[Validation of a symptom scale for COVID-19 patients in ambulatory care]. [门诊COVID-19患者症状量表的验证]。

Scale development should be accompanied by an assessment of its reliablility and validity.</p><p><strong>Objective: </strong>To develop and measure the psychometric characteristics of a COVID-19 symptom scale to be answered by either healthcare personnel or adult patients in ambulatory care.</p><p><strong>Material and methods: </strong>The scale was developed by an expert panel using the Delphi method. We evaluated inter-rater reliability, where we defined a good correlation if Spearman's Rho was ≥ 0.8; test-retest, where we defined a good correlation if Spearman's Rho was ≥ 0.7; factor analysis using principal component methodology; and discriminant validity using Mann-Whitney's U test. A p < 0.05 was considered statistically significant.</p><p><strong>Results: </strong>We obtained an 8 symptom scale, each symptom is scored from 0-4, with a total minimum score of 0 and a maximum of 32 points. It appears as a subjective phenomenon and its main symptoms are the loss of enthusiasm towards work, a feeling of professional failure, feelings of guilt, emotional exhaustion and indifference to patients' problems.</p><p><strong>Objective: </strong>To evaluate the prevalence of BS in health personnel who care for cancer patients in a tertiary hospital.</p><p><strong>Material and methods: </strong>Descriptive cross-sectional study. The sample consisted of 41 health professionals dedicated to providing direct care to cancer patients, which were selected through an intentional non-probabilistic sampling. The Questionnaire for the Evaluation of the Burnout Syndrome was applied.</p><p><strong>Results: </strong>In the sample studied, BS presented a prevalence of 51.21% at the medium level, 9.75% at the high level and 2.43% at the critical level. Significant differences were found between groups by service and work seniority.</p><p><strong>Conclusions: </strong>A high prevalence of symptoms of BS was found in the study participants, derived mainly from the excessive workload, the type of care provided, as well as experiences related to contact with people living with cancer, the hospital environment, and the type of interpersonal relationships that emerge there. {"title":"[Results of abdominal wall closure using the 4:1 suture/wound lenght technique].","authors":"Carlos Alberto Córdova-Velázquez,&nbsp;Enrique Jesús Rodríguez-Espino,&nbsp;Juan Manuel Martín-Bufajer,&nbsp;Erick Servín-Torres,&nbsp;Natalia Guadalupe Lerma-López","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Background: </strong>Laparotomy is a daily procedure for the general surgeon and its main complication is the formation of hernias.</p><p><strong>Objective: </strong>To determine if the suture length to wound length ratio 4:1 for wall closure decreases the incidence of hernia.</p><p><strong>Material and methods: </strong>Data from patients (n = 86) in whom abdominal wall closure was performed from August 2017 to January 2018 were prospectively reviewed. Patients who could not undergo adequate follow-up, those managed with open abdomen, or those with use of non-absorbable suture materials were excluded. 2 groups were formed: in one, the suture length to wound length ratio 4:1 technique was used as wall closure, and in the other it was used conventional suture; the length of the wound-suture length was measured, and the follow-up was post-surgical. For statistical analysis it was used descriptive statistics and inferential statistics (chi-squared and Mann-Withney's U).</p><p><strong>Results: </strong>The 2 groups had similar characteristics in all the inclusion criteria. There was a statistically significant difference in dehiscence and hernias. For both complications, the 4:1 suture is a protective factor. Despite its importance we do not have accessible markers in intensive care. In other entities (post-surgical and COVID-19) the neutrophil/lymphocyte and platelet (N/LP) ratio has been associated with acute kidney injury; however, this relationship has not been studied in a pathology with a severe inflammatory response such as sepsis.</p><p><strong>Objective: </strong>To demonstrate the association between N/LP with AKI secondary to sepsis in intensive care.</p><p><strong>Material and methods: </strong>Ambispective cohort study in patients over 18 years who were admitted to intensive care with a diagnosis of sepsis. The N/LP ratio was calculated from admission up to the seventh day and up to the diagnosis of AKI and outcome. Initial experience at IMSS].","authors":"Jorge Arturo Santos-Franco,&nbsp;Carlos Antonio Cruz-Argüelles,&nbsp;Adrián Alejandro Abrego-Salinas,&nbsp;Martín Roberto Casas-Martínez,&nbsp;Enrique DeFont-Reaulx Rojas","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Background: </strong>pCONus2 device has been used in some countries as coadyuvant in the treatment of wide-neck bifurcation aneurysms with coils.</p><p><strong>Objective: </strong>To present the first series of brain aneurysms treated with pCONus2 in the Mexican Institute for Social Security (IMSS).</p><p><strong>Material and methods: </strong>We retrospectively present the first 13 aneurysms treated from October 2019 to February 2022 with pCONus2 device at a third level hospital.