In the United States, more than 50 million endoscopic procedures are performed annually and in both cases these figures are increasing every year. This retrospective observational study assessed the efficacy and safety of neostigmine in treating Ogilvie syndrome at our center from January 2021 to December 2023. Thirteen patients received neostigmine after failing initial measures such as electrolyte correction, decompression, and intravenous prokinetics. Patients had a mean age of 66 years, predominantly male (77%), with 54% on polypharmacy. Neostigmine was administered in critical care settings under continuous monitoring. Symptom resolution occurred in 70% of cases, with a single dose achieving an 86% success rate in 54% of patients. Two doses were required for the remaining 46%, with a 50% success rate. No severe adverse reactions were observed. Six months before admission, he experienced the onset of swelling in the right inguinal region, accompanied by pain in the right lower abdomen during ambulation. He was admitted to the hospital due to the progressive enlargement of the swelling and the onset of severe abdominal pain. Physical examination revealed a protruding mass in the right groin, which was apparent upon standing and did not recede upon compression of the internal ring orifice. During the unilateral inguinal hernia repair procedure, multiple nodular masses were identified at the internal ring's opening, with the largest measuring approximately 2 cm in diameter and exhibiting a hard texture. Subsequent tissue excision for pathological analysis revealed heterogeneous cellular structures and swelling that extended along the spermatic cord, reaching the vas deferens and spermatic vessels, and invading the peritoneum. The hernial sac was smooth and devoid of hernial contents, leading to the resection of the affected peritoneum. During the surgical procedure, the spermatic cord was transected and ligated proximal to the testis, followed by its removal. Subsequently, the posterior wall of the inguinal canal was reinforced with sutures, the surgical field was irrigated with no evidence of active bleeding, and the incision was closed. This study aims to characterize a population with symptomatic Meckel's diverticulum and to investigate the utility of cellular indices in its differential diagnosis with appendicitis. During follow-up, an asymptomatic elevation of CA 15-3 was detected. A CT scan revealed rectal wall thickening suggestive of neoplasia, with no other lesions observed. Colonoscopy demonstrated a congested, thickened, and infiltrative mucosa in the rectal ampulla extending from the internal anal margin, accompanied by an impassable stricture 8 cm from the anal margin. Initial endoscopic biopsies were negative for malignancy. Endoscopic ultrasound (EUS) revealed rectal wall thickening, loss of normal wall stratification, and a regular outer border, findings consistent with rectal linitis plastica. The stricture prevented further advancement of the echoendoscope, but fine-needle biopsies were performed. Proctological examination revealed grade II internal hemorrhoids, but flexible sigmoidoscopy uncovered a 6 mm-pedunculated polyp in the sigmoid colon, which was found to result from inflammatory reaction to Schistosoma mansoni egg deposition. The patient had no signs of portal hypertension and was successfully treated with praziquantel. Risk factors for increasing the occurrence of stenosis includes long type lesions, lesions infiltrating into the muscularis mucosa and muscular layer injury. In this case, postoperative stricture of the patient was highly likely occurred. Smectite is a mucosal protective agent that can resist external stimuli, promote wound healing, and reduce inflammatory factors. Therefore, it can effectively prevent the formation of scars on wounds. This study was carried out in humans for the first time and was successful.
