The diarrhea was lienteric, occurring day and night, with mucus but no blood. She had no significant weight loss, abdominal pain, or other systemic symptoms, and no notable medical history. Initial stool tests showed leukocytes and blood but no parasites. Routine lab tests were normal. Due to the chronic symptoms and nocturnal diarrhea, a colonoscopy was performed. It revealed numerous mobile, white worms in the right colon, identified as Trichuris trichiura, causing friable mucosa. She had undergone the placement of a non-endoscopic, swallowed intragastric balloon the day before. Physical examination revealed significant tenderness in the upper abdomen without signs of peritoneal irritation. Examination showed tenderness without guarding. Blood tests showed leukocytosis (12200/microl) and elevated C-reactive protein (190 mg/L). Abdominal CT showed ascending colon thickening (~10 cm), mesenteric fat densification, and small locoregional ganglia, without signs of arterial/venous obstruction. Given the diagnostic doubt, colonoscopy was performed and revealed the cecum mucosa and ileocecal valve with a black color, with superficial ulceration of the proximal limit and sharp transition to normal mucosa. Biopsies of the cecum confirmed severe ischemic colitis with necrosis. After indigo carmine mixed saline staining, the lesion showed a type II-open pit pattern, which grew into the appendiceal residual lumen. The single-channel transcolonic endoscopic appendectomy (TEA) was performed. The histopathologic analysis confirmed a sessile serrated lesion (SSL), and serrated morphology was observed in multiple crypts, with mild nuclear atypia. Water exchange (WE) colonoscopy, one of the WAC techniques, has demonstrated higher intubation rate, higher adenoma detection rate (ADR), and reduced patient discomfort compared to gas insufflation colonoscopy. In our clinical practice, we have observed increased mucus production in the rectosigmoid segments during withdrawal when WAC techniques are used with water infusion compared to saline infusion. This white, opaque mucus often requires further mucosal lavage and removal, which can be time-consuming and could decrease polyp detection rates. Since low ADR is correlated with the risk of interval colorectal cancer (CRC), further attempts to improve ADR are needed. Two recent studies have explored this phenomenon. In 2020 Rahyel et al. reported higher mucus production with room-temperature water compared to saline infusion during colonoscopy insertion. In 2023 Cheng et al. randomized patients to colonoscopy with CO2 insufflation or WE colonoscopy with warm water, 25% saline or 50% saline.
