Although HIV infection can also cause neuritis and myelitis, the entity of HIV related to NMO has recently been elucidated, however, little is known about the context of this disease. Objective: To describe the clinical characteristics, imaging, treatment, and functional prognosis in an HIV-positive patient who developed an episode of longitudinally extensive transverse myelitis (LETM) with positive anti-AQP4 antibodies.</p><p><strong>Clinic case: </strong>36-year-old man with a history of HIV diagnosed in 2017, on antiretroviral treatment. The most frequent symptom is acute abdominal pain, although can be accompanied by seizures, neuro-psychiatric alterations or symmetrical motor neuropathies, which in some patients can progress to respiratory musculature paralysis.</p><p><strong>Objective: </strong>To describe an atypical presentation of acute porphyria to be considered as differential diagnoses in abdominal pain.</p><p><strong>Clinical case: </strong>We present a case of a patient with AIP, presenting acute abdomen, seizures, later developed neuropsychiatric compromise and symmetrical motor neuropathy, and was admitted to mechanical ventilation. Due to the severity of the neurological involvement, he received hemin arginate, presenting with transient hypertransaminemia, an adverse event not previously reported. The evolution was favorable, with mechanical ventilation and hospital discharge withdrawn.</p><p><strong>Conclusions: </strong>The diagnosis of AIP should be considered in cases of acute abdominal pain associated with neurological and/or psychiatric symptoms, particularly young women. Most of the times associated to crossed reactivity and molecular mimicry.</p><p><strong>Clinical case: </strong>: We present a case report of a previously healthy young woman with an acute and multifocal clinical course, preceded by a viral respiratory tract infection, followed by a rapid disease progression and a delay in the diagnosis. The clinical, neuroimaging and cerebrospinal fluid featured suggested the diagnosis of AHLE, despite efforts and management with immunosuppression and intensive care, the response to treatment was poor leaving the patient with a severe neurological impairment.</p><p><strong>Conclusion: </strong>There is little evidence regarding the clinical course and treatment of this disease, and more studies are needed to better characterize it and to provide further information about its prognosis and management. {"title":"[Acute cholangitis secondary to periampullary duodenal diverticulum. Case report].","authors":"Verónica Elizabeth Masabanda-Celorio, Erik Daniel Alvares-Sores, Ulises Lara-Orosco","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Background: </strong>Periampullary duodenal diverticula are rare and pancreaticobiliary complications infrequent, however, when they are diagnosed and associated with symptoms, they warrant urgent intervention. The aim of this article is to present a clinical case of severe cholangitis secondary to the presence of a periampullary diverticulum successfully treated endoscopically.</p><p><strong>Clinical case: </strong>A 68-year-old man with a history of diabetes and hypertension, was admitted to the emergency room with symptoms of abdominal pain, fever, and tachycardia. With acute kidney injury and alterations in liver function tests, ultrasound with dilated common bile duct and gallstones. Magnetic resonance cholangiography is performed, showing duodenal diverticulum and choledocholithiasis. Antibiotic management is given, and endoscopic retrograde cholangiopancreatography is decided, finding a duodenal diverticulum with stones and pus inside, sphincterotomy, transpapillary dilation and multiple sweeps are performed. {"title":"[Autoimmune/inflammatory syndrome induced by adjuvants. Case report].","authors":"Sergio Ignacio Moreno-Urbina, Carla Paola Gameros-Palma, Jorge Gaspar Ramos-Estrada, Alberto Hiracheta-Torres","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Background: </strong>The indiscriminate application of substances for aesthetic purposes, such as silicone in breast implants, leads to the production of common local signs such as inflammation, skin irregularities, edema, erythema, vascular neoformations, and ulcers, which can evolve into general symptoms such as fever, asthenia, weakness, arthralgia or activate the immune system abnormally, causing the appearance of autoimmune diseases. This set of signs and symptoms is called adjuvant-induced autoimmune/inflammatory syndrome.</p><p><strong>Clinical case: </strong>We present the case of a 50-year-old woman with a history of silicone-based breast implants who spontaneously developed a hemorrhagic coagulopathy, type A acquired hemophilia was documented, that is, autoantibodies against coagulation factor VIII. Early diagnosis and monitoring of patients by cervical cytology and colposcopy are currently the preferred screening methods for identification and prevention of this disease.</p><p><strong>Objective: </strong>To describe the epidemiological panorama of cervical dysplasia diagnosed in a first-level care hospital.