Although pharmacologic therapy remains central to symptom control, real-world treatment effectiveness is frequently compromised by suboptimal medication adherence. To synthesize contemporary evidence on the determinants, clinical consequences, and targeted strategies related to medication nonadherence in osteoarthritis. A narrative review of literature published between January 2021 and February 2026 was conducted using PubMed, Scopus, and supplementary sources. Studies examining determinants of nonadherence, associated outcomes, and adherence-enhancing interventions in OA were thematically integrated. Thirty-nine studies were included. Medication nonadherence in OA is multifactorial, reflecting the interaction of patient-related, therapy-related, and healthcare system determinants. Aging, multimorbidity, cognitive vulnerability, and polypharmacy contribute to unintentional nonadherence, whereas risk perception, safety concerns, and financial barriers frequently drive intentional dose modification or discontinuation. Inconsistent medication use is associated with persistent pain, functional limitation, reduced quality of life, and increased healthcare utilization. Emerging evidence supports patient-centered education, shared decision-making, regimen simplification, pharmacist-led interventions, and digital adherence tools as promising strategies. Medication nonadherence represents a clinically consequential yet underrecognized determinant of treatment effectiveness in osteoarthritis. {"title":"COVID-19 aggravates well tolerated spinal cord injury - related neuropathic pain.","authors":"Mohammed K Al-Chlaihawi, Akeel Abd Uljaleel Qyas, Muntadher Zyara, Salim Kadhim","doi":"10.36740/WLek/216216","DOIUrl":"10.36740/WLek/216216","url":null,"abstract":"<p><strong>Objective: </strong>Aim: To compare the intensity of the pain, medication requirements and the evolution of symptoms of neuropathic pain patients with or without COVID‑19 infection and to determine whether the severity of the infection influenced these variables.</p><p><strong>Patients and methods: </strong>Materials and Methods: In all, 400 adults with chronic neuropathic pain (≥3months duration) were included-200 positive and 200 negative for SARS‑CoV‑2. Baseline demographic and clinical characteristics were comparable between groups (age, sex, BMI, comorbidities, pain duration; all p>0.22) to meaningful outcome comparison. Pain severity, analgesic consumption, pain course and symptom deterioration were assessed pre-/post-COVID‑19 or similar follow-up for the controls.</p><p><strong>Results: </strong>Results: In COVID‑positive group score values raised from 4.6 ± 1.1 to 7.7 ± 1.4 (p<0.05), at a more significant extent in women (4.7-8.2, p<0.01) as in diabetic patients (4.6-8.1, p<0.001). The COVID-negative group had no significant change (4.2-4.3, p =0.42). Post-COVID, 72% of infected patients needed more analgesics and 65% of them other drugs, vs. 15% and 12%, respectively, in the negative group p<0.001. Pain curves trajectories evidenced steeper increments in COVID‑positive (85% vs. 25% in severe vs. mild COVID-19 worsening, p<0.001, revealing a directly related pain severity to the infection.</p><p><strong>Conclusion: </strong>Conclusions: SARS‑CoV‑2 infection is an aetiologic factor that independently leads to a substantial worsening of chronic neuropathic pain, an increase in analgesic needs, and a more severe symptom burden. The impact is most pronounced among women, diabetic patients and those with severe COVID‑19. However, inappropriate antibiotic use remains common concern in many healthcare settings. Therefore, the aims was to assess the use of surgical antibiotic prophylaxis.</p><p><strong>Patients and methods: </strong>Materials and Methods: A retrospective cross-sectional study was conducted among 419 discharged surgical and gynecological patients at Al-Salam Teaching Hospital, Mosul, Iraq, during August 2023. Patient records were analyzed for SAP indication, antibiotic selection, timing, and duration according to international guidelines. This study aimed to assess both the prescribing patterns and the appropriateness of surgical antimicrobial prophylaxis practices.</p><p><strong>Results: </strong>Results: SAP was indicated for most patients; however, appropriate antibiotic selection was documented in only 35.1%. Ceftriaxone was predominantly prescribed (73.0%), followed by amoxicillin (8.6%), and cefotaxime (6.2%). Furthermore, most patients received antibiotics after surgical incision, and the duration exceeded 7 days in a significant proportion of cases, contrary to standard recommendations. Of 419 patients, 117 (27.9%) received appropriate prophylaxis and 302 (72.1%) inappropriate prophylaxis. The mean antibiotic cost per patient was 2.16 USD in the appropriate group compared with 4.85 USD in the inappropriate group. Inappropriate prophylaxis accounted for 85.3% of the total antibiotic expenditure.</p><p><strong>Conclusion: </strong>Conclusions: The study highlights substantial gaps in adherence to SAP guidelines, emphasizing inappropriate antibiotic selection, timing, and excessive duration. Despite their ever-growing use in healthcare, artificial intelligence is not up to the mark yet. We need to search and critically overcome the hurdles preventing their safe and reliable use in surgical care. This narrative review aims to find and analyze the main limitations and challenges of artificial intelligence in predicting surgical outcomes. Across the reviewed literature, key limitations were identified in four domains: data-related, methodological limitations, performance and generalizability, and barriers to clinical implementation. Common issues included missing and imbalanced datasets, small sample sizes, retrospective single-center designs, high risk of bias, and inadequate external validation. Although several studies reported high predictive performance, these findings were often derived from non-representative datasets and lacked prospective validation. Additional concerns included limited interpretability, ethical and privacy risks, workflow integration difficulties, and potential amplification of healthcare disparities. Despite their potential, AI models for surgical outcome prediction remain constrained