Krajnc","doi":"10.18690/actabiomed.159","DOIUrl":"https://doi.org/10.18690/actabiomed.159","url":null,"abstract":"Purpose: The aim of this analysis was to determine the proportion of pregnant women referred by gynaecologists to the outpatient diabetes clinic of the University Medical Centre Maribor with a diagnosis of gestational diabetes mellitus (GDM) based on inappropriately measured fasting plasma glucose (FPG). Additionally, we wanted to compare pregnancy outcomes in subgroups diagnosed later as having GDM vs. no–GDM with oral glucose tolerance testing (OGTT) in the 24th to 28th week of pregnancy, but declared as “probably not having GDM” in early pregnancy. \u0000Methods: We reviewed retrospectively data on 81 pregnancies referred to our centre, from August 2013 to July 2014, with a diagnosis of early GDM that we did not con-firm with OGTT retesting. After retesting, women were followed up routinely by a gynaecologist, but those with a diagnostic OGTT, performed in the 24th to 28th week, were referred back to a diabetologist. \u0000Results: The age of the entire cohort was 30.5±4.7 years, and 91% were ≥25 years old; 40% were overweight before pregnancy. The period of fasting before FPG measurement was less than 8 hours in 36%. When comparing the GDM vs. no–GDM subgroups, a higher proportion of women with GDM were fasted for less than 8 hours before FPG measurement (75% vs. 32%; p=0.022), and birth weight was higher for women with GDM (3778±588 g vs. 3316±618 g; p=0.048). There were no differences in macrosomia, eclampsia, Caesarean delivery and birth trauma. \u0000Conclusion: The accuracy of the diagnosis of early pregnancy GDM can be improved by following the FPG measurement protocol. When a single FPG in early pregnancy is near normal we suggest retesting with an OGTT. The preferred site for central venous cannulation is the right internal jugular vein. \u0000Case Report: We present the case of a male patient who underwent renal transplantation for end-stage renal disease. During preparation for surgery, the right common carotid artery was cannulated instead of the right internal jugular vein. The situation was immediately recognized and managed by a vascular surgeon. The patient recovered completely without neurological deficit. \u0000Conclusion: Central venous cannulation is usually performed using the landmark technique. The main goal was to devise a novel candidate method to measure the optic nerve sheath diameter that is easy to perform, straightforward to interpret, and highly reproducible. \u0000Methods: The approval of the National Medical Ethics Committee of the Republic of Slovenia was obtained; the study was registered under No. 30/03/12. Nine healthy young adults were examined with high–resolution MRI and contrast–enhanced ultrasound (CEUS). Measurements of the optic nerve sheath diameter were performed 3 mm behind the bulb, using both methods. The datasets were anonymized and the readers, blinded. Statistical analysis included evaluation of agreement using Bland–Altman plots. The assessment of inter– rater reliability was achieved by calculation of the intra class correlation coefficient. \u0000Results: Bland–Altman plots showed favorable overall agreement between both methods, with clinically acceptable limits of agreement. The intra class correlation coefficient calculated for both methods suggests that the CEUS method was at least as reproducible as high–resolution MRI. \u0000Conclusion: The described method, using CEUS to measure the optic nerve sheath diameter, facilitates the identification of the surrounding anatomy. When CEUS is employed, the measurement points can be easily and reliably set, and retro bulbar artifacts are less confounding. Based on results from a relatively small sample, this method seems to be comparable with high–resolution MRI. Therefore, detection of a decrease in segmental myocardial contractility at the initial stage of the disease presents a diagnostic problem. \u0000Methods: The clinical characteristics of the patient, study results, and analysis of the left ventricular myocardial strain using 2D speckle-tracking mode in an echocardiographic examination are presented. \u0000Results: A decrease in strain indicators in the anterioseptal, anterior, and anterolateral segments of the left ventricular myocardium and a compensatory increase in the contralateral segments were revealed. Crnjac","doi":"10.18690/actabiomed.196","DOIUrl":"https://doi.org/10.18690/actabiomed.196","url":null,"abstract":"Purpose: We present a case report of successful surgical treatment of a 58-year- old severely immunocompromised male patient with invasive fungal infection. \u0000Case presentation: A 58-year-old male with a smoking history of 30 years was admitted to hospital because of sudden shortness of breath. Radiographic imaging showed an extensive pneumothorax. The patient had a history of T-cell lymphoma, B-cell MALT lymphoma, and chronic obstructive pulmonary disease. He had also previously undergone chemotherapy. During hospitalization, his health state started to deteriorate. On the basis of imaging and microbiological culture results, he was diagnosed to have a fungal infection with Aspergillus fumigatus. After careful preoperative care, preparations, and several consultations, we performed video-assisted thoracic surgery. Because  of poor visibility, inaccessibility, and rupture of an intercostal artery, we performed thoracotomy with excision of the right lung apex. Histopathology and microbiology results confirmed the initial diagnosis. Postoperatively, a 1-year treatment regimen with voriconazole was introduced. The follow-up CT scan showed no signs of fungal infection progression. Arko","doi":"10.18690/actabiomed.183","DOIUrl":"https://doi.org/10.18690/actabiomed.183","url":null,"abstract":"Purpose: To analyze the distribution and pattern of neck metastasis in breast cancer (BC) patients. \u0000Methods: The clinicopathological characteristics of 41 patients with BC and confirmed neck metastasis (NM) treated at our institution between 2010 and 2016 were retrospectively analyzed by reviewing medical records. The diagnosis of NM was confirmed by fine needle aspiration cytology, and node localization was classified into six levels according to Robbins' classification. Statistical