Material and methods: To find appropriate publications there were used keywords: vertigo, balance disorders, questionnaires assessing vertigo, diagnostics of vestibular disorders. Due to access to Scopus, Web of Science, ScienceDirect and PubMed databases, Polish and foreign publications were analyzed. They describe the process of creating new research tools and validation of the questionnaires assessing vertigo and balance disorders carried out by various medical centers in the world. Also materials regarding otoneurologic diagnostics were used. Conclusions: In everyday clinical practice, the role of questionnaires which are reliably filled by the patient is a valuable source of information. They can help to obtain information about the intensity of vertigo and balance disorders, their impact on the daily functioning of patients and the effectiveness of treatment. The article presents a complex symptomatology of this syndrome based on the latest literature reports and current diagnostic criteria. Due to the variety of symptoms that constitutes the clinical picture of Fahr syndrome, differential diagnosis is a challenge for clinicians and requires interdisciplinary collaboration with representatives of many medical specialties. The coexistence of hearing disorders in patients with Fahr syndrome may disturb the diagnostic and therapeutic process and require the involvement of an otolaryngologist or audiologist. The understanding of the term „phonemic hearing” adopted in this work refers to the phonological system in its segmental aspect. A properly educated phonemic hearing allows the child to properly decode the meaning of words, to differentiate syllables and voices, which at the early childhood education stage is essential for the development of speech and communication, reading and writing. The results presented in the article refer to the group of 7-year-olds surveyed and constitute a contribution to planned phonemic hearing analyzes in children from the first grades in the context of reading and writing.Aim: The aim of the study was to examine the phonemic hearing of students from the first class of primary school starting to learn to read and write, and to analyze the educational difficulties of children diagnosed with distorted speech features.Material and method: The research group consisted of 20 students at the age of 7 attending the 1st class with an integrational profile. In this class, two students with multiple dyslalia and three with motor aphasia were diagnosed. The control group consisted of 20 students from a parallel class without an integration profile. The research was carried out with a standardized tool by J. Gruba: Test for phonemic hearing (Gliwice, 2012).Results: The diagnosis was made at the end of the first half-year of school. Difficulties in the auditory differentiation of sounds from phonological oppositions, differing in the place of articulation, two distinctive features, manner of articulation and voicing, were noted in 8 students. Phonemic hearing disorders constituted a part of problems in the acquisition of the language system in 3 children with motor aphasia, 2 students with multiple dyslalia, but also 3 first-grade students who were not diagnosed with speech impediments or speech disorders. In the control group, the two children did not differentiate correctly the phonemes differing in the two distinctive features and the way of articulation.Conclusions: The conducted research is an element of assessment of school readiness of children starting education. Phonemic hearing disorders of the studied students are one of the causes of persisting speech defects, difficulties in reading by analytical method and writing from hearing. The results indicate the need to continue screening tests of phonemic hearing in first grades. Zielińska, Marcin Migaczewski, Rafał Nieckarz, Paweł Stręk","doi":"10.17431/1003447","DOIUrl":"https://doi.org/10.17431/1003447","url":null,"abstract":"Aim: The aim of this study was to prove the role of high BMI as the risk factor of laryngeal manifestation of esophageal reflux disease with acknowledgement of innovative and non-invasive forms of diagnostics of reflux disease on example of laryngography.Material and method: From January to March 2017 thirty two patients with diagnosed GERD and extralaryngeal symptoms were chosen in the Second Department of General Surgery UJ CM. Patients were referred to Otolaryngology Department and Clinic UJ CM to extend the diagnostics towards laryngopharyngeal reflux LPR. On basis of calculated BMI and laryngological examinations results 20 patients with LRP symptoms and BMI >24,9 were chosen. The patients were then referred to the Pedagogical University of Cracow for laryngographic examination and statistical analysis. After the laryngographic examination patients were qualified for conventional pharmacotherapy or surgery. After the 3rd part of study each patient was ordered to abide to antireflux diet and increase previous physical activity. After 6 weeks since the end of the treatment control examinations were made at Otolaryngology Department and Clinic UJ CM (laryngological