Materials and methods. The study included 15 patients (11 women and 4 men) with acute herpes zoster. The age of the patients ranged from 38 to 70 years. The patients had severe clinical symptoms (typical vesicular rash, itching of varying intensity, burning sensation, intense pain along the affected nerves and sleep disturbance). The localization of pathological foci was unilateral: in 12 patients on the skin of the trunk, in 1 patient on the buttock and in 2 patients on the skin of the thigh. All patients reported a decrease in quality of life due to the disease. The complex treatment included Valavir, one of the valacyclovir drugs, according to the recommended regimens, which was administered orally in 1000 mg tablets three times a day for 7 days. Acute herpes-associated pain and heartburn were relieved with analgesics and topical application of Lipster Mint cream, manufactured by Farmak (Ukraine), which contains acyclovir 5 % and menthol. A distinctive advantage of Lipster Mint is that it contains the highest dose of acyclovir permitted for topical products — 5 %, as well as menthol which cools and reduces itching. The effectiveness of treatment of herpes zoster was assessed by the dynamics of clinical symptoms and subjective feelings on the 2nd, 5th and 10th days of treatment. Results and discussion. As a result of the treatment, positive dynamics of all clinical manifestations was achieved. Inflammatory phenomena such as swelling and hyperemia of the skin significantly decreased, the appearance of fresh herpes rash stopped within 24 hours, body temperature normalized on the 2—3rd day, general condition improved, pain and discomfort in the affected areas significantly decreased, partial epithelialization of erosions and the appearance of crusts were observed on the 5—7th day. With the topical application of Lipster Mint, under the influence of menthol, from the first minutes of application, patients significantly reduced the sensations of burning, tingling and pain in the areas of skin lesions. Conclusions. Despite the available arsenal of specific and non-specific antiherpetic drugs, treatment of patients suffering from herpes zoster continues to be an urgent medical problem that requires further improvement and individualized approach. The prevalence of atopic dermatitis (AD) in the general population reaches 10 % among adults and 20 % among children. Despite the significant deepening of our understanding of the complex pathogenesis of allergic dermatoses and the establi­shing role of many genetic and epigenetic factors in provoking exacerbations of AD or predisposition to the development of allergic contact dermatitis (ACD), topical corticosteroids remain the cornerstone treatment for flare­ups of these disorders.We investigated the effectiveness of the mometasone furoate 0.1 % topical cream (Elocom®) for the treatment of patients with allergic dermatoses. The study included 20 patients whose participation was determined based on clinical anamnesis and examination, diagnosed with AD (n = 13) and ACD (n = 7). The duration of treatment was from 7 to 30 days. During the study, all the 20 patients (100 %) noted a significant improvement in the skin condition, 18 (90 %) — improvement during the first days of treatment. Throughout the study, none of the patients experienced side effects linked to the use of the medication. The average value of the Dermatology Life Quality Index among patients before the start of treatment was (13.0 ± 6.83) points, after treatment — (4.8 ± 3.56) points. A decrease in the Index by an average of 8.2 points indicates a significant improvement in the patients’ quality of life. The obtained results demonstrate the high effectiveness of topical mometasone furoate in the treatment of patients with allergic dermatoses. The relevance and medical and social importance of parapsoriasis is due not only to the permanent increase in the number of patients with this pathology, but also to the variety of clinical manifestations of the disease, the increase in severe aggressive course, errors in diagnosis and the lack of effective treatment methods. Objective — to study the effectiveness and safety of narrow-band phototherapy with a wavelength of 311 nm in the treatment of patients with parapsoriasis. Materials and methods. 16 patients with parapsoriasis were examined and treated, among whom large plaque was diagnosed in 4 (25 %), small plaque in 4 (25 %), lichenoid chronic in 5 (31.25 %), lichenoid and pustular acute parapsoriasis in 3 (18.75 %) patients. The age of the patients ranged from 19 to 53 years, there were 5 women (31.25 %), 11 men (68.75 %), the duration of the disease was from 6 months to 5 years. The treatment involved the appointment of patients with narrow-band phototherapy with a wavelength of 311 nm using the Dermalight 3000 phototherapy device (Dr. K. Hönle Medizintechnik GmbH, Germany). Results and discussion. The therapeutic effectiveness of the treatment was evaluated by the dynamics and degree of elimination of objective clinical manifestations and subjective signs of the disease, as well as by the long-term results of the treatment and the stability of remission. At the end of the course of treatment, clinical recovery was observed in 12 (75 %) patients, significant improvement in — 4 (25 %) patients. All patients underwent treatment without complications, side reactions and unwanted effects were