR&lt;/i&gt;=76.50, &lt;i&gt;P&lt;/i&gt;&lt;0.001)] influenced the necessity for auxiliary bone grafting with implant placement after ARP. After multivariate analysis of the above influencing factors, it was found that smoking [&lt;10 cigarettes/d (&lt;i&gt;OR&lt;/i&gt;=7.02, &lt;i&gt;P&lt;/i&gt;=0.009);≥10 cigarettes/d (&lt;i&gt;OR&lt;/i&gt;=10.27, &lt;i&gt;P&lt;/i&gt;=0.026)] was an independent risk factor for the need of auxiliary bone grafting with implant placement after ARP. The area under the ROC curve for internal verification was 0.90 (95&lt;i&gt;%CI&lt;/i&gt;: 0.84-0.97), and the H-L goodness of fit test results were χ&lt;sup&gt;2&lt;/sup&gt;=4.79, &lt;i&gt;P&lt;/i&gt;=0.780, indicating a good agreement. The area under the externally verified ROC curve was 0.97 (95&lt;i&gt;%CI&lt;/i&gt;: 0.92-1.00), suggesting that the fitting effect was slightly lower than that of the modeling group, and the predicted value of the model was slightly lower than the true value, which might underestimate the risk of additional surgery in patients. &lt;b&gt;Results:&lt;/b&gt; of H-L goodness of fit test were χ&lt;sup&gt;2&lt;/sup&gt;=5.03, &lt;i&gt;P&lt;/i&gt;=0.754. {"title":"[Research on surface modification of pure titanium with histatin coating and its effects on adhesion and spreading of mouse bone marrow mesenchymal stem cells].","authors":"S Wang, B Y Chen, L Zhao, C Liu","doi":"10.3760/cma.j.cn112144-20241024-00398","DOIUrl":"10.3760/cma.j.cn112144-20241024-00398","url":null,"abstract":"<p><p><b>Objective:</b> To prepare histatin 1 (Hst 1) and D-histatin 1 (D-Hst 1) coatings on pure titanium surfaces and to evaluate their impact on the initial adhesion and spreading of mouse bone marrow mesenchymal stem cells (mBMSC). <b>Methods:</b> The test pieces were divided into four groups: pure titanium, piranha solution-treated, 3-aminopropyltriethoxysilane-treated, Hst 1 modified groups or D-Hst 1 modified groups, with five pieces in each group. The surface hydrophilicity of the test pieces was detected by contact angle measurement. The surface microtopography was observed using atomic force microscopy (AFM), and the surface elemental composition was analyzed by X-ray photoelectron spectroscopy (XPS). The modification of Hst 1 on the titanium surface and the early adhesion and spreading of the third-generation normal mBMSC on Hst 1/D-Hst 1 modified group were observed using confocal laser scanning microscopy (CLSM). <b>Results:</b> The Hst 1 modified group had better surface hydrophilicity (contact angle 51.63°±3.80°) compared to the pure titanium group (contact angle 60.36°±4.38°). AFM observations revealed shallow mechanical scratches on the surfaces of all test pieces. CLSM and XPS elemental analysis confirmed the successful modification of Hst 1 on the titanium surface. Compared with the pure titanium group, both Hst 1 and D-Hst 1 could promoted the early adhesion of mBMSC. The Hst 1 modified group promoted the spreading of mBMSC after early adhesion, while the D-Hst 1 modified group showed no statistically significant difference in promoting mBMSC spreading compared to the pure titanium group (<i>P</i>>0.05). <b>Conclusions:</b> The modification of Hst 1/D-Hst 1 on pure titanium surfaces using Schiff base reaction is effective. {"title":"[Feasibility study on early orthodontic intervention in stage Ⅳ periodontitis].","authors":"J Q Bao, L H Lei, Z X Wang, Y M Feng, W L Sun, L L