{"title":"Quality of life in patients with chronic slow-transit constipation according to the PAC-QOL scale one year after surgical treatment: comparison with preoperative data and reference values.","authors":"Ivan M Leshchyshyn, Leonid Yu Markulan, Olga I Okhotska, Yaroslav M Susak, Pavlo L Byk","doi":"10.36740/WLek/203846","DOIUrl":"https://doi.org/10.36740/WLek/203846","url":null,"abstract":"<p><strong>Objective: </strong>Aim: To assess the impact of surgical treatment on quality of life in patients with chronic slow transit constipation according to the PAC-QOL scale one year after surgery.</p><p><strong>Patients and methods: </strong>Materials and Methods: PAC-QOL scores were studied in 107 patients with chronic slow-transit constipation (main group) before and one year after total colectomy (57), subtotal colectomy (29) and colectomy with low rectal resection (21). 70 patients were included into the reference group. Open surgery was performed in 70 (65.4%) patients, while laparoscopic access - in 37 (34.6%) patients.</p><p><strong>Results: </strong>Results: Despite the long-term conservative treatment PAC-QOL scores for all subscales significantly exceeded the reference values in all patients before surgery (all p<0,05). The indication for neoadjuvant hormone therapy (NAHT) is HR+/HER2- breast cancer of stage II-III. However, there are currently insufficient data on its impact on the IHC biological characteristics of tumors and clinical outcomes.</p><p><strong>Patients and methods: </strong>Materials and Methods: The study was conducted at the Uzhhorod Treatment and Diagnostic Oncology Center during 2015-2023. At the diagnostic stage, morphological verification and IHC profile were determined based on trephine biopsy material. The study included 28 patients with HR+/HER2- BC, aged from 38 to 75 years old. All patients were prescribed NAHT with the use of aromatase inhibitors for 6.10±2.23 months. The expression levels of ER, PR, Ki-67, category G were assessed before and after NAHT using histological and IHC analyses.</p><p><strong>Results: </strong>Results: After NAHT, we observed a decrease in Ki-67 levels from 25.31% to 10.25%, ER expression levels from 97% to 84%, and PR expression levels from 87% to 11%. There were changes in category G: G1 was observed in 45.83% of patients, G2 in 54.17%. The staging of BC also decreased. A moderate degree of curative tumor pathomorphosis was achieved in 76.47% of patients, and complete regression (pCR) in 5.88%.</p><p><strong>Conclusion: </strong>Conclusions: Neoadjuvant hormone therapy demonstrated high efficacy in the treatment of patients with luminal HR+/HER2- BC of stage II-III. In most cases, it was possible to reduce the proliferative activity of the tumor, which was accompanied by a decrease in tumor staging. Microscopic specimens of the heart, lungs, liver, kidneys and intestines have been reviewed, divided by the proposed four levels of pathological changes and comparable between the two groups, depending on the presence of AСS (25 and 28 patients).</p><p><strong>Results: </strong>Results: When comparing in two groups the levels of lesions of each studied organ, according to given levels of change, a statistically significant difference between groups in the level of kidney and intestine lesions (P <0.05, Mann-Whitney U-Test) was obtained. The changes in moderate and severe levels were significantly dominating in the microscopic specimens of the kidneys and intestines of group A compared to group B (p <0.001).</p><p><strong>Conclusion: </strong>Conclusions: Histopathological changes in the examined organs of patients who died from severe acute pancreatitis complicated by abdominal compartment syndrome were characterized by more pronounced ischemic and inflammatory damage in the kidneys and intestines. The variability of extrapancreatic pathological changes in SAP patients depended on intra-abdominal pressure levels. There were five groups of cadets engaged in various sports and motor activities. Research methods included bibliosemantic, psycho-diagnostic, medical and biological, and mathematical statistics methods.</p><p><strong>Results: </strong>Results: In the groups of cadets who systematically practiced the chosen sport during training (groups 1-4), mental and physical health indicators at the end of the research were better than in the cadets who practiced according to the current method of motor activity (group 5). The levels of stress resilience, nervous and emotional tension, reactive anxiety, emotional state, body mass index, vital index, strength index, and Robinson's index were significantly (p ≤ 0.05-0.001) better in groups 1-4 compared to group 5. Instead, no significant difference was found between the indicators of groups 1-4 (p > 0.05). Methods: analysis and generalization of literary sources, method of computerized stabilography (statokinesiometry), methods of mathematical statistics. The complicated Romberg stance assessed students' balance.</p><p><strong>Results: </strong>Results: The biomechanical indicators of the statokinesiogram were investigated to assess the criteria of students' stability before and after physical exertion during physical education training sessions. Students demonstrate stable statokinesiogram indicators with a significantly unchanged ellipse area of the COM oscillations before and after exertion (SEllS - 3606 and 4004 mm2; α > 5 %). The leading role of proprioceptive, amplitude-frequency characteristics of the body's COM oscillations in balance control has been proven.