{"title":"Potential cardioprotective effect of trimetazidine in mice model of endotoxemia: role of AMPK-Nrf2.","authors":"Alaa Kadhum Mosa, Sahar Majeed, Fadhaa Abdulameer Ghafil, Najah Rayish Hadi","doi":"10.36740/WLek/190820","DOIUrl":"10.36740/WLek/190820","url":null,"abstract":"<p><strong>Objective: </strong>Aim: To clarify the potential cardioprotective effect of Trimetazidine against experimentally sepsis-caused endotoxic cardiac injury damage in mice.</p><p><strong>Patients and methods: </strong>Materials and Methods: 24 Mice were divided into four groups (n=6): Sham group, CLP group DMSO group, trimetazidine-treated group 50 mg/kg IP, 1hr before CLP, then the animals were sacrificed 24 hr after CLP and tissue sample was taken for measurement of TNF-α, TNF-αr1, IL-1β, HO-1, MPO, caspase-11, F2-isoprostane and serum troponin by ELISA and gene expression of AMPK-Nrf2 by qpcr and histopathological study.</p><p><strong>Results: </strong>Results: trimetazidine treated group showed significant changes as compared with clp group regarding TNF-α, TNF-αr1, IL-1β, HO-1, MPO, CASPASE-11, F2-ISOPROSTANE as well as affect tissue mRNA expression of AMPK-Nrf2 genes p<0.05.</p><p><strong>Conclusion: </strong>Conclusions: We evaluate that Trimetazidine has cardio protective effects due to its anti-inflammatory and anti-oxidative action. We examined the outcomes of 11 individual clinical trials in conjunction with existing literature to assess the effectiveness of nutritional interventions. Our focus encompassed protein nutrition and its components, the provision of vitamin D. Furthermore, we explored the influence of these substances in addressing sarcopenia within the context of concurrent conditions like osteoporosis, fractures, stroke, or chronic kidney disease. Sarcopenia is a disease of the skeletal muscles that causes a decline in muscle function and mass, which affects mainly elderly people. A progressive course leads to multiple serious complications, such as increased mortality, falls, and co-morbidity. Sarcopenia treatment is limited to physical activity, mainly resistance exercise training and nutritional interventions... Our investigation underscores the significant role of proper nutrition in the treatment process. The following keywords were used to find relevant materials on the research topic: \"GERD\", \"obesity\", \"pathogenesis\", \"treatment\", \"clinical trials\".</p><p><strong>Conclusion: </strong>Conclusions: A series of studies have shown that there are risk factors that increase the development of GERD: stress, bad habits, excessive body mass index (BMI), advanced age and lifestyle. {"title":"Practical experience in the surgical treatment of newborns with isolated gastrointestinal perforations based on scientific evidence.","authors":"Olga M Gorbatyuk, Taras V Martyniuk, Andrey A Momotov","doi":"10.36740/WLek/203901","DOIUrl":"https://doi.org/10.36740/WLek/203901","url":null,"abstract":"<p><strong>Objective: </strong>Aim: To share first-hand experience in the surgical treatment of newborns with isolated gastrointestinal perforations.</p><p><strong>Patients and methods: </strong>Materials and Methods: This study examines 71 newborns with perforated peritonitis: 53 (74.65%) had necrotizing enterocolitis, 14 (19.72%) had isolated gastrointestinal perforations, and 4 (5.63%) had other intestinal perforations. Diagnosis involved clinical, laboratory, radiographic, ultrasound, and histological examinations of surgical and autopsy samples.</p><p><strong>Results: </strong>Results: The most significant risk factors for isolated perforations of the gastrointestinal tract in newborns were acute birth asphyxia and pathology of the respiratory system, which required tracheal intubation in 100% of children. Isolated perforations were localized in the stomach (n=6), jejunum (n=3), ileum (n=2), duodenum (n=1), colon (n=2). Morphological features of isolated perforations are as follows: a rapid muscle layer wasting of the wall, sometimes with the absence of muscle fragments; vascular malformations in the submucosal layer of the wall; ulcerous defect without necrotic changes; absence of pneumatosis of the intestinal wall. In isolated perforations, the operation of choice was closure of perforation in 11 children. In 2 patients direct interintestinal anastomoses were performed. Case mortality rate was 21.43% (3 newborns died).</p><p><strong>Conclusion: </strong>Conclusions: 1. Clinical and pathomorphological features of gastrointestinal perforations in newborns indicate that isolated perforations are a separate nosological entity. 2. Isolated perforations of the gastrointestinal tract in newborns are characterized by such clinical differences as distress syndrome, prematurity, early onset, local lesions of a hollow organ, moderate peritonitis, favorable course and prognosis. 3. Isolated perforations are secondary to fibromuscular dysplasia of the wall of a hollow organ, indicating congenital pathology. 4. The operation of choice for isolated perforations is the excision of the edges of the perforation and closure of the perforation. 