by multiple challenges. Exercise is first-line therapy, yet heterogeneity in modalities, dosing, and delivery limits practical guidance. A narrative review was conducted using PubMed, Scopus, Google Scholar, Frontiers, and ScienceDirect (2021-2026), focusing on aerobic, resistance, aquatic, and mind-body exercise studies. Data were extracted on type, intensity, frequency, duration, delivery, and outcomes, and synthesized narratively to provide clinically relevant insights. Exercise improves pain, function, and quality of life through enhanced muscle strength, joint stability, optimized loading, anti-inflammatory effects, and central pain modulation. Aerobic and resistance training enhance cardiovascular fitness, quadriceps and hip strength, and functional performance. Aquatic therapy reduces joint stress and improves adherence, while mind-body interventions support flexibility, balance, and stress reduction. Long-term, thrice-weekly protocols offer maximal benefit, and hybrid models combining supervised and home-based exercise optimize adherence and outcomes. Exercise is a cornerstone of knee osteoarthritis management. Individualized programs considering patient characteristics, modality-specific benefits, and delivery method are essential. The patients had their first ischemic stroke in the right hemisphere of the brain and unilateral spatial neglect. The average age of the patients was 68.62±10.06 years. The patients were divided into the MG and the CG in 29 individuals. In order to assess the impact of upper limb impairment and the degree of limitations experienced by patients, we used the main section of the DASH questionnaire, which consisted of 30 questions.</p><p><strong>Results: </strong>Results: The recovery of the upper limb in patients in the MG was better than in the CG. The recovery of the upper limb in patients according to the DASH questionnaire in the CG improved by 22%, and in the MG by 43%. Organizational and legal aspects of medical care delivery were analyzed for compliance with legislation and clinical protocols.</p><p><strong>Results: </strong>Results: Systemic deficiencies included the absence of electrocardiographic examination in 90,2 ± 4,2% of cases, regardless of diagnostic accuracy. Significant trends were identified as the hospital stay duration increased: the absence of oxygen therapy showed a downward trend (from 86,7 ± 8,8% to 38,5 ± 13,5%; ptrend = 0,006), while diagnostic discrepancies demonstrated an upward trend (from 6,7 ± 6,4% to 46,2 ± 13,8%; ptrend = 0,016). The median number of deficiencies doubled from 2 (1-3) in the <3 h group to 4 (3-4) in the 12-24 h group (p = 0,002). A moderate direct correlation was established between patient age and the total number of deficiencies (ρ = 0,462; p < 0,001).</p><p><strong>Conclusion: </strong>Conclusions: The identified deficiencies correlate with non-compliance with mandatory regulatory requirements. Two groups of female cadets were formed: an experimental group (EG, n = 27), whose members attended the university's CrossFit sports club, and a control group (CG, n = 29), whose members did not engage in additional sports activities, but only participated in the traditional program during their sporting and mass-participation activities (SMPAs). Research methods: theoretical methods, biomedical methods, statistical methods.</p><p><strong>Results: </strong>Results: It was found that in female cadets who practiced CrossFit, all the parameters studied were significantly (p ≤ 0.05-0.001) better at the end of the research than in female cadets who practiced using the traditional SMPAs method by the BMI - 1.07 kg/m2, the SI - 4.36 %, the VI - 4.14 ml/kg, the RI - 2.99 c. u., HRRI - 7.4 s, HSTI - 6.8 c. u., PWC - 103.1 kgm/min, BF - by 0.4 %, and SHL - by 3.16 points. Dairy sources are rich in calcium and proteins which may interfere with the absorption of thyroxine so dose adaptability might be required. This study assesses the effect of dairy intake on levothyroxine dose and clinical outcomes.</p><p><strong>Patients and methods: </strong>Materials and Methods: Retrospective cohort study over 14 months in an endocrinology center in Najaf, included 150 adult patients with primary hypothyroidism on stable oral levothyroxine therapy. We classified individuals into high, medium and low dairy consumers. A comparative approach was performed between these groups on terms of the dose, time to stabilization, dose response, clinical outcome, and economic outcome.</p><p><strong>Results: </strong>Results: After multivariable analysis, we found that participants who consumed one or more servings of dairy products per day were 6.8 times more likely to report taking their medications at least once with inadequate preparation p<0.001. These findings suggest that dietary interventions targeting dairy intake could stabilize treatment and reduce healthcare costs.</p><p><strong>Conclusion: </strong>Conclusions: Dietary intake of dairy has a major impact on both the absorption and dosing of levothyroxine in hypothyroid patients. Demographics, lifestyle factors, and clinical variables like BMI were collected. Statistical analysis was performed using chi-square testing for association and logistic regression analysis to identify independent factors that predict CLD, p-value of <0.05 was used to identify significance for all information collected.</p><p><strong>Results: </strong>Results: Significant links were found between CLD and several factors. Males were significantly more affected with CLD (78.5%, p < 0.001), making gender one of the factors that predict CLD. Similarly, age was a significant factor, with those aged 40-60 being the most affected (33%, p = 0.039). Abnormal BMI was strongly associated with CLD, found in 84.6% cases (p = 0.008). Alcohol consumption demonstrated a marked association with chronic liver disease (p < 0.001), with former and current alcohol use substantially more common among cases. After logistics regression analysis, male gender, abnormal BMI, and alcohol consumption are significant independent predictors of CLD.</p><p><strong>Conclusion: </strong>Conclusions: The study highlights gender (male), age (40-60), abnormal BMI and alcohol consumption as significant predictors of CLD in the studied population.