</p><p><strong>Conclusion: </strong>Conclusions: It has been established that maintaining balance by a student is a dynamic phenomenon that requires continuous body movement, which is the result of the interaction of vestibular and visual analyzers, joint and muscle proprioception, central and peripheral nervous systems. The cause of body mass center fluctuations is respiratory movements, blood circulation, as well as the functional state of the central nervous system and receptor apparatus. The effectiveness of the stabilography method for assessing the coordination criteria of stability (balance) of students before and after physical exertion has been proven. Research methods were bibliosemantic, medical and sociological, statistical, system analysis, and logical generalization.</p><p><strong>Results: </strong>Results: The peculiarities of stress manifestation in women under war conditions have been investigated. It has been found that 42.0 % of women are constantly in a state of fatigue and exhaustion and assess their stress as maximum. A persistent negative emotional state is experienced by 72.6 % of women, and the inability to experience positive emotions - 52.8 %. More than 70.0 % of the respondents say they do not know how to cope independently with stressful conditions. A set of preventive measures to overcome stress has been proposed and practical recommendations for preventing stress among women have been developed.</p><p><strong>Conclusion: </strong>Conclusions: Effective stress prevention measures include reducing the likelihood of stressors through self-realization, developing knowledge of the value of health, raising awareness of danger, determining one's life strategy, providing timely psychological assistance, learning to cope with stress using anti-stress techniques, and monitoring and adjusting one's reactions to stress. The Focus group discussion guide included four open-ended questions to define mothers' challenges, as well as their coping and adaptation strategies for autistic adults' independent lives.</p><p><strong>Results: </strong>Results: The thematic analysis highlighted six main themes and their subthemes. The subjects' response themes started with autism detection among children and went behind challenges and adaptation and coping mechanisms to create an independent living approach for autistic adults.</p><p><strong>Conclusion: </strong>Conclusions: Mothers emphasized the need for comprehensive support systems, including early intervention, specialized resources, and trained caregiver. The rate of quantitative dominance of other strains is significantly lower: S.epidermidis - 2.56 times, S.haemolyticus - 4.56 times, S.pyogenes - 5.86 times, E.faecalis and K.pneumoniae - 20.56 times. When determining the sensitivity of S. aureus clinical strains to antibacterial drugs, we established high levels of sensitivity to mupirocin, fusidic acid, and oxazolidinones - 95.1%, 90.2%, and 87.8%, respectively. Moreover, S. aureus dominated among the strains with a high ability to form biofilms - 68.2%, with an average ability - S epidermidis, 28.8%.</p><p><strong>Conclusion: </strong>Conclusions: S.aureus is the leading pathogen in the infectious-allergic process with paratraumatic eczema according to the constancy index of each taxon, frequency of manifestation, Margalef's species richness index, Whittaker's species diversity, the value of Simpson's and Berger-Parter's species dominance indices, as well as the population level of each taxon, the coefficient of quantitative dominance and the coefficient of significance. Dental plaque and gut microbiome may be related to EBS in the temporary dentition. The literature suggests a decreased caries prevalence in the presence of EBS which associated with low incidence of caries in children. The nature of the black pigmentation is suggested to be a form of bacterial plaque with an insoluble ferric salt. Tabaco smoke, food and antibiotics that can be also a risk factors for EBS. There are some clinical and localization features that dentist have to pay attention for. Many diseases of various systems and organs are directly related to black plaque. Youth acrobats (elite athletes): girls (n=30), average age 19.36±2.86 years; young men (n=34), average age 20.38±2.89. The computer program \"Diagnost-1\" was used in researches.</p><p><strong>Results: </strong>Results: In female acrobats, the latency period of simple visual-motor information, the latent period of the visual-motor reaction to choosing two out of three stimuli are statistically different from those of young acrobats. Boys acrobats are faster in terms of functional mobility of nervous processes than girls in both age categories (at p<0.05). A greater amount of processed information was found in acrobat boys than in adolescent girls. According to the indicators of cognitive characteristics features of sexual dimorphism between acrobats of different age categories were revealed due to different information processing strategies taking into account sexual characteristics.</p><p><strong>Conclusion: </strong>Conclusions: Boys-acrobats of puberty and adolescence showed more effective processing of simple and complex visual-motor information compared to girls-acrobats of the same age categories.