not registered. Conclusions. Narrowband phototherapy with a wavelength of 311 nm is an effective and safe method of treating patients with parapsoriasis. Materials and methods. Based on the analysis of modern data from special literature and according to the results of our own preliminary studies, the expediency of further in-depth studies of the state of microcirculation and microbiocenosis in skin lesions of patients with lichen planus was determined. Results and discussion. Violations of microcirculation and changes in microbiocenosis in foci of skin lesions in patients with lichen planus are important links in the pathogenesis of this dermatosis, and can also affect the severity of its clinical course. Conclusions. So far, the etiopathogenetic and clinicopathological aspects of lichen planus have been studied in detail, which allows us to evaluate the mechanisms of development, clinical course and approaches to the treatment of this dermatosis in a new way.The state of microcirculation and changes in the microbiocenosis in foci of skin lesions in patients with lichen planus require further in-depth research. To determine how not to violate legal norms, using current regulatory documents of the Ministry of Health, hardware and conditions of their use, as well as to prove that this is relevant for the further adaptation of the system of specialized care provision which was actually destroyed as a result of military aggression, and the development of a modern system of medical care.The work of the regional skin and venereological dispensary in the zone of the deoccupied territories in the first period of Russian aggression is analyzed. For this, regular and non­regular staff was used, according to orders No. 554 of 2007 and No. 312 of 2009.The implementation of modern digital methods of research and objectification of oncoproliferative skin diseases made it possible to optimize the provision of specialized medical care to the adult population in wartime conditions. The use of radio wave removal of skin neoplasms has significantly expanded the possibilities of outpatient admission of patients. Devices for conducting dermatoscopy, which are used for the diagnosis and treatment of patients with oncoproliferative skin diseases, are not included in the list of regulatory documents of the Ministry of Health of Ukraine. It is extremely important to introduce modern accessible (especially through humanitarian channels) methods of diagnosis and treatment into the work of the dermatovenerological care service. The dermatovenereological service does not have time to wait for regulatory documents that do not allow to go beyond the legal field. The study included 104 patients aged 18 to 60 years who underwent sclerotherapy for the treatment of telangiectasias and reticular varicosities of the lower extremities. Patients were divided into subgroups depending on the appointment of phlebotonics and external means after the procedure. Results were evaluated 1, 3, and 6 months after treatment.\u0000Results and discussion. According to the self-assessment of patients, the best cosmetic result was achieved in the subgroup that used phlebotonics in combination with external therapy with heparin sodium gel, compared to the subgroups in which phlebotonics were not prescribed (p < 0.01). In the subgroup where only phlebotonics were used, the result was also better compared to the subgroups in which phlebotonics were not prescribed (p = 0.03; p < 0.01). Self-administration of sodium heparin gel did not provide a statistically significant improvement when comparing the subgroups (p = 0.26). No statistically significant difference in disease recurrence was found between the subgroups (p = 0.729).\u0000Conclusions. Ultrasound Doppler mapping with a detailed definition of the feeding sources of the reticular veins and telangiectasias allows determining the optimal stages of treatment to prevent the recurrence of the disease. The appointment of phlebotonics in the post­procedure period has a positive effect on the cosmetic result of sclerotherapy, especially in combination with external therapy with heparin sodium salt gel (p < 0.01). For treatment we used the Fotona SP Spectro new laser system which has two handpieces — for ablative (wavelength 2940 nm, erbium laser) and non­ablative (neodymium Nd:YAG laser, wavelength 1064 nm) treatment methods, Fotona S11 scanner (neodymium laser, wavelength 1064 nm) and Cry­ac cryospray.Under observation were 45 patients with widespread recurrent papillomatosis, with confirmed papillomavirus infection (ICD10 code D23.5; ICD11 XH17Q9). The lesion area was the front surface of the chest, under the mammary glands, the area of the treatment area — 10 × 10 cm.According to the inclusion and exclusion criteria from the retrospective analysis of the disease histories, three groups of patients were formed: group 1 — treatment according to the accepted method of laser therapy with an erbium (ER:YAG) laser, group 2— combined ablative and non­ablative laser therapy (with erbium and neodymium lasers) and group 3 — cryodestruction of papillomas.\u0000Results and discussion. The best result and the speed of the obtained positive results of the treatment and a significantly less frequent occurrence of new elements during the observation within a year after the start of therapy were shown by group 2 — where combined laser therapy was applied. Significant advantages of laser therapy over cryodestruction were determined according to the results of this study and according to the current literature.