{"title":"Effectiveness of treatment of oropharyngeal carcinoma patients.","authors":"Ervin V Lukach, Viktor Ya Dikhtiaruk, Oleksandr V Kovtunenko, Anatoliy A Bakaiev","doi":"10.36740/WLek202408102","DOIUrl":"https://doi.org/10.36740/WLek202408102","url":null,"abstract":"<p><strong>Objective: </strong>Aim: To analyze the results of treatment of patients with oropharyngeal carcinoma.</p><p><strong>Patients and methods: </strong>Materials and Methods: 276 patients with oropharyngeal carcinoma were treated in 2008-2021. Neoadjuvant chemotherapy consisted of three to six cycles: paclitaxel 175 mg/m2 and carboplatin 350 mg/m2 (or cisplatin 100 mg/m2) on the first day. The interval between cycles was 21 days. After the cycles, all patients were prescribed a course of radiation therapy in a total focal dose (TFD) of 65 Gy. The outcome of treatment was assessed by the degree of tumor regression according to RECIST criteria one month after the end of combination treatment. Statistical processing was performed using STATISTICA 6.1 software (StatSoftInc).</p><p><strong>Results: </strong>Results: The three- and five-year survival rates of the examined patients with oropharyngeal carcinoma after treatment were 40.8% respectively (95% CI 33.7 - 47.9) and 27.0%, (95% CI 20.6 - 33, 4) with a median survival of 36 months with 95% CI (35.5 - 40.2). Early detection and prompt treatment can prevent serious neurological consequences. This has become possible thanks to the implementation of newborn screening programmes. The objective of this review is to provide readers with a comprehensive understanding of the phenylketonuria and the role that neonatal screening plays in the protection of public health.</p><p><strong>Patients and methods: </strong>Materials and Methods: A review of the literature was conducted using the PubMed database, with the search period encompassing the most recently published scientific sources. Analysis of the literature. This article presents phenylketonuria as an example of an inherited metabolic disorder, outlines the treatment options, and discusses the potential implications of hyperphenylalaninemia. Furthermore, it also delineates the various aspects of health that are influenced by newborn screening.</p><p><strong>Conclusion: </strong>Conclusions: Phenylketonuria represents a significant health problem in the population. The development of screening tests has transformed healthcare, including improvements in quality of life, prognosis, and reductions in the number of comorbidities in patients. Depending on the state of reactivity of the organism, the patients were divided into 3 groups: with normo-, hyper- and with hyporeaction. The patients underwent patch surgery. After the surgery, dental biofilm was taken. Microbiological studies included the isolation and species identification of dental biofilm microorganisms, the results of quantitative studies of microflora: the level of colonization was expressed in colony-forming units per 1 ml (CFU/ml); the frequency of microorganisms isolation was expressed in absolute numbers. Statistical processing of the obtained digital data was performed using the computer program Statistica 8.0.</p><p><strong>Results: </strong>Results: The studies have shown that in different states of the body's reactivityi n patients with generalized periodontitis after flap surgery, different quantitative and qualitative composition of the microflora of the dental biofilm is determined. {"title":"Features of the macroscopic structure of the posterior inferior tibiofibular ligament based on anatomical study.","authors":"Ivan V Kucher, Andrii P Liabakh","doi":"10.36740/WLek/191326","DOIUrl":"https://doi.org/10.36740/WLek/191326","url":null,"abstract":"<p><strong>Objective: </strong>Aim: To study the anatomy of the posterior inferior tibiof i bular ligament (PITFL) and specify the features of its morphology and linear parameters.</p><p><strong>Patients and methods: </strong>Materials and Methods: The peculiarities of morphology and linear parameters of PITFL on 10 fresh amputated lower limbs were studied. The average age of the patients was 64.7 ± 9.3 (7 males, 3 females). Macroscopic characteristics, insertion, orientation in relation to the corresponding bony and ligamentous anatomical structures were studied, and linear parameters of PITFL were measured using a caliper. Average values were calculated.</p><p><strong>Results: </strong>Results: PITFL was detected in all the studied samples. It represented a strong, compact anatomical structure of a trapezoidal or triangular shape. The proximal width of the PITFL averaged 20 ± 3.65 mm, while the distal width was 36.6 ± 4.62 mm. The attachment length of PITFL to the posterior part of the tibia was 28.6 ± 5.13 mm and to the fi bula was 17.4 ± 3.2 mm. Research methods: visual method to evaluate the likelihood of a data set's normal distribution; correlation and regression analysis; method for determining the ordinal function of a graph through the application of an adjacency matrix, specif i cally utilizing Demucron's algorithm. This process is accompanied using a topological sorting of the graph.</p><p><strong>Results: </strong>Results: The results showed the lack of correlation between age and VAS, muscle strength and VAS, as well as VAS and the channel space; little feedback between age and the channel space; weak positive relationship between size of herniated intervertebral disc had a with the length of the illness. The data showed, strong feedback was observed for hernia size and the channel space, hernia size and muscle strength, the channel space, and length of the illness.