</p><p><strong>Methods: </strong>The study was observational, retrospective, unicentric, homodemic, transversal. Records from 6,207 women who attended the General Subzone Hospital with Familiar Medicine #8 (HGSZ/UMF 8), in Tlaxcala, Mexico were analyzed. First-time cervical cytologies were analyzed from 2019 to 2021.</p><p><strong>Results: </strong>Cervical dysplasia was found in 2.6% of the patients being the most frequent type of dysplasia NIC 1. Most of the clinical characteristics of patients with dysplasia were in agreement with those of the Mexican population. Important differences were found (comorbilities, mass index, number of sexual partners, births, positivity to changes related to HPV and vaccination) between two population sets defined by age (younger and older than 40 years).</p><p><strong>Conclusions: </strong>The only factor where a tendency to be associated to type 2 and 3 dysplasia in the population younger than 40 years was the sexually active onset of life younger than 18 years, so this possible association should be evaluated in a bigger population. Protective stoma in resection and anastomosis is controversial.</p><p><strong>Objective: </strong>To compare quality of life (QoL), functional capacity (FC) and complications in rectal cancer (RC) patients with low and ultralow anterior resection (LAR and ULAR) with loop transverse colostomy (LTC) versus protective ileostomy (IP).</p><p><strong>Material and methods: </strong>Comparative, observational study in patients with RC and LTC (Group 1) or IP (Group 2) from 2018-2021. FC pre and postoperative, complications, hospital readmission (HR) and assessment by other specialty (AS) were assessed; QoL was assessed with EQ-5D by telephone. Student-t test, Chi-squared test, Mann-Whitney-U test were used.</p><p><strong>Results: </strong>Group 1: 12 patients: mean preoperative FC: ECOG 0.83, Karnofsky 91.66%; postoperative: ECOG 1, Karnofsky 89.17%. Mean postoperative QoL: index value 0.76 and health status 82.5%; HR: 25%; AS: 42%. Group 2: 10 patients: mean preoperative FC: ECOG 0.80, Karnofsky 90%; postoperative: ECOG 1.5, Karnofsky 84%. Mean postoperative QoL: index value 0.68 and health status 74%; HR: 50%; AS: 80%. Both hormones are produced in different brain areas, are transported through the hypophyseal portal system, pass to the anterior hypophysis, and released to reach their target organs. These hormones also act as neuromodulators, where its receptors are found in the lateral septum, the middle amygdala, the hippocampus, the hypothalamus, and the brain stem. These brain structures regulate socio-sexual behaviors in vertebrates. Moreover, the oxytocinergic and the vasopressin systems are sexually different. The sexual steroids promote oxytocin release and the oxytocin receptor synthesis, as well as promoting or inhibiting vasopressin release and its receptor genetic transcription. Both neuropeptides are involved in social recognition, male-female pair bonding, aggression, and cognition. Statins are safe drugs that are part of the routine treatment in patients with Acute Myocardial Infarction (AMI), however, rhabdomyolysis associated with severe myonecrosis due to statins can occur and associated complications such as acute kidney injury increase mortality. The main objective of this article is to report the case of a critically ill patient with AMI who presented severe statin-associated rhabdomyolysis documented with muscle biopsy.</p><p><strong>Description of the case: </strong>A 54-year-old man who presented with AMI, cardiogenic shock, and cardiorespiratory arrest requiring cardiopulmonary resuscitation, fibrinolysis, and successful salvage coronary angiography. Whole blood viscosity (WBV) participates in the pathogenesis of thrombosis.</p><p><strong>Objective: </strong>To identify the most frequent etiologies and their association with WBV index (WBVI) in hospitalized patients with VTED.</p><p><strong>Material and methods: </strong>Observational, cross-sectional, retrospective, analytical study, Group 1: cases (patients diagnosed with VTED) and Group 2: controls without thrombosis. Risk factors for VTED were described and WBVI was calculated from total proteins and hematocrit. Descriptive and inferential statistics were used with Chi-squared test, Fisher's exact test, Mann Whitney U test, bivariate and multivariate logistic regression analysis.</p><p><strong>Results: </strong>We included 146 patients and 148 controls, age 46.3 ±17.7 vs. 58 ± 18.2 years, of both sexes (female, 65.1%). The most frequent etiology was neoplastic (23.3%), followed by diseases with cardiovascular risk (17.8%). Independent risk factors for VTED were age, chronic kidney disease, presence of liver disease or solid neoplasia. WBVI was similar in patients with VTED as in those without thrombosis. We found an association of the presence of deep vein thrombosis and diseases with cardiovascular risk (p = 0.040).</p><p><strong>Conclusions: </strong>The presence of chronic kidney disease, liver disease, and solid neoplasia are independent risk factors for VTED.