\u0000Conclusions. The combined method of laser therapy, especially the mechanisms of the pathogenesis of the obtained therapeutic effect, require careful study. During 2020—2022, 94 patients with a diagnosis of «pyogenic granuloma» (PG) underwent outpatient treatment under our supervision at Dr. Bogomolets Institute of Dermatology and Cosmetology (Kyiv). The largest number of patients with PG examined by us was aged from 1 to 12 years (72 %), which correlates with the data of scientific research by other authors on the relevant problem. Anamnesis was collected in all examined patients (according to the patients and their parents) regarding the possible causes of neoplasms and the nature of their development. All the patients underwent a complex diagnostic instrumental examination of PG (dermoscopy, ultrasound, histopathology). The choice of treatment tactics was carried out individually in each specific clinical case, taking into account the results of the examination and the stage of tumor proliferation (radio wave removal or treatment with a 595­nm pulsed dye laser was carried out).\u0000Results and discussion. A total of 94 patients with PG were examined and treated on an outpatient basis. By age, children from 1 to 8 years old (48 %) and from 8 to 12 years old (24 %) prevailed among the examined. There were 6 % of teenagers and young adults (12—18 years old), 22 % over 18 years old. In 45 patients, neoplasms were localized on the skin of the face, in 7 — on the neck, in 9 — on the upper limbs, in 6 — on the lower limbs, in 27 — on the skin of the trunk. In 87 clinical cases, radio wave removal of neoplasms with their subsequent pathohistological diagnosis was carried out taking into account the results of a comprehensive examination of patients with PG, including the data of ultrasound of the skin in the areas affected by PG, which makes it possible to determine the depth of tumor invasion together with the exophytic part. In 6 clinical cases, PG removal therapy was selected and performed using a pulsed vascular dye laser with a wavelength of 595 nm. In one clinical case, taking into account the sufficiently large size of the tumor (more than 2 cm) and the significant vascularization of the neoplasm revealed by ultrasound, a diagnostic excisional biopsy was performed within healthy tissues. Recurrences of PG after treatment were observed after 2 months in 2 of 94 patients, in particular, after pulsed vascular laser therapy. Subsequently, in these two patients, PG was removed using the radio wave method. No recurrences of the disease were registered.\u0000Conclusions. Reznichenko","doi":"10.30978/ujdvk2023-2-18","DOIUrl":"https://doi.org/10.30978/ujdvk2023-2-18","url":null,"abstract":"In practical dermatology, the problem of seborrheic dermatitis is actively considered as the most common pathological condition among the population\u0000Objective — to study the clinical effectiveness and safety of the use of the Natubiotin drug in the treatment of patients with seborrheic dermatitis.\u0000Materials and methods. 150 patients with seborrheic dermatitis were included in the study. The control group included 50 healthy people of the same age. The severity of seborrheic dermatitis was determined using a scoring scale that included evaluation of the intensity of erythema, swelling, wetting, excoriations, peeling, and fattiness of the skin. The level of anxiety was estimated according to the Spielberger—Khanin self-esteem scale, the level of neuroticism — according to the diagnostic method of L.I. Wasserman. A survey was also conducted using the Beck scale for self-assessment of the severity of depression. Depending on the method of treatment, patients with seborrheic dermatitis were divided into two groups: I (experimental) — 100 patients who received Natubiotin drug in a dose of 5 mg once a day for 3 months; II (control) — 50 patients who underwent standard therapy.\u0000Results and discussion. Additional inclusion of Natubiotin drug in the treatment regimen of seborrheic dermatitis ensured the recovery of 100 % patients with localization of the pathological process on the scalp and face. Only 5.3 % of patients with localization of seborrheic dermatitis on the trunk had slight scaling and fattiness of the skin. After the treatment with the inclusion of Natubiotin drug, the indicators of the point assessment of the severity of seborrheic dermatitis in patients of group I were significantly better than in group II that received standard therapy. The assessment of subjective symptoms such as itching and sleep disturbances were also significantly better after using Natubiotin. Vitamin therapy with biotin contributed to a significant reduction of state anxiety, depression and neuroticism in patients. The obtained data indicate the high effectiveness of Natubiotin for the treatment of seborrheic dermatitis.\u0000Conclusions. The course of seborrheic dermatitis is accompanied by high personal and state anxiety, increased neuroticism and tendency to depression. Therapy of seborrheic dermatitis with the use of Natubiotin is justified and appropriate, as it provides a quick and long­lasting clinical effect, normalization of the skin condition and improvement of the psychogenic state of patients.
