Simultaneously collect the dental follicle and alveolar bone tissue of obstructed teeth that need to be extracted due to orthodontic treatment as the control group. Hematoxylin and eosin (HE) staining was used to observe the morphology of dental follicle cells. Immunohistochemical staining was used to observe the expression of periostin, receptor activator of nuclear factor kappa B ligand (RANKL), and osteoprotegerin (OPG) protein in dental follicle and alveolar bone tissue. And observe the eruption of teeth after removing resistance from the crown of the permanent tooth germ.</p><p><strong>Conclusion: </strong>HE staining of two cases of dental follicle tissues showed that the volume of dental follicle cells decreased, the nuclei were condensed, and there seemed to be cellular fibrosis. The immunohistochemical staining showed that both the dental follicle and alveolar bone tissue exhibited increased expression of periostin, decreased expression of RANKL and OPG proteins, and decreased RANKL/OPG ratio. After removing resistance, the permanent tooth germ often appears to have normal eruption. Tooth eruption disorders may be accompanied by abnormal remodeling of periostin, which affects the differentiation function of osteoclasts in the dental follicle and leads to metabolic imbalance of alveolar bone, resulting in tooth eruption disorders. As an emerging psychosomatic intervention, mindfulness interventions have gained increasing attention. This scoping review systematically searches and filters relevant literature, covering databases from their inception to January 2025, aiming to comprehensively outline the application, research characteristics, and main outcomes of mindfulness interventions in cancer recovery. We employed the Arksey and O'Malley framework, utilizing electronic databases and manual searches to rigorously select studies and extract key data. Results indicate that mindfulness interventions show broad potential in improving cancer patients' mental health, physical symptoms, and quality of life, despite the heterogeneity of the evidence. Future research should focus on higher-quality randomized controlled trials to explore different intervention modalities and long-term effects and to analyze their underlying mechanisms. This is characterized by complete disruption of the perineal body, anterior defect of the internal and external anal sphincter, and loss of the distal rectovaginal and/or anovaginal septum. The common chamber incorporating vagina and recto anal outlet ensues as cloaca.</p><p><strong>Case summary: </strong>We present a case of unmarried 15-year female, who developed hematometra and was diagnosed as cervical ostia obstruction at the age of 12 year. She underwent drainage of hematometra, and later some abdominal and perineal procedures elsewhere and subsequently developed fecal discharge from the vagina. Examination revealed absence of posterior wall of vagina and anterior wall of anorectum. In addition, the anterior half of the anal sphincter was also absent. However, limited studies are available on the diagnostic accuracy of <sup>99m</sup>Tc (Technetium)-PSMA-SPECT/CT. Due to its cost effectiveness and better feasibility, it needs to be explored more extensively for its incorporation into routine clinical practice.</p><p><strong>Aim: </strong>To analyse the diagnostic accuracy of <sup>99m</sup>Tc-PSMA-SPECT/CT for detection of primary prostate carcinoma.</p><p><strong>Methods: </strong>As a prospective study in a tertiary hospital, 99mTc-PSMA-SPECT/CT was performed in 29 outpatients with suspected prostate cancer, with a median age of 66 (range: 50-82) years. The findings were compared to histopathology as the gold standard.</p><p><strong>Results: </strong>Nineteen of twenty-nine patients were positive on <sup>99m</sup>Tc-PSMA-SPECT/CT, of which 16 (84.2%) had prostate cancer on histopathology, while the remaining ten were negative on imaging, of which three had prostate cancer, leading to an overall sensitivity, specificity, and accuracy of 84.2%, 70%, and 79.3%, respectively, on visual analysis. Prostate:background and prostate:liver ratios were 37.18 ± 48.85 and 5.35 ± 7.35 in the malignant group, while 6.65 ± 5.17 and 1.14 ± 0.56 in the benign group, respectively. The area under the curve values for prostate:background and prostate:liver ratios were 0.833 (95% confidence interval [CI]: 0.677-0.990, <i>P</i> = 0.005) and 0.767 (95%CI: 0.596-0.937, <i>P</i> = 0.024), respectively, on receiver operator curve analysis. {"title":"Evidence for the association between psychological stress and peri-implant health among middle-aged and elderly adults: A systemic review.","authors":"Yen-Lan Chang, Gen-Min Lin, Shih-Ying Lin, Ren-Yeong Huang, Po-Jan Kuo, Nancy Nei-Shiuh Chang, Kun-Zhe Tsai","doi":"10.12998/wjcc.v13.i23.105762","DOIUrl":"10.12998/wjcc.v13.i23.105762","url":null,"abstract":"<p><strong>Background: </strong>Chronic psychological stress (CPS) is increasingly recognized for its detrimental effects on systemic and oral health, yet its impact on peri-implantitis remains underexplored.</p><p><strong>Aim: </strong>To evaluate the evidence linking CPS to peri-implantitis.</p><p><strong>Methods: </strong>This systematic review was conducted according to the PRISMA guidelines. Publications searching PubMed, EMBASE, MEDLINE, Cochrane Library, and ClinicalTrials.gov for human studies published in English from 1983 to December 2024. Additionally, quality assessment of selected full-text articles were performed using the modified Newcastle-Ottawa Scale.</p><p><strong>Results: </strong>From an initial total of 3964 studies, 4 cross-sectional studies comprising 432 participants met the inclusion criteria and consistently demonstrated a positive association between CPS and peri-implantitis. However, the findings are compromised by small sample sizes, study design limitations, methodological heterogeneity, and inadequate adjustment for critical confounders such as smoking and prior periodontitis.