5. The prognosis for the gastrointestinal isolated perforations is favorable. This can result in serious complications such as foot instability, degeneration, or chronic pain. Due to the absence of clear diagnostic guidelines, this article reviews existing literature and research to propose an algorithm for identifying potential Lisfranc injuries. The literature review summarizes information on diagnostic methods for Lisfranc complex injuries. It was executed using PubMed, Web of Science, and Google Scholar databases. Scientific publications published between 2016 and 2024 were counted in review and scientific before this period. Lisfranc complex injuries, especially subtle cases, are frequently missed due to both patient oversight and physician error. In diagnostic trials, tools like traditional X-rays or CT scans are commonly employed, however MRIs and ultrasounds are often overlooked. Weight-bearing X-rays and weight-bearing CT could be promising due to their high sensitivity. Appropriate patient interview and physical examination along with imaging are essential points for making an accurate diagnosis. It is important to diagnose Lisfranc injuries to prevent the development of serious complications. Greater patient awareness and a comprehensive diagnostic approach by doctors are key to improving detection rates. Subtle Lisfranc injuries are the greatest diagnostic challenge. Weight-bearing CT scans and radiographs could be very helpful in identifying these injuries. The complex etiology and comorbidity of somatic diseases makes diagnosis difficult because depressive symptoms are attributed to the underlying disease. Often, the lack of treatment and ignoring the symptoms by the patient and their relatives can contribute to suicide behavior of seniors. The phenomenon of population aging is observed all over the world. The average lifespan of people is increasing, which means an increase in the number of people in senior age. Depression affects more and more elderly people. Lack of interest from family and society, illnesses, loneliness, isolation make seniors experience depressive disorders more often. It is worth taking a closer look at the problem of depression among the elderly and implementing appropriate actions to minimize the problem. Information about patients and their PD experience was collected and analyzed. The researchers focused on identifying factors related to complications, particularly peritonitis. Peritonitis was diagnosed based on symptoms and the presence of white blood cells in the dialysis fluid.</p><p><strong>Results: </strong>Results: The study included 71 participants, with a slightly higher number of male participants compared to female participants. The primary cause of kidney failure was diabetic nephropathy, affecting over one-third of patients. Diabetes combined with hypertension was the second most common cause. The most common complications experienced by patients were peritonitis, tube obstruction, and surgical site infection. Other, less frequent complications included death, catheter failure, catheter displacement, bleeding, leakage, and bowel injury.</p><p><strong>Conclusion: </strong>Conclusions: Peritonitis is the most common complication of peritoneal dialysis. While other issues can arise, many are treatable and do not harm the dialysis catheter. The aim of this study is to evaluate the psychological consequences of plastic surgery, considering the effects of such interventions on self-esteem, self-worth, quality of life, and the potential risks associated with aesthetic procedures. Research indicates that procedures such as breast augmentation or rhinoplasty can lead to significant improvements in body image perception, increased self-confidence, and enhanced social and professional functioning. However, plastic surgery also carries potential risks, particularly for patients with psychological disorders such as body dysmorphic disorder or eating disorders, where the aesthetic outcomes may not meet expectations, potentially leading to a deterioration in mental health. A critical factor in preventing adverse effects is the careful selection of patients and the conduct of a thorough psychological assessment, which allows for the evaluation of realistic expectations and the identification of potential risks associated with surgery. Two validated and reliable assessment tools were used to measure Emotional Intelligence and Locus of Control. Data were collected between January and April 2024.</p><p><strong>Results: </strong>Results: In Jeddah, 58.2% of students were aged 20-23, while in Riyadh, this age group accounted for 74.9%. More students in Jeddah (63.4%) were at the 8-12 academic level compared to Riyadh (54.1%). A strong positive correlation was found between emotional intelligence (EI) and locus of control (LOC) (r = .730, p < .001), indicating that higher EI is significantly associated with a stronger internal LOC. Additionally, both LOC and EI had weak but statistically significant positive correlations with GPA, with LOC showing r = .171 (p < .01) and EI showing r = .122 (p < .05).</p><p><strong>Conclusion: </strong>Conclusions: This study underscores the importance of Emotional Intelligence and Locus of Control in shaping academic performance among nursing students. The findings contribute to a deeper understanding of psychological dynamics within nursing education. The independent variables included demographic data (age, sex) and radiographic findings (Winter's angulation, Pell and Gregory's classification, and retromolar space), retromolar space was measured from the anterior border of mandibular ramus and the distal surface of mandibular second molar in millimeter. Dependent variables were surgical difficulty determined by technique and operation time.</p><p><strong>Results: </strong>Results: There was a statistically significant relationship between the retromolar space and Pell and Gregory's classification with the duration of surgery and technique of extraction.
