Over 20 years, the patient experienced recurrent gout attacks and developed tophaceous deposits, some of which ulcerated. Walking was severely impaired by foot deformities, pain, and chronic foot ulcers. Despite multiple consultations and radiological evidence of gout, no urate-lowering therapy was prescribed. Finally, microscopic analysis of foot ulcer debris identified urate crystals, confirming gout.
