[Correlation between the shock index and the anaerobic index]. [冲击指数与厌氧指数的相关性]。

The shock index (SI) and the anaerobic index or the relationship between the veno-arterial gradient of carbon dioxide and the difference between the arterial and venous content of O₂ [∆P(v-a)CO2/ΔC(a-v)O2] are markers of systemic hypoperfusion.</p><p><strong>Objective: </strong>To determine if there is a correlation between the SI and the anaerobic index in patients with circulatory shock.</p><p><strong>Material and methods: </strong>Observational and prospective study in patients with circulatory shock. The SI and the anaerobic index were calculated at admission to the intensive care unit (ICU) and during their stay. Pearson's correlation coefficient was calculated and the association of SI with mortality was explored with bivariate logistic regression.</p><p><strong>Results: </strong>59 patients aged 55.5 (± 16.5) years, 54.3% men, were analyzed. The most frequent type of shock was hypovolemic (40.7%). They had SOFA score: 8.4 (± 3.2) and APACHE II: 18.5 (± 6). The SI was: 0.93 (± 0.32) and the anaerobic index: 2.3 (± 1.3). Global correlation was r = 0.15; at admission r = 0.29; after 6 hours: r = 0.19; after 24 hours: r = 0.18; after 48 hours: r = 0.44, and after 72 hours: r = 0.66. The SI > 1 at ICU admission had an OR 3.8 (95% CI: 1.31-11.02), p = 0.01.</p><p><strong>Conclusions: </strong>The SI and the anaerobic index have a weak positive correlation during the first 48 hours of circulatory shock. In recent years, odontology has intervened against obesity through the implementation of intraoral devices that contribute with weight control treatments.</p><p><strong>Objective: </strong>To study the use of a dental occlusal disruptor as a strategy to moderate caloric intake.</p><p><strong>Material and methods: </strong>Pilot study which included 2 patients. It was used a dental occlusal disruptor that has an impact on the smaller amount of food at each bite. Patients attended five appointments in which a stomatological evaluation was carried out and anthropometric measurements were taken. All adverse effects were reported in each patient's clinical history.</p><p><strong>Results: </strong>Patients presented weight and body fat loss, increased muscle mass and decreased both body mass index and waist and hip measurements.</p><p><strong>Conclusions: </strong>The use of the disruptor does not alter the stomatological assessment, it promotes masticatory regulation and the decrease in body weight. Among the mechanisms that predispose to obesity, the interaction between food intake and genetic components has been little explored. Recently we evidenced a significant association between the copy number (CN) of AMY1A and AMY2A genes, the enzymatic activity of salivary and pancreatic amylase, and the frequency of childhood obesity in Mexico, a particular population due to the high consumption of starch in the diet and the high prevalence of obesity in children and adults. This review aims to find a better understanding of the role of amylase in obesity through a description of the evolution of the CN of its genes, the association of its enzymatic activity with obesity, and the effect of its interaction with starch intake on Mexican children. Medical residents of any grade and specialty were included, who signed informed consent. Those with a diagnosis of cognitive impairment were excluded, and who did not complete the tests were eliminated. AMAS-A test was applied to assess anxiety and NEUROPSI: Attention and memory test for cognitive characteristics. Mann-Whitney's U and Spearman's rho were used, p≤0.05 was considered significant.</p><p><strong>Results: </strong>155 residents were evaluated, 55.5% men, mean age 32.4 years. Internal Medicine was the predominant specialty (25.2%). AMAS-A identified 94.19% residents with anxiety. NEUROPSI reported Attention and memory domain (38.7%) in normal classification, Memory (34.2%) in high normal, and Attention and executive functions (32.3%) in severe alteration as predominant assessments. Only Memory showed a significant difference between residents with and without anxiety (p=0.015). Attention and executive functions-Physiological anxiety (r=-0.21, p=0.009) and Attention and memory-Social concern (r=-0.268, p=0.001) correlations were significant.