Chen","doi":"10.3760/cma.j.cn112144-20241205-00458","DOIUrl":"10.3760/cma.j.cn112144-20241205-00458","url":null,"abstract":"&lt;p&gt;&lt;p&gt;&lt;b&gt;Objective:&lt;/b&gt; To compare the effects of early orthodontic intervention and conventional sequential periodontal-orthodontic treatment to periodontal health in patients with stage Ⅳ periodontitis. &lt;b&gt;Methods:&lt;/b&gt; A total of 30 patients with stage Ⅳ periodontitis, who underwent combined periodontal and orthodontic therapies at the Department of Periodontology, the Second Affiliated Hospital of Zhejiang University School of Medicine, from January 2018 to August 2024, were included. Patients who underwent early orthodontic intervention were initiated simultaneously or within one month after supragingival scaling and subgingival root planning (&lt;i&gt;n&lt;/i&gt;=15). While patients in control group accomplished supragingival scaling, subgingival root planning, and corresponding periodontal surgeries to achieve inflammation control before starting orthodontic treatment (&lt;i&gt;n&lt;/i&gt;=15). Periodontal parameters, including clinical attachment loss (CAL), probing depth (PD), and bleeding on probing (+) % [BOP (+) %], were measured at baseline, one year after orthodontic treatment, and at the end of combined periodontal-orthodontic therapy respectively. Improvements in periodontal parameters and differences in tooth loss between the two groups were compared. &lt;b&gt;Results:&lt;/b&gt; After receiving combined periodontal-orthodontic treatment, the CAL of the early orthodontic intervention group significantly decreased from (4.39±0.90) mm before treatment to (2.41±0.35) mm at the end of treatment (&lt;i&gt;t&lt;/i&gt;=7.92, &lt;i&gt;P&lt;/i&gt;&lt;0.001). Similarly, the PD significantly reduced from (4.20±1.04) mm before treatment to (2.20±0.38) mm at the end of treatment (&lt;i&gt;t&lt;/i&gt;=7.01, &lt;i&gt;P&lt;/i&gt;&lt;0.001). The BOP(+)% also showed a significant improvement, decreasing from 89.29% (68.00%, 100.00%) before treatment to 13.04% (7.14%, 17.86%) at the end of treatment (&lt;i&gt;Z&lt;/i&gt;=-3.41, &lt;i&gt;P&lt;/i&gt;&lt;0.001). There were no statistically significant differences between the early orthodontic intervention group and control group in terms of baseline mean CAL, mean PD, and BOP(+)% (&lt;i&gt;t&lt;/i&gt;=1.30, &lt;i&gt;P&lt;/i&gt;=0.205; &lt;i&gt;t&lt;/i&gt;=1.28, &lt;i&gt;P&lt;/i&gt;=0.212; &lt;i&gt;Z&lt;/i&gt;=0.58, &lt;i&gt;P&lt;/i&gt;=0.559). Furthermore, the improvements in CAL and PD between the two groups were not significantly different compared to baseline (&lt;i&gt;Z&lt;/i&gt;=-1.10, &lt;i&gt;P&lt;/i&gt;=0.272; &lt;i&gt;Z&lt;/i&gt;=-0.93, &lt;i&gt;P&lt;/i&gt;=0.351). However, the number of missing teeth was significantly lower in the early orthodontic intervention group than in the control group (χ²=3.96, &lt;i&gt;P&lt;/i&gt;=0.047). The duration of combined periodontal-orthodontic treatment in the early orthodontic intervention group was [33.13 (23.37, 36.20) months], which was significantly shorter than that in the control group [37.47 (32.33, 50.90) months] (&lt;i&gt;Z&lt;/i&gt;=2.07, &lt;i&gt;P&lt;/i&gt;=0.037). &lt;b&gt;Conclusions:&lt;/b&gt; Both early orthodontic intervention and conventional periodontal-orthodontic treatment significantly improved CAL, PD, and BOP(+)% in stage Ⅳ periodontitis patients. Samples were taken on the 7th and 14th day after the operation. Micro-CT and HE staining were used to evaluate bone regeneration. Tissue proteins in the modeled