</p><p><strong>Results: </strong>6 aneurysms located at anterior communicating artery, 3 at middle cerebral artery bifurcation, 2 at internal carotid artery bifurcatión, and 2 at the tip of basilar artery were treated. Device deployment was performed without complications and it was possible to embolize aneurysms with coils in 12 patients (92%), while on an internal carotid bifurcation aneurysm (8%) there was an incident of a pCONus2 petal migration toward vascular lumen caused by coils mesh pressure, situation that was solved by placing an nitinol self-expandable microstent. In 7 cases (54%) we performed coiling technique after microcatheter passage through pCONus2, while in 6 cases (46%) we used the jailing technique without complications.</p><p><strong>Conclusions: </strong>pCONus2 is a useful device for wide-neck bifurcation aneurysms embolization. In Mexico our experience is yet limited; however, the first cases have been successful. Furthermore, we showed the first cases treated using jailing technique. Its central element is the sense of coherence (SOC). The relationship of SOC with different phases of diabetes has been demonstrated, but not in diabetic debutants.</p><p><strong>Objective: </strong>To evaluate the magnitude of the association of SOC between type 2 diabetes mellitus (T2DM) debutants and absentees in people detected in the PREVENIMSS module.</p><p><strong>Material and methods: </strong>case-control design. Cases were T2DM debutants with fasting plasma glucose ≥ 126 mg/dL; controls, people with plasma glucose < 100 mg/dL. Sample size was estimated for independent groups (101 cases and 202 controls) in which the SOC-29 questionnaire was administered; socio-demographic data was recorded and their file was reviewed. Virtual conferences were held for medical personnel from all over the country with the possibility of interacting with various specialists. In 2020, 215 sessions were held and 158 in 2021. That year educational content was expanded and included topics for other health categories, such as nursing and social work. In October 2021, it was established the Health Educational System for Well-being (SIESABI), with the aim of promoting continuous and permanent education for health workers. It currently offers face-to-face and virtual courses, permanent seminars, and telementoring, with the possibility of providing academic follow-up to its subscribers and linking priority courses that are on other platforms. There are several treatments, out of which panretinal photocoagulation (PRP) stands out.</p><p><strong>Objective: </strong>To compare the level of pain in patients undergoing PRP with different impulse.</p><p><strong>Material and methods: </strong>Comparative, cross-sectional study that compared the level of pain in patients undergoing PRP with a 50-millisecond pulse (group A) versus conventional 200 milliseconds pulse (group B). Mann-Whitney U test was used.</p><p><strong>Results: </strong>There were 26 patients, 12 (46.16%) female and 14 (53.84%) males. The median age was 58.73 ± 7.31 (40-75) years. 40 eyes were studied, 18 (45%) right and 22 (55%) left. The mean level of glycated hemoglobin was 8.15 ± 1.08 (6.5-12) %. The mean laser power was 297 ± 53.61 (200-380) and 214.5 ± 41.73 (170-320) milliwatts; the mean fluence was 18.85 ± 5.28 (12-28) J/cm2 and 65.9 ± 12.87 (52-98) J/cm2; the mean level of pain was 3.1 ± 1.33 (1-5) and 7.5 ± 1.23 (6-10) points for group A and B, respectively, and there was statistically significant difference (p ˂ 0.001) in the level of pain. {"title":"[Instrument validation that measures disruptive education on clinical training].","authors":"Angel Omar Saenz-Acuña,&nbsp;Haydeé Parra-Acosta","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Background: </strong>During the contingency derived from the COVID-19 pandemic, there were no instruments assessing the aspects of clinical training, which is why it is necessary to have a questionnaire that let us know the opinion of medical students about the disruptive education.</p><p><strong>Objective: </strong>To validate a questionnaire designed to know the opinion of medical students about disruptive education in their clinical training.</p><p><strong>Material and methods: </strong>Validation cross-sectional study which was developed in three phases: 1) Elaboration of the questionnaire aimed at undergraduate medical students who include clinical science subjects in their curricular program; 2) validation of content by Aiken's V test with 7 expert judges and reliability estimation with Cronbach's alpha coefficient in a pre-sample test with 48 students; 3) analysis of the information through descriptive statistics, where the following results were observed: Aiken's V index of V = 0.816; Cronbach's alpha coefficient = 0.966. We included patients hospitalized from August 2020 to February 2021, with severe COVID-19 pneumonia, not vaccinated against SARS-CoV-2. Data was collected from admission to discharge. We used descriptive and analytical statistics according to the data distribution. ROC curves were used to determine the cut-off points with the highest predictive performance for HG and mortality, with the IBM SPSS program, version 25.</p><p><strong>Results: </strong>We included 103 patients, 32% women, 68% men, age 57 ± 13 years; 58% were admitted with HG (191, IQR 152-300 mg/dL) and 42% with normoglycemia (NG < 126 mg/dL). Mortality was higher in HG at admission 34 (56.7%) than in NG 13 (30.2%) (p = 0.008). HG was associated with diabetes mellitus 2 and neutrophilia (p < 0.05). The risk of death increases 1.558 times (95% CI 1.118-2.172) if HG is at admission and 1.43 times (95% CI 1.14-1.79) during hospitalization.
