The procedure was performed through the external auditory meatus, the tympanomeatal flap was raised, and the triangular/rectangular window bounded by the internal carotid artery, jugular vein, and round window of the cochlea was drilled out to the petrous apex. \u0000Conclusion: Infracochlear approach to petrous apex is a safe operating procedure that preserves the normal external auditory canal and middle ear conductive mechanism. {"title":"Assessment of left ventricular impairment by calculating left ventricular impairment index using doppler echocardiography in chronic heart failure patients","authors":"I. Krajnc, A. Sinkovič","doi":"10.18690/actabiomed.185","DOIUrl":"https://doi.org/10.18690/actabiomed.185","url":null,"abstract":"Purpose: In order to assess global left ventricular impairment in chronic heart failure (HF) patients, it is necessary to consider both functional and structural changes of the left ventricle (LV), in particular, stroke volume (SV) and left ventricular end-diastolic diameter (LVEDD). Based on SV as a marker of LV function, LVEDD as a simple indicator of structural LV changes, and height of an individual patient (H), the left ventricular impairment index (iLVI) can be calculated. The purpose of this study was to assess the diagnostic value of iLVI in chronic HF patients. \u0000Methods: This prospective study included a total of 50 chronic HF patients and 50 healthy individuals, whounderwent standard two-dimensional echocardiography. The iLVI was calculated according to the formula: iLVI = SV x H / LVEDD2 (mL/cm). Results:The iLVI accurately separated healthy subjects from patients (AUC = 0.989), with the highest sensitivity and specificity for proper identification of HF (96.0% each),in comparison with ejection fraction (EF), stroke volume index (SVI), and SV/H. Takač","doi":"10.18690/actabiomed.174","DOIUrl":"https://doi.org/10.18690/actabiomed.174","url":null,"abstract":"Purpose: Vaginal cancer is a rare malignancy that is approximately 3% of all malignant neoplasms of the female genital tract and accounting for approximately 20 new diagnoses each year in Slovenia. The aim of this study was to evaluate characteristics of patients with vaginal cancer and to analyse the epidemiologyand describe clinical features, types of treatments applied and factors affecting occurrence of the disease. \u0000Methods: A retrospective study of all vaginal cancers in patients residing in northeastern Slovenia during the years 1995 and 2017 was performed. Patient demographics, disease characteristics, clinical features, referring cytology and histological findings and type of therapy were recorded. Associations among various demographic factors, tumour characteristics, treatments and risk factors of vaginal cancer were evaluated. \u0000Results: A total of 33 patients aged 38–87 years were identified over the 23-year period. The mean age of patients was 64 years. Altogether, we found five different types of neoplasia and 97% of affected patients were between 50 and 80 years old. The most common histologic type of vaginal malignancy was squamous cell carcinoma (78%). \u0000Conclusion: Our results suggested that in eastern Slovenian population, squamous cell carcinoma is the most common vaginal malignancy; it affected primarily older women. This research is first of its kind in the field of vaginal malignancies in the reported region. Their presence in retroperitoneum is extremely rare, as the incidence of retroperitoneal cysts, even of diverse morphology and etiology, is approximately 1 in 100,000 adult admissions. Moreover, they can be classified into lymphatic, mesothelial, enteric, and urogenital cysts. Urogenital cysts are further categorized into pronephric, mesonephric, metanephric, and Müllerian types. However, in both sexes the most common location of the Müllerian cyst is in the pelvis; in men they reside mostly near the prostatic utricle and the appendix of the testis. The aim of this study was to determine how many patients that undergo this procedure survive the hospital stay and are alive 1 year after the procedure. \u0000Methods: We retrospectively included 144 patients who had tracheotomies during their ICU stay at the UMC (University Medical Centre) Maribor in 2015 and compared the survival between patients hospitalized in the MICU (Medical Intensive Care Unit) and SICU (Surgical Intensive care unit). \u0000Results: This study showed that survival after tracheotomy in critically ill patients is low. Only 9.7% of all patients live without a tracheostomy after 1 year. The differences that were statistically significant between the MICU and SICU were observed only after the discharge from the ICU, with more people survivingthe MICU stay. \u0000Conclusions: There is little information involving 1-year survival rates of patients undergoing tracheotomies. {"title":"Impact of Combined Continuous Treatment on the Symptoms and Quality of Life of Children with Attention Deficit Hyperactivity Disorder: A Prospective Observational Study","authors":"Saška Venek, H. The purpose of the current