</p><p><strong>Conclusion: </strong>Conclusions: The evaluation on the complexity and depth of the interconnections between clinical and instrumental metrics in athletes was conducted. The hierarchical organization within the graphical representation was ascertained. Horbachevsky Ternopil National Medical University we actively use the e-learning system Moodle (Modular Object Oriented Distance Learning Environment), to which there is free and unrestricted access. Using this program, the student takes on the role of an active subject who independently acquires knowledge, forms his/her own system of skills, of course with the help of certain sources, and the role of the teacher in this scheme is to motivate and suport learners, prepare information sources used in self-study, etc. This is due to a significant increase in the requirements for quality training of future specialists n the medical field and market conditions in Ukraine, which set before the higher school the task of training specialists of the new generation who would be highly qualified, competitive, literate, and have perfect command of their professional terminology. The Moodle system is able to optimize the learning process, promote the formation of terminological competence and master professional vocabulary. When creating educational and methodological complexes for the formation of terminological competence, the means of teaching, as well as the ways of presenting educational material and the principles of organizing the educational activities of students become important. The electronic platform Moodle, which has a wide range of resources for teaching and testing, is able to expand the communicative competencies and skills of students needed to effectively master professional Latin terminology. {"title":"Pseudomembranous colitis as a complication in Covid-19.","authors":"Stepan S Filip, Rudolf M Slivka, Andrii M Bratasiuk, Yuriy P Skrypynets, Anatoly I Shitev","doi":"10.36740/WLek202403112","DOIUrl":"https://doi.org/10.36740/WLek202403112","url":null,"abstract":"<p><strong>Objective: </strong>Aim: To improve the results of treatment of patients with pseudomembranous colitis against the background of coronavirus infection.</p><p><strong>Patients and methods: </strong>Materials and Methods: The study presents the results of a retrospective analysis of 96 patients with pseudomembranous colitis, who were treated in the infectious Covid department at the base of the Uzhhorod City Clinical Hospital since 2020 to 2022. The average age of patients was 55.2 years, there were 38 (39.5%) men and 58 (60.5%) women. Diagnosis of complications - pseudomembranous colitis (PMC) - was based on clinical data, ultrasound and CT of the abdominal organs, fibrocolonoscopy, laparoscopy.</p><p><strong>Results: </strong>Results: The frequency of PMC from the total number of patients who were in hospital treatment (8205 patients) due to COVID-19 was 1.17%, and this indicator was 0.62% in 2020, and 2.28% in 2021. Indications for operative treatment were: colon perforation - 9.4% of patients; peritonitis (diffuse, widespread) without obvious perforation of the colon wall - 85.5% of patients; mesenteric thrombosis - 4.1% of patients. In the case of perforation of the colon, resection of the colon was performed with the formation of a proximal colostomy and ileostomy. In case of mesenteric thrombosis, resection of the affected part of the small intestine was performed. Indicators of students' functional state and physical development after 1 and 3 years of HIIT were studied. The functional state was assessed by indicators of the cardiovascular and respiratory systems, and physical development - by physical education tests.</p><p><strong>Results: </strong>Results: It has been found that HIIT has a positive effect on the indicators of the functional state of the cardiovascular and respiratory systems of the body and the physical development of students. After three years of HIIT sessions, students significantly improved their heart rate, vital capacity of the lungs, Stange test, Genchi test, duration of recovery heart rate, as well as the level of development of speed qualities, strength qualities, endurance and flexibility.</p><p><strong>Conclusion: </strong>Conclusions: It has been established that the sports-oriented form of organization of physical education training sessions with the use of HIIT based on the CrossFit system is quite effective in improving the indicators of the functional state and physical development of students. Methods: analysis and generalization of scientific and methodological literature, anthropometry, physiometry, testing, and statistical methods. The health status was assessed by body mass index, Robinson index, and vital index.</p><p><strong>Results: </strong>Results: It was found that the body mass index of law enforcement officers for one year of service in practical units deteriorated in both men and women by 1.3 kg/m2 and 0.9 kg/m2, respectively. More negative changes were observed in men. At the same time, after one year of service, the number of men with overweight (27.5 %) and even obesity of the first (13.9 %) and second (5.2 %) degrees increased significantly. In women, the changes were less pronounced. The dynamics of Robinson and vital indices, as well as the level of physical fitness, were also negative.</p><p><strong>Conclusion: </strong>Conclusions: The results of the research indicate a negative impact of professional factors in terms of professional activities on the health of law enforcement officers.