area were collected, and Western blotting analysis (WB) was used to detect the protein expression levels of alkaline phosphatase (ALP), Runt-related transcription factor 2 (RUNX2), and osteocalcin (OCN). BMSC were isolated from WT and Ckap4&lt;sup&gt;-/-&lt;/sup&gt; mice. The expression of surface markers CD29, Sca-1, CD44, CD45, CD34, and CD11b was detected by flow cytometry, and cell proliferation ability was detected by 5-ethynyl-2'-deoxyuridine (EdU). After 7 days of osteogenic induction, real-time fluorescence quantitative PCR (RT-qPCR) and WB were used to detect the expression levels of RUXN2, ALP, OCN, protein kinase B (AKT), and phosphorylated protein kinase B (p-AKT). After 21 days, alizarin red staining and cetyl pyridine chloride quantification were used to detect the differences in mineralized nodule formation in each group. In CKAP4 gene knockout BMSC, the small-molecule AKT agonist sc79 (4 μg/ml) was added as the intervention group (Ckap4&lt;sup&gt;-/-&lt;/sup&gt; +sc79), and dimethyl sulfoxide (DMSO) treatment was used as the control group (Ckap4&lt;sup&gt;-/-&lt;/sup&gt; +DMSO). After osteogenic induction, RT-qPCR, WB, and alizarin red staining were used to compare the osteogenic differentiation differences between the two groups of cells. &lt;b&gt;Results:&lt;/b&gt; The micro-CT results showed that at 7 days and 14 days after surgery, the new bone volume in the Ckap4&lt;sup&gt;-/-&lt;/sup&gt; group [(0.070±0.010) and (0.146±0.019) mm&lt;sup&gt;3&lt;/sup&gt;] was significantly lower than that in the WT group [(0.094±0.006) and (0.196±0.013) mm&lt;sup&gt;3&lt;/sup&gt;] (both &lt;i&gt;P&lt;/i&gt;&lt;0.01). HE-stained histological sections showed that the area of new bone tissue in the Ckap4&lt;sup&gt;-/-&lt;/sup&gt; group at 7 days and 14 days after surgery [(0.101±0.008) and (0.158±0.010) mm&lt;sup&gt;2&lt;/sup&gt;] was also significantly lower than that in the WT group [(0.116±0.005) and (0.183±0.008) mm&lt;sup&gt;2&lt;/sup&gt;] (both &lt;i&gt;P&lt;/i&gt;&lt;0.05). WB was used to detect the tissue proteins in the maxillary modeling area of mice in the two groups 7 days after surgery. The results showed that the expression levels of ALP, RUNX2 and OCN in the Ckap4&lt;sup&gt;-/-&lt;/sup&gt; group were significantly lower than those in the WT group. BMSC from wild-type mice and CKAP4 knockout mice were both positively expressed for CD29, CD44, and Sca-1, and basically not expressed for CD45, CD34, and CD11b. However, its self-repair ability is limited, which often leads to functional degradation. The development of tissue engineering technology provides a new treatment strategy for TMJD repair, in which the scaffold provides basic physical support and suitable cellular microenvironment. Scaffold materials include naturally derived scaffolds, decellularised extracellular matrix scaffolds and synthetic polymer scaffolds. Currently, advancements in material composite techniques, surface modification, and chemical modification, as well as the optimization of electrospinning and three-dimensional printing technologies, have significantly enhanced the overall performance of the scaffold. These improvements exhibit promising potential for the repair of TMJD with varying degrees of defects. This paper provides an overview of the progress in tissue engineering scaffold materials in repairing of TMJD, focusing on ideal biological properties, material types, scaffold optimization strategies and clinical application.