</p><p><strong>Conclusion: </strong>Cortisol levels in peri-implant sulcus fluid were linearly correlated with probing depth, with evidence suggesting this relationship may be independent of hyperglycemia. Depression emerged as the most significant CPS subtype associated with peri-implantitis. Additionally, CPS may amplify peri-implantitis inflammation by modulating cytokine expression effects. Long-term studies with larger, more diverse patient populations and careful control of confounding variables are needed to establish causality and understand the underlying mechanisms. One of the major concerns with NOM is the recurrence rate of AA following NOM.</p><p><strong>Aim: </strong>To investigate the impact of the presence of an appendicolith and its specific characteristics on the recurrence rate of AA following NOM.</p><p><strong>Methods: </strong>A retrospective analysis identified all patients treated with NOM for AA, at our institute between 2016 and 2024. Patients with an appendicolith on imaging were identified and their course and outcomes were compared with patients who were treated with NOM without an appendicolith. The primary outcome was defined as a recurrence of AA.</p><p><strong>Results: </strong>During the study period, 797 patients were treated with NOM for AA. Their mean age was 25.4 years ± 14.4 years, and 45.4% were females. Only 68 patients (8.5%) had an appendicolith identified on imaging. Patients with an appendicolith had a larger appendix diameter (10.2 mm ± 4 mm <i>vs</i> 8.5 mm ± 2.1 mm, <i>P</i> = 0.001). There was no difference in the recurrence rate of patients with and without an appendicolith (26.5% <i>vs</i> 19.1%, <i>P</i> = 0.14), however patients with an appendicolith presented with a shorter time to recurrence of appendicitis (3.9 months ± 10.4 months <i>vs</i> 5.9 months ± 8.1 months, <i>P</i> = 0.04). Due to its low recurrence rate and minimal complications, MMAE has gained increasing acceptance among clinicians in recent years. This report presents a case of diplopia following MMAE due to the presence of a potential anastomotic artery, aiming to enhance awareness of this complication.</p><p><strong>Case summary: </strong>A 60-year-old male patient presented with a headache following head trauma, and cranial computed tomography revealed a left-sided CSDH. The patient underwent left MMAE; however, polyvinyl alcohol particles inadvertently flowed into the lacrimal artery through an anastomotic artery, resulting in diplopia due to impaired abduction of the left eye. The diplopia resolved by postoperative day 40. The patient's headache resolved by postoperative day 7, and the hematoma completely resolved by postoperative day 108.</p><p><strong>Conclusion: </strong>Potential anastomotic arteries in the middle meningeal artery (MMA) can lead to serious complications. Superselective angiography of the MMA or its branches prior to embolization is essential. {"title":"Prediction of the efficacy of first transarterial chemoembolization for advanced hepatocellular carcinoma <i>via</i> a clinical-radiomics model.","authors":"Kai-Fei Zhao, Chao-Bang Xie, Yang Wu","doi":"10.12998/wjcc.v13.i23.101742","DOIUrl":"10.12998/wjcc.v13.i23.101742","url":null,"abstract":"<p><strong>Background: </strong>Hepatocellular carcinoma (HCC) is a common tumor with a poor prognosis. Early intervention is essential; thus, good prognostic markers to identify patients who benefit from first transarterial chemoembolization (TACE) are needed.</p><p><strong>Aim: </strong>To investigate the efficacy of computed tomography (CT) radiomics in predicting the success of the first TACE in patients with advanced HCC and to develop an early prediction model based on clinical radiomics features.</p><p><strong>Methods: </strong>Data from 122 patients with advanced HCC treated with TACE were analyzed. Intratumoral and peritumoral areas on arterial and venous CT images were selected to extract radiomic features, which were screened in the training cohort using the minimum redundancy maximum correlation. Then, support vector machines were used to construct the model. To construct a receiver operating characteristic curve, the predictive efficacy of each model was evaluated on the basis of the area under the curve (AUC).</p><p><strong>Results: </strong>Among the 122 patients, 72 patients were effectively treated <i>via</i> TACE, and in 50 patients, this treatment was ineffective. In the radiomics model, the areas under the curve of the venous phase model were 0.867 (95%CI: 0.790-0.940) in the training cohort and 0.755 (0.600-0.910) in the validation cohort, indicating good predictive efficacy. The multivariate logistic regression results indicated that preoperative alpha-fetoprotein levels (<i>P</i> = 0.01) were a risk factor for TACE. The screened clinical features were combined with the radiomic features to construct a combined model. This combined model had an AUC of 0.92 (0.87-0.95) in the training cohort and 0.815 (0.67-0.95) in the validation cohort.</p><p><strong>Conclusion: </strong>CT radiomics has good value in predicting the efficacy of the first TACE treatment in patients with HCC. Their ability to degrade over time and avoid secondary hardware removal makes them advantageous. However, complications such as breakage and intra-articular migration of screws can cause significant clinical issues, including joint pain, swelling, and cartilage damage. Early diagnosis and management are critical in such cases.</p><p><strong>Case summary: </strong>A 26-year-old male presented with knee pain and swelling one year after ACL reconstruction using a hamstring graft and bioabsorbable tibial interference screw. The patient had been engaged in rigorous physical activity as part of military training. Clinical examination revealed mild effusion without instability, and imaging showed screw breakage with intra-articular migration. Therapeutic arthroscopy confirmed intact graft tension, and broken screw fragments were removed successfully. The patient resumed normal activity two weeks after surgery.</p><p><strong>Conclusion: </strong>This case highlights the potential complications associated with bioabsorbable screws, emphasizing the need for meticulous surgical technique, postoperative monitoring, and timely intervention.
