It is most common 21-hydroxylase deficiency. Renal disease in patients with CAH is not common but is usually associated with hypertension. Here we present the case of hypotensive terminal chronic renal diseases, which required the support of hemodialysis in patients with noncompliance CAH intermediation with a loss of salt. There is much divided opinion on the need for biopsy in patients with prostate-specific antigen (PSA) between 4 and 10 ng/ml. The positive biopsy outcome (PC) in these patients ranges from 20 to 39%. Low sensitivity and specificity of PSA in predicting positive biopsy outcome results in a large number of unnecessary biopsies and treatments. In order to better select candidates for biopsy, several risk stratification models for PC have been proposed in recent years, among them the PAMD score. Aim: The aim of this study was to examine the value of the PAMD score in the assessment of positive biopsy outcomes in our population of patients, as well as to examine individual risk factors for PC in patients with PSA values between 4 and 10 ng/ml treated in Serbia. Material and methods: The study involved 50 patients at the Clinic of Urology, University Clinical Centre of Serbia, whose PSA value were measured in the range from 4 to 10 ng/ml. In all the patients we measured PSA and %fPSA, and performed DRE, as well as magnetic resonance imaging (MRI) to evaluate prostate volume (PV) and PI-RADS score. All patients underwent TRUS-guided systemic prostate biopsy. In accordance with the data from literature, PAMD score was determined for all the patients. Results: A PAMD score > 3 showed a high specificity in the prediction of PC, as well as an association with a higher frequency of highgrade PC. A positive finding on DRE, %fPSA< 16, age above 69 years and PI-RADS > 3 showed a statistically significant association with the existence of PC. A high individual predictive value in assessing the presence of PC was confirmed for DRE, %fPSA, PV, and PI-RADS score. Pekmezović","doi":"10.5937/medist1701036p","DOIUrl":"https://doi.org/10.5937/medist1701036p","url":null,"abstract":"Chronic non-communicable diseases (CNDs) are a leading cause of death worldwide; preterm mortality in people younger than 70 years accounts for 40% of the total of 38 million deaths due to CNDs. Heterogeneity and complexity of CNDs cause shifting the prevention focus towards the molecular level in order to contribute to the global decrease of disease burden. Given that fact, the aims of molecular prevention are determination of crucial genetic, epigenetic and environmental factors that influence different responses to agents, as well as those that modify responses to agent exposure and tendency of development of chronic diseases. Also, it is important to mention the recognition of new pathways of pharmacologic modulation. Although basic postulates of molecular preventions are still at their beginning, the fact that significant results in the field of clarifying CNDs etiology and early pathogenesis, risk assessment and modeling, as well as targeting of agents with high preventable efficacy have already been achieved, it is clear that there is a possibility to decrease CNDs burden in the earliest phases of its natural course. Accordingly, it is important to change the belief that a person without clinical symptoms and signs of disease is necessarily healthy. On the other hand, there is a need to balance the risks and health; molecular prevention has its own place in that interspace. Studies investigating the effects of potential preventive measures at molecular level have clearly determined high-risk cohorts, outcomes and other design elements, similarly to clinical studies. There is an intensive development of new research fields, like nutrigenomics which investigates the impact of diet on metabolic pathways and homeostasis, that is, their regulation in early stages of diseases associated with nutrition, as well as the level of susceptibility of person with certain genotype to those diseases. The investigations in the fields of molecular prevention may contribute to new biomarkers development or help the setting of strategies for CNDs molecular prevention and nanotherapy. A growing number of researches in the past few decades have brought phenomena of adult neurogenesis into light. Ideas of therapeutic possibilities of neural stem cells in managing brain stroke, traumatic brain and spinal cord injury, as well as growing number of neurodegenerative diseases, represent the basis of huge research projects. After the development of CNS is finished, neurogenesis continues in two regions of the adult brain: sub ventricular zone of lateral ventricles and sub granular zone of dentate gyri of hippocampus. The process of neurogenesis brings two main questions concerning the regulatory mechanism: which factors enhance or suppress it and what is the significance of the process in humans. Brain development is under crucial influence of steroid hormones (effects are mediated through gene interaction or by neuromodulation of ion channel), so their influence on behaviour cannot be neglected. Studies have shown that hormones modulate learning and memory, but the specific roles of each of them should be monitored under a wide context of time, pre-exposition test manipulation, training as well as type of testing. Stress is another important factor in the regulation of adult neurogenesis, but current results highlight the importance of the opposite direction as well and young neurons interaction activity with HPA axis. Neurosteroids (allopregnanolone, dihydroepiandrosterone) are synthesized in the brain, and their concentrations are found higher than in blood of mammals. A number of steroidogenic enzymes (rate limiting enzymes in synthesis from cholesterol) are targeted in the brain, spinal cord and peripheral nervous system. The significance of neurosteroids' existence in brain tissue is explored through experiments of epileptogenesis. Numerous researches are trying to determine whether and how hormone alterations in neuroplasticity and neurogenesis are related to changes in cognition. Scientific investigations and their results have outstanding significance in determination of standards of safety and quality of drinking water. The respondents were divided into 3 groups. The first, the A group (63 respopndents), consists of women with (not) completed elementary school. The second, group B (60 resppondents), consists of women who have secondary education, while the third, group C (33 respondents) make women with college or university degrees. The aim is to analyze the impact of the level of education on early diagnosis of breast carcinoma in women. The observed parameters are the age of