</p><p><strong>Conclusions: </strong>The percentage of anxiety and cognitive alterations in residents physicians is high. The origin of autoimmune disorders can be multifactorial and, there are no specific therapy for these diseases. Primary immunodeficiencies are a group of immune disorders that affect different components of the innate and adaptive responses. Interestingly, patients with primary immunodeficiencies have an increased susceptibility to infectious diseases and non-infectious complications including allergies, malignancies, and autoimmune diseases. The molecular mechanism for development of autoimmunity in immunodeficiencies is unclear. The study of the complex immune regulatory and signaling mechanisms is revealing the relationships between primary immunodeficiency syndromes and autoimmune diseases. Newly, it has been demonstrated that a deficient maturation of immune cells; the deficiency of proteins important for T and B lymphocyte function and impaired signally pathways that include key molecules in regulation and activation of immune cells are associated with the development of autoimmunity in patients with primary immunodeficiencies. The data in the cost matrix was executed with the Office Excel 2010 program; the most prescribed drug was identified and compared with monotherapy and bitherapy.</p><p><strong>Results: </strong>The annual direct medical costs of the total population were drug cost $118,561.70MN, hospitalization cost $243,756.00MN, consultation cost $327,414.00MN and clinical trial cost $2416.79MN, obtaining an annual total of $692,148.58MN. metformin was the most indicated in monotherapy (88.4%) and as standard therapy it has higher cost-effectiveness compared to glibenclamide. It is classified by the WHO as a malignant epithelial tumor of the pancreas, its prevalence is low, it occurs in only 1 to 2% of all malignant tumors of the pancreas, it usually affects mainly young women, its origin is still unclear, it is generally It presents as a solitary, encapsulated lesion, without invasion of peripancreatic tissues with rare cases of metastasis, which is why it is considered a low-grade malignant tumor by the WHO. The objective of this article is to present three clinical cases and to evaluate the epidemiology, clinical manifestations, morphology and immunohistochemical expression of the tumor in a review of the bibliography, as well as to compare it with the cases already reported on the subject.</p><p><strong>Clinical case: </strong>Three cases of Frantz tumor were diagnosed by the pathology department of a tertiary hospital are presented, which correspond to two women aged 17 and 34, as well as a 52-year-old man whose presentation by age and sex is rare.</p><p><strong>Conclusions: </strong>After the bibliographical review and the analysis of the cases presented, we verified the difficulty to make a correct diagnosis, since its presence is rare in the daily practice of the surgical pathologist. If they had any condition that met the suspected definition of COVID-19, they underwent polymerase chain reaction testing for SARS-CoV-2; they were treated on an outpatient or hospital basis without interfering with their treatment. They followed their evolution every 24 hours until they became asymptomatic or died.</p><p><strong>Results: </strong>Thirteen patients (6.6%), nine diffuse cutaneous (dcSSc), and four limited cutaneous (lcSSc) developed COVID-19 during nine months of follow-up. The immunosuppressants used at the time of the disease were: mycophenolate mofetil, methotrexate, and prednisone, in low doses. Seven patients had interstitial lung disease (ILD). The main symptoms were chest pain, cough, dyspnea, dysgeusia, and anosmia, 1 with mild symptoms without pneumonia, 11 with mild pneumonia, and one with severe pneumonia that required hospital management. Consecutive subjects born and residents of the cities of La Paz and El Alto (Bolivia), both genders, without cardiopulmonary disease or physical limitation were studied. Their altitude, hematological, demographic and simple spirometry variables were informed. The differences were calculated with the t-test for independent or dependent groups according to the type of comparison. A p < 0.05 was considered significant.</p><p><strong>Results: </strong>110 subjects were studied, at an altitude of 3673 ± 250 meters above sea level, age 24 ± 5 years old, 67 (60.90%) were women. The hemoglobin was 15.20 ± 2.46 g/dL. Prior to the test in 37 (33.63%) subjects the partial oxygen saturation was < 92% (90.92 ± 0.92%), its correlation whit meters walked, r = - 0.244, p < 0.010. Total meters walked: 581 ± 35 (627.3 ± 52.88 sea level); with the reference equations from Enright PL: 542 ± 75 and Osses AR: 459 ± 104, both obtained at < 1000 meters above sea level. The objective of this work is to provide evidence and analyze the main gaps in mental health such as: the invisibility of some people with disabilities for the design of policies, legislation, or public programs; the predominance of the medical model, in which the substitution of decision-making in informed consent prevails, which violates the rights of legal personality, equality, freedom, security and respect for personal integrity, among others.
