Rogers","doi":"10.1016/j.jsams.2024.10.009","DOIUrl":"10.1016/j.jsams.2024.10.009","url":null,"abstract":"<div><h3>Objectives</h3><div>To identify descriptors associated with success in apprentice jockeys and to determine optimum numbers of jockeys for safer race riding.</div></div><div><h3>Design</h3><div>Retrospective cohort study.</div></div><div><h3>Methods</h3><div>Incidence-rates for jockey falls and success (wins per 1,000 race-starts), time and number of races spent at different apprentice levels were calculated for 807 apprentice and professional jockeys over 19 years of Thoroughbred flat racing in New Zealand (n = 524,551 race-starts). Survival analysis was used to compare career progression for jockeys that fell and those that did not, and individual seasonal fall incidence-rates were modelled.</div></div><div><h3>Results</h3><div>Apprentices had the highest fall incidence-rate in their first year of race riding (2.4, interquartile range 1.7–3.2 vs 1.1, interquartile range 1.0–1.2, p &lt; 0.05) and a lower success incidence-rate compared to non-apprentice jockeys (71, interquartile range 67–75 vs 97 interquartile range 96–98, p &lt; 0.05). Jockeys who fell during their apprenticeship rode in more race rides to progress towards professional status than those who did not. There was an inverse power relationship between fall incidence-rate and race rides per season for jockeys, with the inflection point at 33 rides per season. Half (48 %) of the jockeys rode fewer than 33 rides per season.</div></div><div><h3>Conclusions</h3><div>There is a surplus number of jockeys, riding at high fall risk, produced than is required by the number of race riding opportunities. Additionally, to identify factors associated with participation in PE by analyzing longitudinal data.</div></div><div><h3>Design</h3><div>An explorative analysis of the NACOX study, a prospective multicenter cohort study.</div></div><div><h3>Methods</h3><div>Patients with an acute ACL injury aged 15–40 years were followed for three years. The patients regularly reported participation in PE, knee function, new injuries, and surgeries. State sequence analysis and Hierarchical Cluster Analysis were used to analyze participation in PE. Clusters were compared using an analysis of variances, the Chi-square test, and Fischer's exact test.</div></div><div><h3>Results</h3><div>The study included 275 patients, 52 % female, with a mean age of 25 years. Four clusters were identified, varying from low to high participation in PE after an ACL injury. Clusters differed regarding the number of ACL reconstructions (ACLR) (p = 0.03); time from injury to ACLR (p = 0.03); patient-reported knee function at the follow-ups at 3 months (p = 0.02), 6 months (p = 0.02), and 12 months (p = 0.02) after injury; PE level before injury (p &lt; 0.001); and number of new injuries and surgeries (other than ACLR) (p = 0.001).</div></div><div><h3>Conclusions</h3><div>Four clusters, varying from low to high participation in PE after an ACL injury were identified. Tol","doi":"10.1016/j.jsams.2024.11.007","DOIUrl":"10.1016/j.jsams.2024.11.007","url":null,"abstract":"<div><h3>Purpose</h3><div>To compare patient-reported outcomes between acute ligamentous ankle injuries 1) without anterior syndesmosis involvement and 2) with anterior syndesmosis involvement (without clinical instability).</div></div><div><h3>Study design</h3><div>Prospective cohort study.</div></div><div><h3>Methods</h3><div>Between September 2016 and December 2020 all athletes (≥<!--> <!-->18) with an acute ankle injury presenting within 7 days post-injury were screened for eligibility. Athletes were excluded if imaging demonstrated a frank fracture or 3T magnetic resonance imaging could not be acquired within 10 days post-injury. Athletes with unstable syndesmosis injuries or athletes who underwent surgery within 1-year post-injury were excluded. Athletes underwent criteria-based rehabilitation and functional outcomes (Karlsson &amp; Peterson score, Foot and Ankle Outcome Score) were assessed at 6 weeks, 6 months, and 1 year.