In this review, studies estimate a 21% prevalence of ADHD among individuals with problematic substance use, who often exhibit earlier substance initiation, faster dependency transitions, and higher relapse risks. This review outlines that evidence suggests diagnostic and treatment practices differ by age and gender, with a marked increase in adult diagnoses and higher treatment rates-especially among women-than cohort studies predict. This may indicate overdiagnosis. While medication is the most evidence-based treatment, multimodal intervention is strongly recommended. This review focuses on non-pharmacological interventions for ADHD that have been investigated in recent reviews and meta-analyses. In an individualised case management approach, it may be relevant to include interventions that are not (yet) evidence-based if personal factors indicate benefit. {"title":"[Overlap between PTSD, ADHD, and narcolepsy].","authors":"Tania Mariager, Helene Speyer, Laura Ponsaing","doi":"10.61409/V10240757","DOIUrl":"https://doi.org/10.61409/V10240757","url":null,"abstract":"<p><p>This case report describes the diagnosis of ADHD in a 29-year-old woman suffering from psychiatric and metabolic conditions and type 2 diabetes, receiving treatment for severe PTSD. She developed excessive daytime sleepiness, which, due to diagnostic overshadowing, initially was attributed to her psychiatric illness, delaying diagnosis and treatment. This jeopardized her daily life due to incidents of sudden sleep attacks and much longer sleeping times. The diagnosis of narcolepsy type 2 was given based on polysomnography, multiple sleep latency tests, and examination of the cerebrospinal fluid. Any causal relationship has yet to be proven but observational findings suggest an increased risk of cardiovascular disease associated with treatment - especially higher doses over longer periods. Follow-up with clinical assessment, blood pressure, and resting heart rate is advised. Prevalences, key issues in differential diagnosis, and the different aetiological background of the two categories are presented in this review. In depth, clinical diagnostic evaluation of both disorder syndromes, not only based on use of questionnaires, is needed to sufficiently distinct the separate main categories. Over 20 years, the patient experienced recurrent gout attacks and developed tophaceous deposits, some of which ulcerated. Walking was severely impaired by foot deformities, pain, and chronic foot ulcers. Despite multiple consultations and radiological evidence of gout, no urate-lowering therapy was prescribed. Finally, microscopic analysis of foot ulcer debris identified urate crystals, confirming gout. Based on clinical presentation and affected brain region, AHS has three variants: frontal, callosal and posterior. Treatment of AHS is based on anecdotal reports and includes clonazepam, botulinum toxin injections, cognitive behavioural therapy, visuospatial coaching, distracting tasks and mirror box therapy. In this case report, we present two patients admitted to rehabilitation with AHS after an ischaemic stroke. In recent years, considerable knowledge has been gained in the treatment and diagnosis field. This review will address RA in general, focusing on the effect of the newest treatment option: janus kinase (JAK) inhibitors. Furthermore, using JAK inhibitors to treat RA in a Danish context will be discussed.