study was todetermine the effect of combined (pharmacologic and non-pharmacologic) continuous treatment on the symptoms and QOL in children with ADHD after initial stabilization of symptoms. \u0000Methods: Seventeen children with ADHD (mean age = 8.5 years; 15 boys and two girls) were included in this prospective observational study. The children were assesed at baseline and 9 months later based on the ADHD Rating Scale (RS), Clinical Global Impression-Severity (CGI-S), and Child Health and Illness Profile/Parent Report Form-76 (CHIP-CE). \u0000Results: After 9 months of treatment, the ADHD RS showed improvement in symptoms in two items closely related to attention (p <0.05); the global value remained unchanged. Furthermore, the CGI-S showed improvement in the severity of ADHD (p <0.05). {"title":"Overcoming cultural cleavages: results from a health promotion intervention among Roma","authors":"Erika Zelko, I. Švab","doi":"10.18690/actabiomed.130","DOIUrl":"https://doi.org/10.18690/actabiomed.130","url":null,"abstract":"Purpose: The “SASTIPE” (health in Romani language) pilot project grew from the increasing awareness that the Roma generally have not participated in community-based healthy lifestyle promotion programs. The increasing burden of chronic disease and awareness of the need for a healthier lifestyle amongst the Roma have contributed to the development of an intervention program that was implemented in Roma settlements. \u0000Method: The program took place in nine Roma settlements from December 2012 to March 2013. We provided workshops focused on diabetes, chronically pulmonary diseases (COPD), healthy foods and blood pressure, and emergency preparedness and used specific materials that were developed together with the Roma. Quantitative data was collected using the adapted and validated versions of the EUROPREV and EuroQol questionnaires before and after the intervention. The assessment of the pilot project was made with data from the responders in the intervention group and the control group (response rates 53% and 74%, respectively), where the control group were populations that did not preform any other activities. \u0000Results: The program increased knowledge about some observed medical parameters in the intervention group (p<0.001), and we observed changes in the awareness of healthy lifestyles, especially smoking habits, which we noticed a significant difference between the control and intervention group in the pilot project (p<0.001). \u0000Conclusion: We demonstrated that together with the target group it is possible to develop a successful intervention project. We achieved changes in medical knowledge and increased awareness of the need to change smoking habits and lifestyle within the intervention group. {"title":"Surgical management of traumatic spondylolisthesis of the axis","authors":"I. Many conservative and anterior techniques have been used to treat these injuries. \u0000Case report: We describe a 29– year–old woman who presented to the emergency department with severe neck pain after falling down a flight of stairs. Cervical plain radiographs and computer tomography (CT) scan demonstrated a major C2 body anterior dislocation with bilateral C2 pedicle fractures. A Crutchfield extension was applied, with four kilograms of traction initially, with the neck in slight extension. After two hours of traction, plain radiographs showed near–anatomical reduction. The same day the patient underwent an anterior C2/C3 discectomy and autologous bone fusion with a cervical spine locking plate (CSLP). There were no neurological deficits postoperatively and the patient was asymptomatic. I guess there is no physician without such an experience in their daily practice. According to several surveys, health is our greatest value; however, we only begin to appreciate it, when it is no longer there. At that point, a disease appears as a negation of our health and treatments required. Medicine, scientifically dealing with treatment, comes into play. Medicine is, of course, more than just science. It comprises everything that stimulates health, cures and prevents diseases. Principally, medicine is about humanity, philanthropy and helping people not to be taken ill (prophylaxis),to recover (curative) and to suffer as little as possible (palliative). At least this is how it is supposed to be, and I believe it truly is. We describe a 63-year-old female in whom a mass was found upon ultrasonographic examination of the abdomen after persistent pain in the right upper quadrant. CT and MRI revealed a malignant intraluminal growth in the inferior vena cava. Radical resec- tion was undertaken to remove the tumor. The inferior vena cava was reconstructed using a PTFE graft. Resection margins were free of tu- mor. Postoperative complications included extended thrombosis below the surgical site and smaller pulmo- nary emboli. A vena-cava filter was inserted to prevent further complica- tions. Twelve months after the first procedure local subcutaneous metas- tases were observed. Metastases were subsequently removed.