</div></div><div><h3>Results</h3><div>A total of 94 athletes were included. In 28 athletes (30 %) the anterior syndesmosis was injured. In the first six weeks post-injury, median Karlsson &amp; Peterson score improved from 37 (interquartile range 20.5–49) to 80 (interquartile range 70–90) for the group without involvement versus 35 (interquartile range 25–62) to 82 (interquartile range 72–87) for the group with involvement. FAOS sports improved similarly in both groups. It included athletes who underwent preparticipation cardiovascular screening between February 2002 and September 2023.</div></div><div><h3>Results</h3><div>A total of 6125 players (median age: 18) were included (2496 white, 2004 mixed-race, and 1625 black). Ninety-seven percent of the ECGs were normal. Sinus bradycardia was observed in 50.1 %, while left ventricular hypertrophy in 35.1 %, with a higher prevalence in black players (41.7 %) compared to white (31.7 %) or mixed-race (34.1 %) players. Early repolarization was most prevalent among black athletes (48.2 %) compared to white (34.3 %) and mixed-race (40.5 %) athletes. The mean PR interval was 153.0 ms, higher in black players (156.8 ms) than in white (150.6 ms) or mixed-race (152.8 ms) players. Black athletes had more first-degree AV block (4.1 %) compared to mixed-race athletes (2.7 %). Conversely, white athletes (15.4 %) had more incomplete right bundle branch block than mixed-race (11.4 %) or black (9.8 %) athletes. The ‘African/Afro-Caribbean pattern’ was found in 1.8 % of black players.</div></div><div><h3>Conclusions</h3><div>Ninety-seven percent of male Brazilian soccer players had normal ECG patterns. Training-related ECG findings were more common in black players compared to white or mixed-race players. The present study investigated athlete profiles and performance predications based on the anaerobic speed reserve framework in 400m athletes.</p><p><strong>Design: </strong>Descriptive laboratory study.</p><p><strong>Methods: </strong>Maximal oxygen uptake, lactate threshold (vL4), maximal sprinting speed, maximal aerobic speed, and speed reserve ratio (speed reserve ratio; maximal sprinting speed:maximal aerobic speed) of national level and elite German 400m-sprinters (n = 13 females, age [yrs]: 20.8 ± 3.1, personal best (PB400) [s]: 55.1 ± 3.0 & n = 5 males, age: 22.8 ± 3.1, PB400: 46.7 ± 1.0) were assessed. A prediction model for 400m performance was computed via stepwise multiple regression. K-means clustering was calculated based on the speed reserve ratio.</p><p><strong>Results: </strong>Maximal sprinting speed, maximal aerobic speed and vL4 showed moderate to large negative bivariate correlations with 400m performance (-0.61 < r < -0.94; p ≤ 0.008). Backward stepwise regression revealed maximal sprinting speed and maximal aerobic speed as strong predictors for 400m performance (adjusted R<sup>2</sup> = 0.90, standard error of the estimate = 1.447 s [2.6 %]). K-means clustering revealed two distinct subgroups along the speed reserve ratio-continuum (sprint-type: speed reserve ratio ≥1.81; endurance-type: speed reserve ratio ≤1.77).</p><p><strong>Conclusions: </strong>Maximal sprinting speed and maximal aerobic speed are powerful predictors for 400m performance, with vL4 also being associated with 400m performance. Speed reserve ratio calculation enables a differentiation between 400m sprint-type and endurance-type athletes. The interplay of maximal sprinting speed and maximal aerobic speed enables a broader understanding of the contributing factors to 400m performance. The questionnaire was, however, not developed for adolescent athletes. This can limit the usefulness of the questionnaire in practice and compromise data quality in research studies.</p><p><strong>Design: </strong>Adaptation of a self-report measure and qualitative data analysis.</p><p><strong>Methods: </strong>Through cognitive interviews of South African athletes (seven boys and three girls, aged 10-18 years), we aimed to understand how adolescents comprehend the concepts included in the questionnaire and provide an adapted version of the OSTRC-H2 for adolescent athletes.</p><p><strong>Results: </strong>Our findings indicated that although adolescents can provide relevant examples of injuries, illnesses and health problems, there is a variation in how these are understood. More severe health problems, and those with persisting symptoms at the time of responding, are more likely to be captured. Smaller, resolved and mental health problems are likely to be underreported. Not all concepts addressed in the questionnaire were fully comprehended by participants; these required text modifications and addition of specific examples.