analysis was performed using descriptive statistics, Pearson’s correlation and multidimensional correspondence analysis. All statistical analyses were performed using SPSS. \u0000Results: At the time of neck ultrasonography, 34 (85%) patients had developed distant metastases, 38% of the patients were asymptomatic, and 17% had no palpable nodes. The average time from BC diagnosis to the development of NM was 5.8 years (range, 0–26 years). Lymph node metastasis occurred at level V in 53.7% of patients and level IV in 19.5% of patients. Two-level involvement occurred in 9.8% and one-level involvement in 75% of patients. There were significant correlations (p < 0.01) between patient age andtime from diagnosis to development of NM, age and stage of the disease, and stage and appearance of conglomerates. \u0000Conclusion: The most common site of NM from BC was next to the supraclavicular fossa, although they occurred at other levels. Tumor specimens were divided into two groups: GS 6 (52 cases) and GS 7 (48 cases). Intratumoral microvasculature was determined by immunohistochemistry using an antibody against endoglin. Endoglin stained microvessels were observed in and around the tumor, but weak or no staining of blood vessels was seen in non-neoplastic tissue. Areas of maximal angiogenesis within the tumor were identified and microvessels were counted at ×400 magnification (0.19 mm2 field). \u0000Results: The GS 6 specimens did not significantly differ in MVD per field (24.5 vs. 29.0; P=0.46) or MVD per mm2 (109.3 vs. 129.6; P=0.78) when compared with the GS 7 specimens. No significant difference in mean serum levels of PSA between the two groups was observed (8.5 vs. 10.1 ng/mL; P=0.66). MVD per mm2 was not correlated with PSA (r=0.1; P=0.62). Patient assessments reflect both the quality of chronic illness care and provide feedback to healthcare workers about their work. The study aim was to investigate the patient-centered assessment of coronary heart disease (CHD) patients and its correlation with the care that was delivered. \u0000Methods: This cross-sectional study evaluated data obtained from the patient medical records and surveyed patients using the Patient Assessment of Chronic Illness Care (PACIC) questionnaire. A descriptive analysis of the overall and domain-specific responses to the PACIC questionnaire was conducted and the association of the results with delivery of care data in the patient records was determined by Pearson's correlation coefficient. \u0000Results: The study sample included 768 of 1080 CHD patients (71,1%) at 36 family medicine practices who completed the PACIC questionnaire. The mean age of the respondents was 68.3 ± 10.7 years and the overall PACIC score was 3.3 ± 0.9. The highest PACIC scores were delivery system design (3.7), patient activation (3.7), and problem solving (3.6). Follow-up received the lowest score (2.8). Quality of delivered care and PACIC scores were correlated (r = 0.10, p = 0.009). \u0000Conclusions: CHD patients highly rated all aspects of chronic care included in the PACIC questionnaire. They were least satisfied with the follow-up and coordination aspects of chronic care. A healthy lifestyle, which is crucial to maintain and protect health, is determined by many variables, both positive and negative. \u0000Methods: A total of first, third and fifth year medical students at the Faculty of Medicine of the University of Maribor (165 students, 57 males and 108 females) were enrolled in the study during the academic year 2017/18. Lifestyle data, including physical activity, nutrition, sleep hygiene, and drug, tobacco and alcohol use, were collected using a standardized self-assessment questionnaire, the FANTASTIC Lifestyle Assessment Instrument, which had been translated into Slovenian. The questionnaire was supplemented with further questions to explore psychological aspects of lifestyle, such as mental well-being, social inclusion, altruism and student activities. \u0000Results: Our sample of first, third and fifth year medical students revealed unhealthy changes in life habits, which were closely associated with the length of training. For the third year students, the situation had reached a crisis point, probably because the initial enthusiasm over choice of study had worn off, and stress had become less managable in the transition period between preclinical and clinical training. During their third year, the students consumed the largest amount of controlled substances and prescription and OTC drugs (p=0.003), lacked goals and purpose in life, were less connected with the faculty (p=0.000) and showed a decline in altruism (p=0.007). \u0000Conclusions: Our results show that the psychophysical stress burden among medical students increases with duration of training, reaching crisis point during the third year. Two of the most commonly used treatments are intrauterine insemination (IUI) and in vitro fertilization (IVF). The aim of our research was to analyze and compare the success rates of IUI and IVF procedures in the treatment of unexplained infertility. \u0000Methods: This was a retrospective study including 914 cycles of IUI performed for 355 couples, and 302 IVF cycles performed for 161 couples. All cases involved unexplained infertility and were treated in our department between 2012 and 2014. We analyzed differences in pregnancy, live birth rate, multiple pregnancy rate, and predictive factors for IUI. \u0000Results:There was a significant difference between IUI and IVF in terms of cumulative pregnancy and live birth rates after 4 cycles (22.0% vs. 71.8%; p < 0.001 for clinical pregnancy rates and 16.1% vs. 64.8%for live birth rate). Differences between the two techniques in terms of twin live birth rates were not significant (16.5% for IUI vs. 17.6% for IVF; p=0.936). Using logistic regression, we found that the duration of infertility, total sperm count, and the type of stimulation, were significant predictive factors for IUI (p = 0.014, p = 0.036 and p = 0.046, respectively). \u0000Conclusion: As expected, according to pregnancy rate, IVF is a better option for the treatment of idiopathic infertility than IUI. However, IUI should not be underestimated since it offers reasonable success rates with significantly less invasiveness.