and videostroboscopic examinations) and Pedagogical University of Cracow (laryngographic examination).Results: Laryngographic examination revealed increased SDFx and SDQx. The increase in these quantities shows the larynx dysfunction and higher exacerbation of GERD/LPR. In control examinations 6 weeks after the end of study the improvement of function of larynx was observed by the decrease of SDFx and SDQx. The videolaryngostroboscopic examination showed the decrease of laryngeal mucositis symptoms in comparison with laryngeal picture before the treatment.Conclusions: The study showed that high BMI is a risk factor of laryngeal manifestation of esophageal reflux disease LPR. As audiometric tests: pure tone audiometry and behavioral observation audiometry, allow only to assess the level of sound detection, it is necessary to use questionnaires that help to assess the child’s auditory development and the benefits from the devices used. The application of questionnaires allows to monitor the progress of the auditory development of children who use hearing aids and to compare their perception with normally hearing children. Aim: The aim of this study is: (1) the assessment – based on the results of behavioral observation audiometry and the LittleEARS questionnaire – auditory development of children up to two years of age with moderate hearing loss, after the use of hearing aids and (2) the assessment of the effectiveness of auditory prosthesis used.Material and method: The research group consisted of 30 patients from the Institute of Physiology and Pathology of Hearing, children aged 7–23 months on the day of the study. To assess the threshold of hearing in children, auditory brainstem response and behavioral audiometry were performed. Twenty children were provided with classical behind-the-ear hearing aids, in 10 children bone conduction hearing aids have been applied. The development of children auditory performance after the application of hearing aids was assessed with the LittlEARS questionnaire completed by parents and a re-examination of the behavioral observation audiometry about 6 months after the hearing aids were applied. The results of audiometric tests and the LittlEARS questionnaire were used to assess the auditory reactions and skills, determine the children’s auditory ages, delay of the auditory development and the effectiveness of the hearing aids used.Results: The results of audiometric tests obtained from behavioral observation audiometry (BOA) in a wide range of frequencies (250–4000 Hz) showed improvement in hearing in all children. The analysis of the LittlEARS questionnaire answers indicates progress in the areas of auditory skills of children who use hearing aids. The delay of the auditory development in hearing impaired children using hearing aids against normally hearing children is on average 4 months. 70% of the research group achieved results within the normal range (the average achieved by children with normal hearing).Conclusions: The use of hearing aids in children with moderate hearing loss allows proper development of the auditory perception. The use of the questionnaire supports the audiological evaluation and allows monitoring of auditory development. When assessing the child’s hearing reactions and development after using hearing aids, there is a significant improvement in the development of listening skills. Skarżyńśki","doi":"10.17431/1003642","DOIUrl":"https://doi.org/10.17431/1003642","url":null,"abstract":"Introduction: The subjective assessment of benefits and satisfaction with a cochlear implant (CI) is an essential index of the effectiveness of that medical intervention in different groups of patients with hearing disorders. Regardless of pre- or postlingual onset of deafness, the adult users assess their CI satisfaction as high or very high. There are few published studies of the psychological determinants of the subjective assessment of CI benefits and satisfaction, such as personality traits or mental health status.Aim: The study aims to assess the self-acceptance of deaf persons in the group of prelingually deaf CI users, and the assessment of CI benefits and satisfaction. The question was if there is a relationship between the self-acceptance of deaf persons and perception of CI benefits and satisfaction.Material and methods: The study included prelingually deaf adults aged from 18 to 45 years, using one (81%) or two cochlear implants. The group size was n = 84. Age at the time of the first implantation varied within the range of 2–42 years. Age at the second implantation was on average 22 years (SD = 6.67). The mean time of cochlear implant use for the first implant was 11 years, for the second 4 years. Studied persons were highly competent in Polish phonic language. The study had been conducted by mail; the rate of returned responses was 32.2%. Self-acceptance of deaf persons and CI satisfaction have been measured using the VAS-type scales, CI benefit has been measured using the Nijmegen Cochlear Implant Questionnaire (NCIQ) that allows for describing the subjective benefits from the CI in three areas: basic and advanced