respondents and the primary risk factors. Parameters for the comparison of the results were: the attitude of the respondents to breast tumors, history of the breast self-examination, breast ultrasound examinations, data on mammography, as well as information on the diameter of the tumor at the moment of the discovery. There was no statistical difference in attitude of respondents about breast tumors when it comes to fear and / or phobia of breast carcinoma. It is almost identical to the number of respondents who have no opinion on breast tumors. Concerning the attitude of respondents that there is no risk of breast carcinoma, if nobody in the family previously had breast carcinoma, then in terms of the attitude that they do not want \"their breasts to be explored\", as well as the attitude that \"any breast examination does not help much\", a statistical difference between the studied groups was found. It was found that breast self-examination is performed by most of the respondents, but that patients in group C do it more often. It has been shown that ultrasound and mammography are more often performed by patients in group C and this characteristic makes a significant statistical difference. In Group C, the tumor was detected in diameter to 2 cm which proved statistically different. No viable evidence was found that medical therapy can slow the growth of small AAAs. The beneficial role of propranolol, angiotensin-converting enzyme inhibitors and doxycycline in reducing the growth rate of AAA was ruled out by randomized controlled studies, whereas the efficiency of statins, macrolides and anti-platelet therapy remains controversial. On the other hand, smoking cessation is the only established lifestyle change that was effective in impeding the AAA expansion. Radak","doi":"10.5937/medist1401032r","DOIUrl":"https://doi.org/10.5937/medist1401032r","url":null,"abstract":"Disseminated intravascular coagulation (DIC) represents a pathological activation of coagulation (blood clotting) mechanisms that happens in response to a variety of diseases. Diseases of the blood vessels, such as aorta aneurysmal disease, could also cause local activation of coagulation. We report a case of DIC in a patient after the surgery of abdominal aorta aneurysm. 64-year-old woman was admitted to the Clinic for vascular surgery for the diagnosis and surgical treatment of abdominal aorta aneurysm. Multidetector computed tomography (MDCT) angiography verified abdominal aorta aneurysm 5.2 cm in diameter with significant aortoiliac stenosis and thereby aneurysm resection with aortobifemoral reconstruction was indicated. The operation went uneventfully but after completed reconstruction and declamping thrombosis of both limbs of Y graft was verified. Immediate bilateral thrombectomy was performed with the blood being sent for detailed analysis that revealed the presence of DIC. Regular monitoring of hemostatic parameters (coagulation factors, platelet function and rotational thromboelastometry (ROTEM)) enabled timely substitution and administration of anticoagulant therapy during and after surgery. On the 9th postoperative day the patient was discharged in good general condition. Forty days after surgery, the patient feels good with lower limbs vascularization well preserved. Material: Cross-sectional case study included 1 eye from 55 consecutive subjects containing: 27 healthy subjects and 28 patients with early Primary open angle glaucoma (POAG). Only 1 eye per subject was randomly selected, if both eyes met the inclusion criteria. Methods: All the patients underwent OCT and visual field examination in addition to full ophthalmic examinations. Receiver operating characteristic curves (ROC) were studied for all parameters. The sensitivity and specificity of the differences between normal and early glaucomatous eyes, the areas under the receiver operating characteristic curves (AROC) and positive, negative likelihood ratios were evaluated for all the single parameters and selected combined parameters. Results: Individuals with early POAG had significantly higher values of FLV at GCC, MD and sLV at Pulsar perim-etry compared to healthy individuals. There were 1.84% of cases with early glaucoma FLV, whereas there were 0.5% of cases with healthy eyes. Considering Pulsar perimetry, patients with early glaucoma had higher values of MD (0,23dB) and sLV (1,97dB), compared to MD (0,23dB) and sLV (1,62dB) in healthy individuals (p<0.05). The highest values of AROC had the following parametres: FLV AROC (0,648), MD AROC (0,691), sLV AROC (0,696). According to AROC we combined the best three single parameters, FLV at GCC, MD and sLV at Pulsar perimetry, using a logistical diagnostic model. The largest AROC obtained was at the value of 0.790. Conclusion: In conclusion, parameters obtained by OCT (FLV at GCC), and Pulsar perimetry (MD and sLV) were able to show the significant differences between early POAG patients and normal subjects. Končar","doi":"10.5937/medi55-41690","DOIUrl":"https://doi.org/10.5937/medi55-41690","url":null,"abstract":"Introduction Screening of males over 65 years of age for abdominal aortic aneurysm (AAA) has been proved effective in some countries. The incidence of AAA in Serbian population has not been evaluated yet. The aim of the study was to assess the ratio between elective and urgently operated patients with AAA and to estimate hospital incidence of treated AAA based on the population of 100 000 inhabitants. Methods Data were obtained from Vascular surgery registry - Serbvasc which has been created by 17 institutions. Data containing demographic characteristics were obtained from the publication of the Statistical office of the Republic of Serbia. For statistical analysis, the methods of descriptive statistics were used and the values were expressed per 100 000 inhabitants. Results During the year 2021, 422 operations were performed in 7 hospitals due to asymptomatic, symptomatic and ruptured AAA in 323 (76,54%), 37(8,76%) and 62 (14,69%) patients, respectively. Hospital incidence of electively operated AAA is 8,06 cases per 100 000 inhabitants, whereas the incidence of operated ruptured AAA is 1,55 cases per 100 000 inhabitants. Overall hospital incidence of all operatively treated AAA is 10,53 cases per 100 000 inhabitants. Out of the total number of operated patients with AAA, 134 (31,75%) were younger than 65 years, while 22 (35%) out of 62 patients treated for rAAA, where younger than 65 years. Conclusion The incidence of operatively treated elective AAA is lower than the values reported in literature, while the rate of ruptured AAA is more than twice as high as the rate reported by recent papers. One third of treated patients were younger than 65 years.