</p><p><strong>Conclusions: </strong>Whilst we embed new definitions and additional examples in our adapted questionnaire, we highlight the importance of explaining the purpose of the questionnaire, and the problems it aims to capture, when distributed to young athletes. This study aimed to quantify the propulsive force deterioration rate and its relationships with deceleration rate, maximum velocity (v<sub>max</sub>), and finish velocity (v<sub>finish</sub>) in 100 m, 200 m, and 400 m elite-level sprinters. Specifically, it aimed to quantify how differences in event-specific demands influence these relationships.</p><p><strong>Design: </strong>Cross-sectional.</p><p><strong>Methods: </strong>Publicly available elite-level men and women split times from 180 sprinting performances were used to estimate propulsive force deterioration rate, rate of deceleration, v<sub>max</sub> and v<sub>finish</sub> and other explanatory variables. These data were analysed using analysis of variance and multivariate regression to determine event-specific differences, associations and interaction effects.</p><p><strong>Results: </strong>Increasing event distance was associated with smaller propulsive force deterioration rate, lower v<sub>max</sub> and lower v<sub>finish</sub> (p < 0.05). The propulsive force deterioration rate was significantly and positively associated with deceleration rate and negatively associated with vfinish in all events (p < 0.05). The propulsive force deterioration rate was also significantly and positively associated with vmax in the 200 m and 400 m events (p < 0.05) but not in the 100 m.</p><p><strong>Conclusions: </strong>Elite-level 200 m and 400 m sprinters exhibited reduced propulsive force deterioration rates which were associated with lower v<sub>max</sub> and greater v<sub>finish</sub>. MacDonald","doi":"10.1016/j.jsams.2025.03.010","DOIUrl":"10.1016/j.jsams.2025.03.010","url":null,"abstract":"<div><h3>Objectives</h3><div>The study aimed to provide evidence for validity and reliability of the Physical Literacy in Children Questionnaire (PL-C Quest).</div></div><div><h3>Design</h3><div>Cross-sectional.</div></div><div><h3>Methods</h3><div>Data were obtained from 664 primary school children (52.3 % girls; aged 10.0 ± 0.9 years; 80.6 % white British). Children completed the PL-C Quest: 30 pictorial dichotomous items relating to four Physical Literacy (PL) domains (physical, psychological, social, and cognitive). A convenience sub-sample (n = 66, 54.4 % girls; aged 10.1 ± 0.9 years) completed the PL-C Quest again within 10 days. Construct validity (Confirmatory Factor Analysis) and internal consistency (Polychoric Ordinal Alphas) were assessed on the full sample and test-retest reliability (Intraclass Correlation) was assessed on the sub-sample.</div></div><div><h3>Results</h3><div>Goodness of fit values for construct validity showed good model fit. All items (PL elements) significantly loaded on domains and all domains significantly loaded on to PL (higher order factor). Nearly all items (28 out of 30) loaded on close or above the standard of λ = 0.3. Removing these two items did not significantly improve model fit. The model was sex- and age-invariant showing evidence of validity for both sexes and school years 4, 5, and 6. Reflexive thematic analysis, with an emphasis on an Aboriginal and Torres Strait Islander standpoint, was used to analyse verbatim transcripts or notes in place of audio recordings.</p><p><strong>Results: </strong>Three themes were generated from the data: 1) Creating a space of healing, highlighting the importance of the culturally safe environment within the programme and associated social facilitators; 2) happier, healthier and learning life skills, detailing the new physical, social, emotional, physical and cultural skills and knowledge learned by participants throughout their programme experience; and 3) opportunities for improved practice, exploring recommendations for enhancing the programme for the next generation of Tiddas (i.e., programme participants).</p><p><strong>Conclusions: </strong>Findings support the overall feasibility and acceptability of the Tidda Talk programme. Critical factors in the programme's success were its holistic health approach, dual-component programme structure and flexible, place-based delivery.