auditory perception, speech, and psychosocial functioning as a (deaf) person with CI, which encompasses self-esteem, perception of limitations in activities and social interactions.Results: Self-acceptance as a deaf person was on average 74.5% (SD = 25). The mean CI satisfaction was 80%. The prelingually deaf persons experience the largest CI benefits in the area of Activity Limitations (AL – 74.5%) and Social Interactions (SI – 73.6%), the smallest in the field of Self-Esteem (SE – 54.4%). Self-acceptance of a deaf person is related to psychosocial functioning with a CI, which means that higher level of self-acceptance of a deaf person is linked to perceived higher benefits in these areas. Similarly, higher level of self-acceptance of a deaf person is related to a higher CI satisfaction.Conclusions: Self-acceptance of a deaf person is one of the important factors deciding about the CI benefits and satisfaction in the studied persons’ subjective assessment. This relationship exists only in the sphere of psychosocial functioning of the prelingually deaf CI users. The prelingually deaf CI users should be offered a psychological intervention to improve their acceptance of self as a deaf person and own deafness. A retrospective study was conducted through the analysis of preventive check-ups of employees of the Metallurgical Factory. The age, working time, exposure to noise and exposure to other factors that may affect the hearing organ were analyzed. Average hearing loss was compared after 3 and 10 years of work.Results: The average age of the surveyed employees was 46.1 years and ranged from 35 to 60 years. Everyone worked in a 3-shift system, 8 hours a day, in noise. An additional factor that could affect the hearing organs were vibrations, where their average level was 0.96 NDN (maximum admissible intensity). The noise range to which workers were exposed ranged from 80 to 88 dB in the workplace, and its average level was 87 dB. The average hearing loss, calculated as the arithmetic average for 0.5 Hz, 1 kHz, 2 kHz and 4 kHz, in these employees, after three years of work was 11.7 dB for the right ear, and 11.9 dB for the left ear. However, after 10 years of work, the average hearing loss was 16 dB for the right ear and 17,2 dB for the left ear. The largest hearing loss occurred at frequencies 2 and 4 kHz after 10 years of work. It was 14 dB for 2 kHz and 24 dB for 4 kHz in the right ear. And in the left ear for 2 kHz – 15 dB and 28 dB for 4 kHz.Conclusions: The analysis of the presented results shows the harmful effect of noise on the condition of the hearing organ. There was an increase in the hearing threshold by 4.3 dB for the right ear and by 5.27 dB for the left ear after 10 years of work. However, a thorough examination of the electrically stimulated auditory pathway, and then appropriate - optimal for a given patient - setting of stimulation parameters is a prerequisite for the proper functioning of the implant. To this end, both psychoacoustic and objective methods are used. Until recently, with the help of objective methods in users of cochlear implants, it was possible to carry out measurements of the functioning of the auditory nerve endings and subsequent, higher levels of the auditory pathway. In children with various types of hearing loss, delayed or non-harmonious psychomotor development is often observed. The Psychomotor Development Evaluation Cards (KORP) allow to study many developmental spheres, and thus allow also the rapid detection of developmental abnormalities. Using KORP, one can examine both the motor, cognitive (including linguistic), as well as socio-emotional, and behavioral functions, the level of knowledge and learning skills.Aim: The aim of the study was to present by means of KORP three different profiles of psychomotor development of children with hearing impairments: a prelingually deaf child, a child with unilateral deafness and a child with central auditory processing disorders.Material and Method: The study presents the results of three children with hearing disorders obtained by the evaluation of the level of psychomotor development using KORP.Results: In a girl M.L., aged 4, using CI, the test performed 2 years after the device activation indicates a low level of child’s functioning in terms of language skills and learning (hearing-related tests). A girl R.P., aged 8, with central auditory processing disorders, shows a low level of functioning in 6 out of 7 spheres of functioning. The boy P.A., aged 4, with unilateral deafness, examined after 2 years of using the CI, shows normal psychomotor functioning in all studied areas in relation to his biological age.Conclusions: KORP can be used as a tool enabling initial diagnosis of psychomotor development of children with hearing and speech disorders. The results obtained in this study may facilitate the planning of multi-specialist assistance for children with impaired psychomotor development. The results may indicate which areas of the child’s functioning should be subjected to a deeper specialist diagnosis, and after confirmation, which treatment and rehabilitation should be applied.
